ABSTRACT
The terminology ‘person-centeredness’ is being used world widely in context of health sector and it reflects a quality of care in which the patient/ client is at core of nursing care. This paper deals with person- centered care which is principle one of practice development. The aim of this paper is to explore theory related to person centeredness and how it can be applied into clinical practice. It begins with description of person centeredness concept and theory, after which differentiation of person-centeredness and patient centeredness, and lastly implication of it in clinical setting by using CIP (collaboration, inclusive, and participatory) principle within health care team.
Introduction
In the context of health sector, different approaches are being adopted for promotion of quality health care delivery of nation. One of the best approaches is practice development which is popular particularly in UK and Australia (McCormack, 2003). Although practice development is applied commonly in globe, its concept was nebulous and lacks consistency and clarity. It means that nurses, who are adopting practice development, are facing problems to make their performance. It is argued by McCormack et al. (2001) that it is hard to distinguish practice development work and other initiative work for promotion of changes in health care systems. Person centred care means basing the care and support of a person around them. Looking at things from their perspective, promoting their beliefs, preference, likes and dislikes. They are involved in the development of their support plans, risk assessments and what they want to achieve. They determine what they want and how they want things doing. It promotes their individual needs and what is important to them. We listen to the individual and find out about their wishes and look at ways of carrying this out as safely as possible. D1: I have decided to look at a 6 year old going through bereavement. Bereavement means to lose an individual very close to you. When children go through bereavement they are most likely to feel sad and upset about the person’s death. Children at a young age may not understand when a family member dies. Children may not understand bereavement. For example a 6 year old’s father been in a car crash and has died from that incident. Death is unpredictable and children can’t be prepared for a death of a family member as no one knows when someone is going to die or not. Unfortunately every child can experience bereavement even when a pet dies. It is important that we are aware that effects on the child so we can support them in the aftermath.
For the case study one considered the overall working environment of the organisation, with a particular client situation to apply the case study arguments around. This client was experiencing a catastrophic reaction to an event. One applied an integrated person-centred approach which considered meeting their needs by listening to the issue, and working with the person, and their family, as well as care staff, Registered Nurses (RN’s) and the Director of Nursing (DON). In order to find a resolution and meet the client’s needs. As well as, adding to their care plan strategies to assist with future behavioural and psychological symptoms of dementia (BPSD). This particular situation fit perfectly within the two questions of; does the organisation prevent me from providing person-centred care, and do we have formal team meetings to discuss residents’ care.
What does ‘care’ mean? Care is the provision of what is necessary for your health, welfare and protection of someone or something. However when you talk about ‘care’ in a care practice the term changes and becomes more about enabling people to meet all their needs which would refer to their social, physical, emotional, cognitive and cultural needs. The individual is central to the meaning of care in this context.
Person centred care is defined as health care professionals work together for people who use the health care services. Person centred care also helps to support the patient’s knowledge and also helps the patient to develop an understanding of their health condition and also gives them the confidence to effectively manage and make educated decisions about their own health and also the health care in which they receive. (Health Foundation 2014). This suggests that each individual needs to be treated with the same amount of respect and they also need to be treat equally. In contemporary nursing practice, nurses need to integrate scientific knowledge and nursing theories prior to providing optimal health care. Nursing theories guide nurses to treat clients in a supportive and dignified manner through client centred approaches. However, it is challenge for nurses to practice client centred care in daily realities due to heavy workloads. In order to assist nurses to decrease the gap between ideal and real practice, Registered Nurses Association of Ontario (RNAO) develops Best Practice Guideline of Client-centred-care (Neligan, Grinspun, JonasSimpson, McConnell, Peter, Pilkington, et al., 2002). This guideline offers values and beliefs as foundation of client-centred care, and the core processes of client-centred care can facilitate provision of optimal nursing care. These four core processes of client-centred care include identifying concerns, making decisions, caring and service, and evaluating outcomes. According to RNAO (2006), ongoing dialogue with clients and self-reflection are essential for nurses to develop their nursing skills and knowledge on client-centred care. As a nursing student, I reflected on written transcripts of interactions between patients and me, so that I could gain insights into client-centred care for further improvement. Therefore, the purpose of this paper is to discuss importance of the core processes of client-centred care in nursing practice through identifying and critiquing blocks to conversation. Based on the guideline of RNAO (2006), respect, human dignity, clients are experts for their own lives, responsiveness and universal access will be elaborated in each core process of client-centre care as reflecting on three dialogues with patients.

Conversely, the introduction of “patient-centered care” has been pushed immensely, to ensure that patients and families feel they get the medical attention they are seeking and paying for. Unlike years past, patient centered care places the focus on the patient, as opposed to the physician.1 The Institute of Medicine (IOM) separates patient centered care into eight dimensions, including respect, emotional support, coordination of care, involvement of the family, physical comfort, continuity and transition and access to care.2
Person centred values influence all aspects of health and social care work because it is a law requirement in regards to the Human Rights Act 1988, Health and Social Care Act 2012 along with codes of practice for social care workers, health and social care should be based on person centred values. Person centred values in care is an approach that respects and values the uniqueness of the individual and seeks to maintain or if required, restore their individuality. This is done by creating an environment that promoted the personal worth, uniqueness and social confidence of the individual as well as respect, independence and hope. My most valuable work with patients is to listen and learn from each one, and let them determine their goals. The person in person-centered in care is more of an expert in what’s best for them than I am. As shocking as this fact was to me, it’s been liberating to let go of having to know everything about a patient that I just met. As a result of my experience with my therapy patients, person-centered care has now become the foundation of my practice.
Person-centred care is the treatment and care provided by any health service. This particular concept places the person at the centre of their own care and considers the needs of the individual and the surrounding family members.
