A. Clinical Strengths & Needs
Strengths:
I had a great day at clinical, where I learned and perfected many skills. I really enjoyed working with my primary resident, answering call lights, and meeting potential second resident. Strengths included communicating effectively with staff (objective 2), implementing strategies to promote, maintain and restore health (objective 4), and demonstrating professional behaviors at a beginning level (objective 6). An example of objective 2, was when caring for my primary resident, the first thing I checked was his oxygen tank level and for any redness behind his ear; that could demonstrate irritation from the oxygen tube. I feel this is important because in the healthcare field we are a team and need to communicate with each other. I want my resident to get the best care, therefore, I need to be an advocate for him. An example of objective 4, was I received helpful and safe tips from the CNA about transferring my resident and then implementing the strategies on my own. Strategies included having the wheelchair closer to the bed, having the resident hold onto my forearms, and pivot him to his wheelchair. It’s still very difficult to safely move my resident, thus, I have Lucia assist me. I know with time I will improve on transferring my resident, but for now I need to keep practicing. I feel the number one priority is to make sure my resident and myself are safe during transfers. An example of objective 6, was at the end of the day, I made sure to thank the CNA and med. nurse for answering questions that I had. C. Clinical Learning Objectives
• List 2-3 learning objectives for this week
1) Do well on my third pathophysiology exam on Friday. I plan to do this by studying at least 2 hours each day, studying with a partner, and on Wednesday I am going to meet with my tutor.
2) Continue to improve my medication knowledge. This was one of my goals last week, but I still feel I need to improve of this skill. I plan to do this by reviewing my medication cards daily.

My last week, course objectives were:
1) Improve my documenting skills. I will be more detailed in my notes when assessing my resident. I will do this but taking more time to assess my resident.
2) Improve my medication knowledge. I plan to do this by reviewing at least two medications from my RCP each day. Being able to recognize medications is extremely important to be able to safely administer

The role of a Registered Nurse cannot be neglected in the provision of quality and safe care to patients and adopt procedures adequate for the condition of the patients because they work at the front line level; moreover, they have direct dealing with patients and integration of personal and professional skills is necessary. Therefore, there are certain attributes that are necessary to be present in a Registered Nurse for accurately performing various tasks. These include; Workload management, leadership qualities, interpersonal skills, control of practice, professional development, effective communication skills and organi zational loyalty (Daly & Carnwell 2003, pp. 158-167). These attributes hold significance in terms of obtaining positive outcome for not only the Registered Nurse but also the organization and the patient. Workload should be managed in such a way that the care process is not affected. Registered Nurse should have leadership qualities to help, motivate and inspire other nurses. Similarly, a Registered nurse should also enable and promote learning opportunities for other nurses. A Registered Nurse has responsibilities towards the subordinates, patients and most importantly to the organization. Effective communication skills can allow Registered nurses to establish a trusting relationship with patients identifying their problems and needs. The code of ethics and principles of practice must be followed and the practice of the nurse should be in the line of the organization’s working principles. The responsibility should be met as accountability factors must be considered significant in healthcare setting (Cornenwett, et al, 2007, pp.122-131; Bradshaw et al 2012, pp.13-14). ...
While on the unit I made sure to use my time wisely, I completed my assessment in a timely manner, advocated for pain medication, checked on fellow nursing students, and kept patient’s rooms organized and clean. Professionalism was also exhibited in my clean outfit, with my hair back, name badge on, nails cut, and appropriate footwear and clinical tools on hand.
