Abstract
The idea of creating a nurse residency program developed through communication with my fellow new graduate nurses with less than one year of experience in our facility and through personal observation. The goal of creating the residency program is to help new nurses to transition from school to practicing. The affected population would be made up of all nurses on the unit and the patients that interact with the new graduates. The background of the problem is the lack of confidence possessed by new nurses and high turnover rates with those nurses. A nurse residency program would benefit new graduate nurses when transitioning from a student to a practice registered nurse utilizing the help of trained, experienced preceptors. The database
used to gather evidence for this project was ProQuest. The evidence that was gathered was all usable information. The data revealed a correlation between new nurse residency programs and new nurse job satisfaction, retention rates when the preceptors are trained accordingly. The collection of evidence means that through the right nursing residency program we can retain nurses and increase new nurse residency satisfaction. The possible constraints on moving forward with the implementation of the nurse residency program is the money involved in training the preceptors to properly educate and guide the practice of the new residents. The College of Nurses of Ontario (CNO) is the governing body of all registered nurses in Ontario and is regulated. The CNO provides expectations and guidelines to follow, which need to be met by each Registered Practical Nurse (RPN) individually. As a nursing student, I am taught about the CNO and the importance of referring back to the guidelines while caring for patients. While gaining experiencing in the nursing field through my clinical settings, I have realized as a nursing student there are areas I need further development in. Transitioning from academic nursing student to Registered Nurse/New Graduate Nurse (NGN) within the healthcare environment is a challenging task for many NGNs. They may encounter a number of challenges, such as the following: transition shock, professional isolation, lack of clinical experience, stress, lack of a support network and cultural incompetence. Some dream of becoming president of the United States of America, or becoming an Astronaut, a lot of little kid’s say that they want to be a Doctor. What kids do not understand is that there are many types of doctors, like an orthopedic surgeon, they work with feet, or an oral surgeon, who work with the mouth. Another similarity to a doctor is a nurse practitioner. They do many of the same tasks, like performing minor surges, fixing broken bones, or simply giving a routine check up. What led me to nursing in the beginning was having a passion for people. I love the holistic approach and level of care that nurses bring to the literal bedside table. I knew throughout nursing school that I wanted to advance my education further than bedside nursing. Working with new graduate nurses has instilled in me the goal to share my knowledge and expertise with others and to teach others the art of nursing. My short-term goals are to complete the Master’s program by the summer of 2014. My second goal is to secure a job as a nurse educator at a hospital and work with my nurse manager to provide program like in-service to the hospital staff and students. There are several nurses that have established themselves successfully in the work I am pursuing. The work mainly involves care management, client teaching and patient advocacy. Care management focuses on ensuring that the care that various health care providers offer is cohesive to meet the patients’ needs. Client teaching deals with enabling patient gain knowledge about the medications and health. Patient advocacy is involved with protecting and advancing patients’ rights.
Being a registered nurse affords one the option of working in many diverse healthcare settings. In any practice setting the climate of health care change is evident. There are diverse entities involved in the implementation and recommendation of these practice changes. These are led by the Robert Wood Johnson Foundation (RWJF), the Institute of Medicine (IOM), nursing campaign for action initiatives, as well as individual state-based action coalitions. Nurses need to be prepared and cognizant of the transformations occurring in health care settings as well as the plans that put them at the forefront of the future.
From the literature I learned about the importance of driving the nursing profession towards the community health-care system. There is currently a need for revitalization of the primary health care system as stated in this document. There is a need to change some of the focus of education and training in new nurses in an effort to respond to the health care demands of today and the future, since many students are not going into this field. In addition, nurses already working in community health should begin focusing on current prevention and promotion aspects of health. I plan to apply these ideas in my own nursing practice to this rotation.
