Together with his colleagues at the University of Washington, Jeffrey R. Harris developed and published an article regarding the most appropriate means for disseminating evidence-based practices. The framework was geared towards spreading evidence-based practices that focus on health promotion. The authors argued that the basis for widespread adoption of evidence-based health promotion practices is mainly dependent on developing and evaluating effective dissemination approaches. Consequently, they created a practical framework based on existing literature on dissemination and their experiences in conducting such practices. The framework is commonly known as the Health Promotion Research Center (HPRC) framework, which was developed by the Prevention Research Center at the University of Washington and funded by the Center for Disease Control. CDC funded the development of this evidence-based health promotion framework to carry out research on community-based prevention and management of chronic diseases.

Conclusion:
The HPRC framework for evidence-based, health promotion practices provides an effective model for increasing the reach of these practices to patient populations. This framework provides important insights for the dissemination of evidence-based practices in order to enhance the impact of such practices. Notably, the authors have provided clear examples of the use of the model and its outcomes with regards to achieving specific goals. The main factor that contributes to the effectiveness of this framework is close partnerships between the main actors i.e. researchers, disseminating organizations or disseminators, and user organizations. Without close partnerships and collaborated initiatives by these parties, the effectiveness of the dissemination of these practices is significantly affected.

The PHN utilizes the primary, secondary, and tertiary prevention to help improve the health of the communities. The PHN process is applied to all levels of practice. Interventions are “actions take on behalf of individuals, families, systems, and communities to improve or protect health status” (Stanhope & Lancaster, 2012, p. 191). This paper will identify PHN interventions such as screening, outreach, and referral and follow-up, health teaching, and counseling that was identified in the PHN in the 21st Century project that this author completed as part of the PHN experience. This paper will also identify if the interventions were at the community, system or individual/family level.
