My first day of clinical was more of an orientation to floor. My preceptor orientated me to the unit by showing me where everything was at for both safety and emergency reasons. This week I took care of care two patients, but my focus was on one patient because the other patient was on comfort care. I did physical assessments on both patients and I administered IV and NG tube medications. I started an IV on one patient on the unit and I performed blood draws on other patients on the unit. I suctioned my patients. I prioritized my patients based on acuity. One of my patients was in ARDs due to Gillian-barre syndrome and the other on was on comfort care. I demonstrated professionalism by showing up on time and asking questions when we were out of the patients’ rooms. I also learned how teamwork is important in setting like the ICU. I grew in my confidence level. I was able to walk into my patient’s room and conduct an assessment while interacting with the family. That's one of the thinking I am working on as well. I also want to continue working on my critical thinking skills and time management.
I hope to take the lead next week. I want to be able to tell my preceptor which patient I will be seeing first and why. I also want to perform patient tasks throughout the shift and give rationales. I also want to be able to give report to the oncoming
System-Based Practice: I demostrated sytem-based practice by being aware of the health care institution in a bigger picture. In addition to collaboration with other faculty members of the health department and the community, the nurse also has to be able to handle diverse populations with a wide variety of health concerns. Patients come into the health department for numerous reasons including, immunizations, birth control, annual physicals, STD testing, prenatal case management and vision hearing and dental screenings. She would start by taking the patients’ weight and blood pressure, while asking the patient focused assessment questions. The assessment questions would focus on why the patient came into the facility and in some cases, she would even talk to the patient on a more personal level. No matter what the problem may have been, my preceptor always had the ability to make the patient comfortable. Most patients have been coming to this health department for years now, so they really respect and trust the nurses working here. The nurse presented as the health educator and patient advocate, while promoting holistic health services. No matter what services the patients’ were in need of, education was always part of the process. In many ways the nurse was doing the job of a nurse practitioner, by prescribing birth control, performing Papanicolaou (pap) smears, and ordering diagnostic tests including STD testing. The nurse also performed duties in her management position, including, ordering supplies for the facility, balancing her budget for purchases, and hiring a new part time nurse. Looking back on my goals for this clinical day, I would state that I did meet my goals. My overall goal was to become familiar with the clinical setting and the tasks that I would be asked to complete, and I feel that I completely met this goal. As the day progressed, I found myself receiving more exposure to the clinical setting and how to effectively perform skills. Following the concept of safety, I do feel that I met a majority of my goals for safety. During the day, I was able to explore my assigned patient, both by viewing her medical record and by interacting with her directly. This process sanctioned me to acquire the necessary information for my three primary diagnoses, medications, and the start of my concept map. While working with my assigned patient, and other patients on the floor, I remained aware of my surroundings and implemented safety precautions when needed. In the process of patient care and safety precautions, I relied on my therapeutic communication skills to drive my interactions with the CNAs, the nurses, and the patients on the floor. Of my goals set, the only one that I did not completely accomplish was working on all of my beginner skills. Two of the skills I did not get to practice were feeding and bathing/showering. Because I did not receive exposure to these skills today, I plan on making sure I practice these two skills on the next clinical day. Overall, I am pleased with the first clinical day and everything I was permitted to accomplish.
While on the unit I made sure to use my time wisely, I completed my assessment in a timely manner, advocated for pain medication, checked on fellow nursing students, and kept patient’s rooms organized and clean. Professionalism was also exhibited in my clean outfit, with my hair back, name badge on, nails cut, and appropriate footwear and clinical tools on hand.
