The evaluation plan is a part of the action cycle, which explains what has been accomplished (National Association of County & City Health Officials, 2014). Determination of what individuals, entities, organizations will be involved is much important before starting the evaluation plan. Process evaluation plan and outcome evaluation plan are the two types of planning available (Capacity for Health, 2014). The process evaluation plan shows how the program is operated and implemented while, the outcome evaluation examines the entire MAPP process (Capacity for Health, 2014).

Assessment of the program will be taken in regard. Since the sub-committee agreed and submitted this evaluation format it would be considered a starting point.
