Volunteers and mentors can be recruited and trained to forward the aims of the program. For a community to effectively improve the health of its members, behavioral modifications must be attained and maintained. The changes may occur through implemented programs, policies and initiatives that are aimed at modifying attitudes, beliefs and social norms. Aspects of organizational infrastructure may have to change in order to affects the desired results. Community level interventions include mass media campaigns, restaurant and supermarket nutritional labeling, partnership with schools, churches social service agencies, advocacy groups, and recreational facilities. In order for laws, public policies and local ordinances that are enacted to stigmatize behaviors, the community must have a readiness and a capacity for change. The Information-Motivation-Behavior Skills (IMB) Model focuses on increasing a person’s knowledge, motivation, and skills essential to performing a behavior and is a useful approach for influencing sexual health behaviors (Rongkavilit et al., 2010). This model has been used in a variety of studies that focus on changing behaviors regarding sexual health. Several interventions have found that the use of the IMB model has increased preventive sexual health behaviors. Fisher, Fisher, Misovich, Kimble, & Malloy (1996) conducted an intervention in which one group of students received an IMB model-based intervention that focused on AIDS risk reduction while another group of students received no treatment. A 1-month follow-up showed that the intervention was successful in increasing AIDS risk reduction knowledge as well as stimulating safer sex conversations and increasing condom use during sexual intercourse. Childhood obesity has reached epidemic proportions with the one of largest subpopulations being African American girls. According to the (The American Public Health Association [APHA], 2003) nearly 22 percent of African American girls ages 6-11 years of age are overweight. Childhood obesity is a considerable predictor of obesity in adulthood and can lead to deleterious consequences if left untreated. Improving the health needs of this vulnerable populations needs to be paramount not only for the overall wellbeing of the individual, but to avoid placing additional burden on the health care system. Health promotions such as educational programs focusing on health risk or behaviors are successful in improving health behaviors (Bellows, McMenamin, & Halpin, 2010). The purpose of this paper is to provide an individual health promotion for an 8 year old African American female who is the 95 weight percentile for weight. Another thought is to designate a school health coordinator who will be responsible for maintaining and continuing the positive work that this study began. Utilizing the success of this program and others like it in this regards will lead to action on all levels of the social ecological framework. Ultimately, a multilevel examination of determinants and interventions needs to be aimed at preventing child overweight and obesity.

Together with his colleagues at the University of Washington, Jeffrey R. Harris developed and published an article regarding the most appropriate means for disseminating evidence-based practices. The framework was geared towards spreading evidence-based practices that focus on health promotion. The authors argued that the basis for widespread adoption of evidence-based health promotion practices is mainly dependent on developing and evaluating effective dissemination approaches. Consequently, they created a practical framework based on existing literature on dissemination and their experiences in conducting such practices. The framework is commonly known as the Health Promotion Research Center (HPRC) framework, which was developed by the Prevention Research Center at the University of Washington and funded by the Center for Disease Control. CDC funded the development of this evidence-based health promotion framework to carry out research on community-based prevention and management of chronic diseases.
Sex and relationship education (SRE) supports children through their moral, physical and psychological aspects of growing up to be an adult, and making sure they have knowledge on relationships, sex, human sexuality and sexual health (Sex Education Forum, 2010). There are three main elements in SRE, the first element is attitudes and values, which is about developing positive values and being able to consider moral issues before they make their decisions on having intercourse (Kirby, 2007). The second element is personal and social skills, which is about teaching children to have the confidence to value themselves and others, to become respectful of individual conscience and the skills to judge the kind of relationship they want; the third element is about accessing the knowledge and understanding of human sexuality, reproduction, sexual health, emotions and relationships (Kirby, 2007). The potential effect of SRE should be seen in these three areas, which include reduction in teenage pregnancy, and the chances of getting sexually transmitted diseases (STDs) and being sexually assaulted, etc. SRE can either be given at home to the child by parents, in school by teachers, or in youth organizations, alternative educational institutions, or youth offending organizations by Shine staff or trained peer educators and volunteers (Levy 1992).
The SMART goal of the obesity program is to reduce the obese population in Pitt County by creating a healthier diet program. Different objectives will be needed in order to reach this goal. The first objective is that from July 2016 through December 2016 100 community members will participate in the health department’s health promotion activities. This objective aligns with the reinforcement construct of the Social Cognitive Theory. By repeatedly participating in the activities, the 100 community members will be rewarded with the knowledge they receive and also motivation to change their behavior. By having an enjoyable time, they will be rewarded and want to keep coming back to the activities. The second objective is that following the 6-month duration; the majority of the participants will lose 2-10% of their original body weight by changing to a healthier diet. The reason the participants need to lose 2-10% of their body weight is to either prevent or reduce the chances of gaining other health diseases associated with obesity, including coronary artery disease and Type 2 Diabetes. This objective aligns with the self-efficacy construct. By losing a little bit of body fat, the participants will believe in their ability to lose weight, and continue to do so. The third objective is that on June 17 and June 24, informational brochures will be distributed to the community members to create awareness and reminders to join the obesity program, and gain knowledge about the health condition of obesity. This objective deals with the behavioral capability construct of the Social Cognitive Theory because the participants will gain the knowledge and skills needed to change their behavior through the brochures.

You can belong to a community through religious beliefs. Through shared experiences or interests. You can belong to a political community where all involved share the same political interests.