Some things I need to work on are making sure I prioritize what should be done, and making sure I delegate any task that can be delegated, if I feel overwhelmed. As graduation comes near, I feel like I have learned so much more from being on this unit alone. I feel like I could take care of a couple of patients by myself, I improved on my medications, educating the patient, and providing adequate care for each and every patient. While on the unit, I got the chance to ask a variety of nurses different questions about their transition from new graduate to register nurse. I asked them why they choose this floor to work on , and a majority of them said because they precept on this floor and feel in love with neuro and the staff here. I also asked, how was your experiencing transferring from school to your first RN job, one nurse said that she only had the opportunity to orient for about three months before they put me on the floor because of a shortage of nurses. She did say that it was an excellent experience and she had a chance to learn so much. Throughout my final ten weeks at my placement, I have grown and overcome so many obstacles. I have accomplished a wide range of skills since the beginning and have been improving on them as I gained experience. At my placement as a student nurse, I have gained a lot of confidence, skills, knowledge and experiences that have helped me act and work in a professional way. All the experiences I have had during the ten weeks of my student years have helped me in shaping me into a professional.
I believe placing student nurses in the clinical setting is vital in becoming competent nurses. Every experience the student experiences during their placement has an educative nature therefore, it is important for the students to take some time to reflect on these experiences. A specific situation that stood out to me from my clinical experience was that; I didn’t realize I had ignored the patient’s pain until I was later asked by the nurse if the patient was in any pain.
This week’s clinical experience has been unlike any other. I went onto the unit knowing that I needed to be more independent and found myself to be both scared and intimidated. However, having the patients I did made my first mother baby clinical an exciting experience. I was able to create connections between what I saw on the unit and the theory we learned in lectures. In addition, I was able to see tricks other nurses on the unit have when providing care, and where others went wrong. Being aware of this enabled me to see the areas of mother baby nursing I understood and areas I need to further research to become a better nurse.
Certainly, I began with the goal getting of his vital sign and giving him a bed bath. Fortunately I had practice how to take a vital sign and giving a bed bath before my first clinical at the university skill lab and I had developed this confidence before my clinical tour. That day will be mastering the skill which I had just learned in manikins, and applying in real human with huge safety precaution. In between morning my instructor was giving me a brief before I getting to the patients room. From her experience she knew what kind of challenges and a patient behavior can be I face. As I proceeded to my own patient, I hit a roadblock that would give me a flat tire for the rest of the shift.
This reflective essay will discuss three skills that I have leant and developed during my placement. The three skills that I will be discussing in this essay are bed-bath, observing a corpse being prepared for mortuary and putting canulla and taking it out. These skills will be discussed in this essay using (Gibb’s, 1988) model. I have chosen to use Gibb’s model because I find this model easier to use and understand to guide me through my reflection process. Moreover, this model will be useful in breaking the new skills that I have developed into a way that I can understand. This model will also enable me to turn my experiences into knowledge that I can refer to in the future when facing same or similar situations. Gibbs model seems to be straightforward compared to the other model which is why I have also chosen it. I would say my greatest change this semester stood as my personal confidence and willingness to try new things. Prior to this semester, I stood fearful of making mistakes in front of others, assuming that is meant that I would not make a “good” nurse. Through this semester, I learned that it is these little learning mistakes that help nursing students grow, as we are not expected to be fully developed from the start. This is a learning environment, where mistakes are meant to be made, as they teach valuable lessons to prevent future errors from occurring. It stands better to ask for assistance when unsure of a task or a reasoning behind an intervention than to simply perform the task, as this does not effectively promote quality nursing care. My professional identity developed through my ability to perceive myself as a future nurse, recognize my limitations and scope of practice, and utilize my knowledge and skills to provide effective patient care. I am no longer afraid to put myself into unknown situations, as these present as the greatest learning
My overall learning experience during my preceptor shifts was amazing. The first day I walked into the Emergency Department for my shift, I was having anxiety through the roof and very nervous. I felt like I did not know anything and it was a completely new environment then I am use to. At this point I feel very comfortable in the environment and felt like I have gained the knowledge to be a competent nurse in practice. I owe a lot of the success I have had in the ED to my preceptor Sam. He was seriously great and very patient with me when I was trying to learn something. I was born in India and as I was growing up I realized that psychological problems were not discussed in public.
