CM Gilmore made an unannounced visit to GRU to ensure safety and well-being of Jaylin Davis. CM Gilmore spoke with Jennie Westbrook, and explained the reason for the visit. Mrs. Westbrook provided CM with documentation of their interaction with Ebony and Jaylin. They also was able to provide medication records regarding Ebony. CM Gilmore explained to Ebony Davis the reason for the visit and Ebony reported the following: • Ebony reported that she wanted to know how to get her SSI check back from April Paulk who is the personal caregiver of Ebony a the personal care home. • Ebony reported that she has never had a history with DFCS outside of food stamps. • Ebony reported that she is married to Robert Davis, who has the other three children that …show more content…

Ebony made the comment that it is to protect her home. • Ebony reported that residing in the personal care home is: April’s four children, client, April, and another mental health client. • Ebony reported that her monthly income is $733 a month, but April gives her $40 for her to be able to purchase the monthly items that she needs. • Ebony reported that she has her birth certificate, but she doesn’t have her social security card, because April is withholding it from her. • Ebony reported that she doesn’t have an ID currently, because April hasn’t taken her to get her ID. Ebony reported that she has been needing her ID to get her medication for schizophrenia, which is also the reason of why she was hospitalized. • Ebony reported that the owner of the personal care home, April Paulk wants to adopt Jaylin; however, Ebony reported that she doesn’t want her to adopt him. Ebony reported that she wants to be able to try to raise the baby on her
own. • Ebony reported that she has to Serenity Behavioral Health to get medical attention for her schizophrenia. • Ebony reported that she has ran out of medication and is concerned that she will not be able to get any additional medication due to her not having an ID. • Ebony reported that Robert Davis who is legally her husband resides in Vidalia Ga. • Ebony reported that MGM is deceased • Ebony reported that PGM is caring for her other two children. • Ebony reported that she is not aware of who the pediatrician will be for Jaylin Davis. CM Gilmore assessed Jaylin Davis, who was unwrapped in his blanket. CM Gilmore voiced her concerns to Ebony regarding the baby being unwrapped. CM Gilmore discussed with Ebony Davis regarding safe sleep, and co-sleeping. Ebony advised that the baby will be in the bassinet. CM provided Ebony with the parental guide Ebony provided signatures for Hipaa and Release of Information.
Hazelwood v. Kuhlmeier of 1987-1988 Background: At Hazel East High School, the school has a sponsored newspaper called “The Spectrum” that is written and edited by the students. In May of 1983, the high school principal, Robert E. Reynolds, received the edited version of the May 13th edition. Upon inspecting the paper, he found two articles that he found “inappropriate.” The two articles contained stories about divorce and teen pregnancy. Hylton denies current suicidal ideation, homicidal ideation, and symptoms of psychosis. She reports today she was asked to come in to ADS to sign an authorization of release of information in hopes of finding substance abuse treatment for her legal issues that the court suggested she engage yesterday. Mrs. Hylton yesterday after court, she proceed to consume an unknown amount of alcohol due to feeling stressed over court. Mrs. Hylton reports informing her primary care physician to tell Melanie at ADS she did not feel well and was not going to her weekly Wednesday meeting this afternoon. She states, "I didn't want to go, I was tired and stressed out, I asked my doctor to call for me." She reports being called by therapist Melanie to come in to sign some papers. Mrs. Hylton reports she was under the impression her going to ADS was to aid her in finding a inpatient treatment program for substance abuse. She reports going in to ADS around 2 pm. She denies consuming alcohol before going to the meeting, however reports having "3 cups of acohol" mixed drinks after leaving ADS. Mrs. Hylton states,"I had things to do, I had to make sure my dog, who is older and sickly, I had to make sure she was okay, and I told them I would go tomorrow because I didn't want to go there with no clothes and haven't showered." On 05/25/2017, at approximately 10:29 am Shawnprece Ivey-Grier (8364873) got off the school bus and decided to damage some of the cars in the staff parking lot. Mr. Crutchfield (Teacher) stated that he looked back to monitor the students movement and notice Shawnprece Ivey-Grier running off the walkway into the staff parking lot and jumped on the hood of one of the vehicle that was parked. The vehicle belonged to Ms. Pamela Brandon (Transition Coordinator).
