Abstract Carrying out a health promotion and disease prevention program in any community is a task that can only be achieved with the involvement of multiple parties. The issues that influence the decision to seek funding for grants that can fund such a program in a community need to be addressed within the grant proposal. For them to addressed sufficiently, the issues must be defined, the implementation plans and strategies for the program identified and discussed in details, and partnerships formed. All these steps during the grant application process are identified and discussed in this paper. Also, our organization will have to come up with a proper description of its history, mission, the programs it will be involved in, the budget, and what makes us unique and the best ones to carry out this program. This description of the problem background in basic terms is important because it helps those who will be addressed by the grant proposal understand how we will use the grant funds to address the health issues in our community and the effect of these grant funds on the community. For example, there will be the need to know the number of participants who can complete the program, those who will go through risk assessment, those who can participate in health education and the development of skill activities, and those who are enrolled in other health promotion and disease programs. Additionally, the number and types of educational materials that will be needed for this program will have to be known in the planning stages. Other program process measures include establishing the stakeholders who will be involved in the program; the people who are and will have to be aware of the message of the program for them to put in place measures for taking health promotion and disease prevention measures; the number of policies that will be developed for the whole program, the various activities and settings for the implementation of the program; the number and characteristics of the individuals who will make up the staff that will take charge of the implementation and offering of the program; and the different resources and contributions that will come from the various stakeholder groups (Kumar & Preetha, 2012). “Healthy People provides science-based, 10-year national objectives for improving the health of all Americans.” ( United States Department of Health and Human Services (HHS), Healthy People 2020 , 2011). It acts as a national guide for disease prevention and health promotion. There are various objectives and goals which serve as the guideline to achieve overall improvement in health . ‘Educational and Community-Based Programs’ is one of them. According to U.S Department of Health and Human Services the main goal of these programs is to “Increase the quality, availability, and effectiveness of educational and community-based programs designed to prevent disease and injury, improve health, and enhance quality of life.” (HHS, Healthy People 2020, 2011). This particular goal of Healthy People 2020 focuses on increasing the number educational and community based programs to increase awareness about disease prevention and health among every individual of the society. It focuses on educating them on various topics including chronic diseases, substance abuse, prevention of injury and violence to encourage and enhance health. It emphasizes on increasing the quantity and quality of such programs so that a larger population could benefit from the program and gain effective knowledge to achieve a healthy life.
Together with his colleagues at the University of Washington, Jeffrey R. Harris developed and published an article regarding the most appropriate means for disseminating evidence-based practices. The framework was geared towards spreading evidence-based practices that focus on health promotion. The authors argued that the basis for widespread adoption of evidence-based health promotion practices is mainly dependent on developing and evaluating effective dissemination approaches. Consequently, they created a practical framework based on existing literature on dissemination and their experiences in conducting such practices. The framework is commonly known as the Health Promotion Research Center (HPRC) framework, which was developed by the Prevention Research Center at the University of Washington and funded by the Center for Disease Control. CDC funded the development of this evidence-based health promotion framework to carry out research on community-based prevention and management of chronic diseases.
For example the state of Florida has a new initiative titled “Healthiest Weight”. This initiative works to tackle obesity which is what they call, “the number one public health threat to Florida’s future ("HealthiestWeight")”. Another example of state health initiative is Oklahoma’s “Shape Your Future” program. This program tackles three different aspects; “eating better, moving more, and living tobacco free ("Shape Your Future")”. Through different community activities, contest, and media advertisements these programs are hoping to help improve the health of citizens throughout their respective states. The main objective that these programs are missing is preventing health issues before they become issues. This is why it’s crucial to begin initiatives at the most basic level and educate children through better physical education
To facilitate planning networking will be important .Key stakeholders, community partners , and leaders would include health professionals, local hospitals ,health departments both state and local , primary care associates , federally funded health programs , local businesses that are health advocates , major university hospitals with research programs, and people from the local communities ( MAPP, 2015) . An important step in the change process is to know how the change will affect the community and use this information to advocate changes for improvement. Networking with these resources will be needed to assist and set priorities to address these deficits in NC. It will also be necessary to tap into local resources such as the Society for Public Health Education ( SOPHE ) who dispenses information to public and health officials about preventing cardiovascular disease.

From my understanding, there is a strong connection to the mindset and self-awareness, as one chooses to mediate for their own reason. Based on the informed information, meditation is used to decrease stress and maintain a healthy lifestyle.
