In a study produced by Dr. Bhattacharya (2009) regarding the ISM code on the management of occupational health and safety in the maritime industry four (4) key findings have been raised questioning its effectiveness.
The first issue that has been identified is the fact that both players (managers and seafarers) involved in the implementation of the ISM code have different perception on the subject. The managers of the shipping companies utilize the ISM code as a managerial tool which is going to improve and promote a safe and healthy working environment onboard their ships. However on the other side seafarers view the ISM code as a bureaucratic formality and burden with little impact in the promotion of a health and safe working environment since as the same study indicates, seafarers rely on their working experience gained and through their daily interaction with their colleagues. Moreover it can be said that the written procedures and guidelines produced by the managers were insufficient.
The second issue that has been raised is the difference in the methodology used in the implementation of the ISM code. The main practices that are being followed to demonstrate compliance with the ISM code is risk assessment, incident / near miss reporting and audit / review. The mentioned study revealed that even though managers and seafarers are able to demonstrate documented compliance with the above mentioned practices, it can be said that there is a difference between its purpose and what it achieved in practice, due to the fact that seafarers involved in risk assessment and incident/near miss reporting practices consider it unnecessary and they produce the necessary documents in order to be showed in compliance with Managers’ requiremen...

‘Health and Safety at work act 1974’ is a very important Legislation when working in healthcare as this is here to keep everybody involved as safe as possible. Section 8 of the Safety, Health and Welfare at Work Act (2005) puts the onus on the employer to ensure systems of work that are planned, organised, performed, maintained and revised as appropriate so as to be, so far as is reasonably practicable, safe and without risk to health. Section 19 of the Act requires the employer to conduct a risk assessment of work activities and Schedule 3 of the Act provides the general principles of prevention to ensure the safety of employees in the workplace.
Under the health and safety act everyone should be thinking about how to keep themselves, others and the environment they work in safe. Adults have a duty of care and should encourage the children to also do this, this will help increase their own awareness of what they are doing and where.
In a culture of safety and quality, all employees are focused on upholding quality in providing safe care. In order to promote patient safety in the hospital setting there should be an exhibition of teamwork irrespective of the different leadership positions. However the leaders show their obligation to quality and safety, and set capacities for their employees to perform a committed and critical role in assuring patient safety.
Creating a Culture of Safety. A culture of safety includes psychological safety, active leadership, transparency, and fairness. As a health care professional, I can create a culture of safety by having a positive attitude and creating an environment within the team that feeds off that optimistic and encouraging behavior. In addition, I can contribute to a culture of safety by using effective communication, the “Fairness Algorithm” to differentiate between system error and unsafe behaviors, and by being respectful and approachable to all my fellow coworkers and patients.

A candidly of risk occurs in every organisation. Governance principals and the occupational health and safety urge that the organisations take reasonable measures to hinder loss, charge or rage to the organisational and all stakeholders/management. Injury and accidents can even happen ultimately with stringent OHS and the fact that an accident when occurs, does not mean that someone is liable if all responsible steps for prevention or minimisation has been taken.
To sum up the ISM code was introduced after in particular the British ship, The Herald of Free Enterprise capsized as a result of various human errors both on the ship and also ashore. The ISM is main objectives are to increase the safety of personal at sea and also to reduce the chance of pollution at sea damaging the marine environment. The ISM code applies to practically all sea going vessels and will itself continually reviewed and changed to always be improving safety standards at sea. Unfortunately this means learning from mistakes of future accidents in order to prevent them from happening in the future and there may be further changes from a result of what happened with the Costa Concordia. As a last point it is clear that any initial costs implementing an effective SMS system will always be overcome by the profits gained by having an effective system.

Therefore, I intend to pursue a continuation of safety certifications that will demonstrate a persistent ability to learn about my career. For the initial duration of my profession, I hope to obtain an Environmental Health and Safety specialist entry level position for an established organization. If the opportunity arises, I will promote within the same organization until