Each individual gets to a point in life whereby they assess their internal qualities, people reflect their profession aspirations and attempt to see which careers will make them happy in their lives. It is also important for a person to consider whether they have the qualities needed for a particular profession and also consider the available resources that will help him or her reach their set goals (Chafey, Rhea, Shannon, & Spencer, (1998). Every human being has weaknesses and strengths in their personal or professional aspirations. Having strength means that someone has the physical and mental power to perform things in a successful manner. On the other hand, weaknesses are the drawbacks, the inadequate lack of strength or power to accomplish important goals in life, and every human has some potential of being a leader in some way (American Association of Critical Care Nurses (2006). Who I am is a brand new nurse that has entered a different environment of healthcare compared to my previous work of the pharmaceutical industry. I am a person who has always had an interest in helping people, doing amazing things on a daily basis and learning consistently. I think that doing the right thing and what is right for the patient while I’m in practice. I also will build on the mistakes that occur, and acknowledge that I made a mistake. From the mistake, I will learn on ways I can improve and not make the same mistake again. By keeping true to my word and being trustful to the patient and people around me, I will continue to grow in my integrity.
During March 2016, in Stage 1 of the pharmacy degree, I attended a two day work placement in a community pharmacy as part of the Work Based Learning module within the Capability unit. The purpose of Work Based Learning is to increase engagement with learning and to develop skills specific to patient pathways. This will link to Stage 4 of the MPharm course in which the main unit is Patient Centred Care. Therefore, it is imperative that I enhance my patient education skills through my own education.
Personal strengths I will contribute to nursing include strong communication, adaptability, and attentiveness to detail. Strong communication is vital between nurses, patients, and their other health care providers. Nurses disclose the information they learn about patients to other nurses and leading physicians, in order to diagnose and treat medical issues. If strong communication is not upheld this has negative side effects on someones healing. When I was in the hospital, I noticed a lack of communication between the nurses and my surgeon. The surgeon was not informed of the severity of my pain after surgery and this lead to a severe complication. I intend to utilize my skills of communication, so complications can be avoided. As a nurse being adaptable to any situation which may arise is crucial. Working in a hospital is unpredictable and nurses have a responsibility as health care leaders to know a course of action in any situation. I am adaptable because when faced with new and unexpected changes I am able to quickly react and think of solutions to any possible issues. Finally, I am also attentive to detail which is important when a nurse has to observe patients for abnormal health changes. Even a slight change can point to a larger health
I chose these goals because I believe that they are relevant to my current professional role as a registered nurse (RN) staff and to my future career as a nurse leader. My first goal will assist me in understanding the different quality improvement goals and outcomes in the institution; hence, motivating me as a nurse to advance my current practice and deliver better patient care. Certainly, I began with the goal getting of his vital sign and giving him a bed bath. Fortunately I had practice how to take a vital sign and giving a bed bath before my first clinical at the university skill lab and I had developed this confidence before my clinical tour. That day will be mastering the skill which I had just learned in manikins, and applying in real human with huge safety precaution. In between morning my instructor was giving me a brief before I getting to the patients room. From her experience she knew what kind of challenges and a patient behavior can be I face. As I proceeded to my own patient, I hit a roadblock that would give me a flat tire for the rest of the shift.
Each day I will chart in Meditrek hours that were obtained in clinical as well as chart on each and every patient that was seen throughout the clinical day. Each day I plan to touch on different objectives in order to obtain all of my objectives throughout the clinical experience. Along with meeting objectives throughout the semester, goals will be met as well.
Additionally, I demonstrate commitment, leadership skills, and passion for working with my patients and multi-disciplinary team members. I have been fine-tuning my practice and immersing myself in learning through the nurse residency program at New York University Langone Health (NYLH). In addition to completing the residency program and continuing education units at NYLH, I mentor new RN’s, participate in unit practice informational meetings, and have presented and launched research findings on my clinical unit.
Nursing is not only a career, but a way to give back to the community by enhancing and maintaining the health of other individuals. Throughout my clinical experiences last year, I witnessed kindness and selflessness among many nurses that have impacted and helped shape my future career choices and life immensely. Throughout this semester, I will assess myself on my nursing skills and will constantly try to strengthen my assessments, communication, clinical judgment and patient safety skills using the Creighton Competency Evaluation Instrument (CCEI, 2014).
