The assessment is described as the “foundation” to formulate treatment and services. Social History is an example of the assessment process. Planning is the next step after the assessment process is completed. Planning is set in place to work collaboratively with the client in developing an initial plan. To produce the best possible outcome for services that have been rendered. In order for planning to be effective for the individual it is important that the case manager is knowledgeable of resources in the community. Resources such as formal agencies, generic resources. support groups to name a few. Once these steps have been completed an Individualized Plan is created. This plan is tailored specifically to each individual person based on their needs, and the resources for those needs. As case managers, it is important that your client feels supported during this process. Monitoring and advocacy are specified ways to ensure that your client is optimizing their potential with the best possible resources. My understanding of case management comes from an accumulation of lecture, readings, and a little bit of research. At first I thought case management meant to manage a case, which it kind of does, but it is a lot of background work that goes unnoticed from the workers part.
