Healthcare Team Work In the healthcare field no one person has the answer to the complex health problems that is seen in the Primary Care setting. In today’s health care field, the delivery system involves many different healthcare professionals. On any given day there can be multiple handoffs among multiple health care providers with varying levels of education and occupational training. During a week’s stay at the hospital patients can see and interact with as many as 50 different employees, such as physicians, Nurse practitioners, nurses, patient care technicians and other health professionals. In this fast paced environment, it is critical to have team collaboration, when healthcare professionals do not communicate it puts patient safety at risk. The term interdisciplinary refers to situations in which various disciplines are involved in reaching a common goal with each contributing his or her specific expertise. Teamwork is essential to reach any set goals or objectives. Every team member should have specific characteristics and functions in order for the team to function properly. Collaboration is important to ensure quality in completion of the task. In health care an interdisciplinary team is a group of health care professionals from diverse fields who work in a coordinated fashion toward a common goal for the patient/client and the patient’s families. Effectively coordinated and collaborative inter-professional teams are essential to the care and treatment of patients (Rowlands & Callen, 2013; Doyle, 2008; Ruhstaller, Roe, Thürlimann & Nicoll, 2006; Simpson & Patton, 2012, p. 300). Communication is a process of conferring information between individuals through use of speech, writing or various other means, and is critical to the success of a multidisciplinary team (MDT) (Higgs, McAllister & Sefton, 2012, p. 5; Rowlands & Callen, 2013; Sargeant, Loney & Murphy, 2008). An MDT must use multiple strategies to enhance communication and ensure their success (Doyle, 2008). An effective MDT generates opportunities that benefit healthcare, which is the reason for the recent dominance of inter-professional care in health practice (Simpson & Patton, 2012, p. 300; Rowlands & Callen, 2013). Many barriers prevent effective communication within inter-professional teams. Lack of communication within MDTs presents challenges to their success, leading to numerous consequences, including the failure of the MDT (London Deanery, 2012; Sargeant et al, 2008). Communication between professionals is the key factor underpinning the potential success or failure of inter-professional teams, the outcome of the functioning of MDTs will either benefit or impair care of patients.
The demands on health care providers to provide the best quality care for patients is increasing. With added responsibilities and demands on our health care workers, it is hard not become overwhelmed and forget the reason and purpose of our profession. However, there is a way where all professionals can meet and come together for a common cause, which is the patient. A new approach in patient care is coming of age. The demand of a constantly developing health service has required each professional to become highly specialised within their own field. Despite the focus for all professionals being on the delivery high quality care (Darzi, 2008); no one profession is able to deliver a complete, tailored package. This illustrates the importance of using inter-professional collaboration in delivering health care. Patient centric care is further highlighted in policies, emphasising the concept that treating the illness alone whilst ignoring sociological and psychological requirements on an individual is no longer acceptable. Kenny (2002) states that at the core of healthcare is an agreement amongst all the health professionals enabling them to evolve as the patient health requirements become more challenging but there are hurdles for these coalitions to be effective: for example the variation in culture of health divisions and hierarchy of roles. Here Hall (2005) illustrates this point by stating that physicians ignore the mundane problems of patients, and if they feel undervalued they do not fully participate with a multidisciplinary team.
The problem of poor communication stems from an environment of high stress levels. After a consulting company scrutinized processes throughout the hospital related to care coordination and patient flow, the evidence was clear. Medical errors can happen in the healthcare system such as hospitals, outpatient clinics, operating rooms, doctor’s offices, pharmacies, patients’ homes and anywhere in the healthcare system where patients are being treated. These errors consist of diagnostic, treatment, medicine, surgical, equipment calibration, and lab report error. Furthermore, communication problems between doctors and patients, miscommunication among healthcare staff and complex health care systems are playing important role in medical errors. We need to look for a solution which starts changes from physicians, nurses, pharmacists, patients, hospitals, and government agencies. In this paper I will discuss how does the problem of medical errors affect our healthcare delivery system? Also how can these medical errors be prevented and reduced?
Today’s complex health care system requires that patient care be delivered by healthcare providers from multiple disciplines since it is evident that no single profession has all of the resources that are needed to address all of a patient’s holistic needs (Weller, Boyd, & Cumin, 2014). Patient safety depends on collaboration among healthcare professionals and their ability to work as a team. Thus, it is no surprise the Quality and Safety Education for Nurses (QSEN) project included teamwork and collaboration as core competencies to ensure patients receive high-quality care (QSEN, 2018). The inability of healthcare professionals to collaborate and work together results in poor patient outcomes, duplicate therapies, higher cost of care, and decreased patient satisfaction. In fact, the Joint Commission found 70% of major medical errors to be the result of ineffective collaboration among healthcare professionals. Therefore, it is imperative every leader in the healthcare system to promote the cultivation of these skills to ensure patients receive high-quality care.
In order to achieve the goal of competent healthcare, all members of the team need to work well together. Time can be a barrier to teamwork in healthcare. Patient ratios to healthcare workers can be overwhelming, resulting in time constraints by all members. Another barrier is effective communication between providers and between the patient and family. Providers can neglect to pass on pertinent information to one another resulting in incompetent care being provided.
The environment of care is one of broken unity and does not allow for a cohesive team. “Within health care organizations, unsuccessful conflict management is a prime cause of stressful work environments…patient and employee dissatisfaction, poor quality of care” (Iglesias & Vallejo, 2012). The patient may receive conflicting data from various members of the interdisciplinary team, which has the potential to lead to noncompliance with treatment and/or medication regimens. In this example, it proved that the successful information exchange within team provided opportunity to notice patient’s concern and the well understood of treatment options allowed for sharing decision making between team. Based on my health care experience, I’ve observed that effective teamwork leads to a better patient outcome. Being a team player is also good for morale.
A number of scientific studies have looked into the sources and outcomes of bad communication in healthcare facilities. Put simply, poor communication between other physicians and physicians; between nurses and physicians; and between other hospitals and hospitals, has stayed mostly unexplored (Taran S. 2010). One of the places where communication could at times be lacking is actually in physician-physician interactions.
