Officer Hickins, this is your annual Employee Performance Appraisal. I have not been your supervisor for this entire evaluation period. This appraisal reflects the observations made by myself as well as Sgt. Mellott B0058 and Sgt. Malast B1962 and includes input from your Employee Performance Notes. During this period you have been assigned to Lower Buckeye Jail on Shift 2. 1 Quantity of Work: Officer Hickins, the amount of work you complete during your shift most often meets standards. You are able to complete your share of the workload in each of the positions to which you have been assigned and you complete your routine duties in a timely manner. During your time here at LBJ you have been assigned to various duty posts to include, escort officer, tower officer, floor officer, and Dorms. For the past year you have been primarily assigned to the Intake area, you have mastered most of the duty posts, with only the Court Desk and Cash positions left to be trained on. …show more content…

Your OJ entries, Incident Reports, and other written documents are complete, accurate and legible. On 1/22/2016, while in the vehicle sally port, you discovered a potential security breech with an outside transporting agency. You discovered that the agency had left their vehicle unlocked with firearms visible in the driver compartment. You immediately contacted the supervisor for intake and took it upon yourself to watch over the transport van to make sure that no inmates could access the unsecured firearms. Great job! In your Blue Team it also have been noted more than once that no matter your assigned position in Intake that if you are “caught up” with your assigned duties you step up and ID as many inmates as possible before returning to your primary
