Introduction
The College of Nurses of Ontario (CNO) is the governing body of all registered nurses in Ontario and is regulated. The CNO provides expectations and guidelines to follow, which need to be met by each Registered Practical Nurse (RPN) individually. As a nursing student, I am taught about the CNO and the importance of referring back to the guidelines while caring for patients. While gaining experiencing in the nursing field through my clinical settings, I have realized as a nursing student there are areas I need further development in. In this paper, I will address two of my learning needs and my goal for each. I will also discuss the plan I created in order to successfully meet my learning needs prior to becoming an RPN, and
I need further development in knowing what the scope of practice of an RPN is because at clinical I am unsure of the difference in roles of the RPN’s that are working on the units compared to the Registered Nurses (RN’s). I feel that knowing and understanding the scope of practice of an RPN is crucial, in order to ensure the safety of the patients, but also to protect your license as a nurse. Also, it is important to understand your role as a nurse to ensure the patients’ needs are being met, and to ensure you stay within your scope of practice while providing care. The CNO’s scope of practice statement is, “The practice of nursing is the promotion of health and the assessment of, the provision of, care for, and the treatment of, health conditions by supportive, preventive, therapeutic, palliative and rehabilitative means in order to attain or maintain optimal function” (College of Nurses of Ontario, 2015). I also plan on taking a course after graduating from the nursing program specific to IV therapy because I feel this is a skill that takes practice and not having the hands-on experience through the nursing program at Niagara College is unfortunate. The IV therapy course is offered at Niagara College for a six-week period. I now that I have the knowledge to aspire to take up my role within one of the identified population foci. APRNs program developed my core competencies by allowing me to be more efficient adaptability with regards to newly emerging APRN roles or population focus. Furthermore, achieving my course objectives enable me to understand the specific APRN roles. For example, course objectives provide me with a better detail, and align my licensure goals with the responsibilities expected of each role. Licensure will provide me and my fellow APRN graduates with the full authority to practice. Also, certification is required to meet the highest possible standards as APRNs are expected to align knowledge, skills and experience with the standards of health care professionals. This field has very narrow margins for error, and it is therefore important, for APRNs to meet the highest and most stringent academic qualifications. It is important that students have the ability of being competent in a clinical setting. To be deemed competent in skills according to nursing regulations and requirements. This is a challenging factor for many students, as they enter transition period. This is due to students feel they do not have the desired clinical competency that promotes the skills and authorities of a registered nurse (Harsin, Soroor & Soodabeh, 2012). Clinical research studies have found that students do have the required expected levels of knowledge, attitude and behaviour’s. As student nurses, I have many goals that would like to achieve upon completion of 150 hours of my capstone course. My plan is to use this opportunity to demonstrate competency skills, and knowledge gained through previous quarters. I have listed seven learning goals that I wanted to achieve during the capstone experience. On my first day of capstone, I have gone over each one with my preceptor and requested from her to provide me with feedbacks, so I can use them as stepping tool to work upon. These goals ranged from critical thinking decision making, culture, growth & development, legal and ethical to clinical competence safety/holistic care, evidence-based practice, and professionalism lifelong learning.
Conflict often arises between new graduate nurses and experienced staff over the ideal patient care. The realities of practice often turn out to be contrary to what graduates were taught in university (Phillips, Kenny, Esterman, & Smith, 2014). These discrepancies in practices have been shown to create distress and reality shock for new RNs (Gardiner & Sheen, 2016). Reality shock is experienced when the considerate effort is put into preparing for a specific employment and new employee finds the candidate not prepared at all (Harwood, 2011). Overall, I retain three goals for this clinical day: Safely and efficently administer medication, enhance my nursing/CNA skills, and determine how to implement infection control into a health care setting. This week reflects my assigned time to administer medication in a health care setting for the first time, with a resident who retains nearly twenty medications. I except this experience will be a great learning experience, but it will also subsist slightly stressful. With the assistance of my FOR, my goal is to administer all of my resident 's medications without complications. Working with multidisciplinary teams within our profession, it is important to acknowledge and recognise the way in which all the professionals are guided by law and their independent regulatory bodies. The needs of the individual patient is to be considered by doctors and nurses alike, who share professional values and are set out in the respective codes of practice, The Code (NMC, 2015) and GMC (2013).
Our course, Transition to Professional Nursing, is barely two weeks old and already I am being enlightened and challenged to expand my experience of nursing. I will attempt to explain my personal journey and experience thus far including how and why I got here, my beliefs about nursing and related values, and my visions for the future.
Drug administration forms a major part of the clinical nurse’s role. Medicines are prescribed by the doctor and dispensed by the pharmacist but responsibility for correct administration rests with the registered nurse (O'Shea 1999). So as a student nurse this has become my duty and something that I need to practice and become competent in carrying it out. Each registered nurse is accountable for his/her practice. This practice includes preparing, checking and administering medications, updating knowledge of medications, monitoring the effectiveness of treatment, reporting adverse drug reactions and teaching patients about the drugs that they receive (NMC 2008). Accountability also goes for students, if at any point I felt I was not competent enough to dispensing a certain drug it would be my responsibility in speaking up and let the registered nurses know, so that I could shadow them and have the opportunity to learn help me in future practice and administration.
