The goal of the Parent and Youth Mediation Program is to reduce conflict and enhance communication between parents and teens. The initial phase of a data collection system would be the referral; a program such as this might receive referrals from the court, probation officers, child protective services, school social workers, teachers, etc. This program could be voluntary or mandatory depending on the referral source. It would be important to track the referral source as this could be beneficial to case and program level evaluations. Once the referral has been received, the family could move on to the intake phase. An intake form would need to be included to capture data; this form might include, referral source, demographic info of the family, reason for seeking services, rating system for current relationship, etc.
The next step would be the assessment phase, it is during this phase that the worker and the family could explore the presenting problem; the worker could also administer an initial/baseline Parent Adolescent Relationship Questionnaire (PARQ) at this initial assessment; the results for this questionnaire could serve as an additional guide for intervention. Termination is where the program is able to determine if objectives were accomplished and whether the program was effective. The aggregated dated paints a primarily objective picture of where the clients are and what they have accomplished. It also provides an opportunity for the client to assess their growth based on the services provided by the worker. Compiled data from the PARQ from all participants at termination is also aggregated and it objectively shows where the client is now. This data can be compared to the baseline and questionnaire and many conclusions can be made from the indicated
