In a work environment, different individuals work together for the care of a patient. The inter disciplinary team must show teamwork. Studies show that when a group of professionals work as a team, better patient quality of care and satisfaction can be achieved. (Grumbach. K., & Bodenheimer, T. 2004). Unfortunately, there are still some barriers that exists when it comes to teamwork and inter professional care and relationships. One of the challenges would be, working together with people of different personalities and title. It is imperative that an understanding of inter professional collaboration and teamwork be understood. In order, to create interdependencies between the different aspects of the healthcare team. A team should have clinical and administrative systems, proper division of labor, proper training and most important of all is communication.

Teamwork is an important part of a healthcare team. In order to achieve it, the clinical team in this situation should be able to work with each other. Teamwork also shows how well the performance of each part of the organizational team. (Welp, A., & Manser, T. 2016). If the team are disorganized, it may cause a strain on their overall well- being which can cause a poor quality of care. All members of the interdisciplinary team have a variety of functions, to include: assessment of the individual, assessment of the home conditions, provide education to patients and families as well as to develop a plan of promotion of health and prevention. Effectively coordinated and collaborative inter-professional teams are essential to the care and treatment of patients (Rowlands & Callen, 2013; Doyle, 2008; Ruhstaller, Roe, Thürlimann & Nicoll, 2006; Simpson & Patton, 2012, p. 300). Communication is a process of conferring information between individuals through use of speech, writing or various other means, and is critical to the success of a multidisciplinary team (MDT) (Higgs, McAllister & Sefton, 2012, p. 5; Rowlands & Callen, 2013; Sargeant, Loney & Murphy, 2008). An MDT must use multiple strategies to enhance communication and ensure their success (Doyle, 2008). An effective MDT generates opportunities that benefit healthcare, which is the reason for the recent dominance of inter-professional care in health practice (Simpson & Patton, 2012, p. 300; Rowlands & Callen, 2013). Many barriers prevent effective communication within inter-professional teams. Lack of communication within MDTs presents challenges to their success, leading to numerous consequences, including the failure of the MDT (London Deanery, 2012; Sargeant et al, 2008). Communication between professionals is the key factor underpinning the potential success or failure of inter-professional teams, the outcome of the functioning of MDTs will either benefit or impair care of patients.
The practice of using inter-professional teams in delivering care is not a new concept but current health policy requires professionals work within a multidisciplinary team Department of Health (2001) and entrenched in the Nursing and Midwifery Council (2008) Code. The principle focus of this essay is to discuss the importance of inter-professional collaboration in delivering effective health care and what challenges and constraints exist. I believe that patients have right to make their own decision for their health. According to text book, future nurses will provide patient-centered care that customized and respects of patient’s needs, value and their preferences (Masters, 2017, p. 85). Patient centered care is involving everyone in hospital from housekeeper to president and patient’s family; they are actively involving in patient’s care as priority. That will bring positive outcome from patient’s care. Also provide adequate comfort care for patient at end of life care.
