Abstract Carrying out a health promotion and disease prevention program in any community is a task that can only be achieved with the involvement of multiple parties. The issues that influence the decision to seek funding for grants that can fund such a program in a community need to be addressed within the grant proposal. For them to addressed sufficiently, the issues must be defined, the implementation plans and strategies for the program identified and discussed in details, and partnerships formed. All these steps during the grant application process are identified and discussed in this paper. Also, our organization will have to come up with a proper description of its history, mission, the programs it will be involved in, the budget, and what makes us unique and the best ones to carry out this program. This description of the problem background in basic terms is important because it helps those who will be addressed by the grant proposal understand how we will use the grant funds to address the health issues in our community and the effect of these grant funds on the community. This is also where we will state that the funds for the program will provide a solution to the issue, and that the issue does not only exist because we do not have money to implement the measures under the …show more content…

For example, there will be the need to know the number of participants who can complete the program, those who will go through risk assessment, those who can participate in health education and the development of skill activities, and those who are enrolled in other health promotion and disease programs. Additionally, the number and types of educational materials that will be needed for this program will have to be known in the planning stages. Other program process measures include establishing the stakeholders who will be involved in the program; the people who are and will have to be aware of the message of the program for them to put in place measures for taking health promotion and disease prevention measures; the number of policies that will be developed for the whole program, the various activities and settings for the implementation of the program; the number and characteristics of the individuals who will make up the staff that will take charge of the implementation and offering of the program; and the different resources and contributions that will come from the various stakeholder groups (Kumar & Preetha, 2012). The problem and needs statement is an important section of this grant proposal, therefore, all information will be based on objective research and not mere subjective
