On July 21, 2017 at 0730 hours, a Social Worker notified police service that she had lost his government issued Prox Card. Investigation revealed that the social worker last saw her Prox card on July 20th, 2017 1540 hours, when she was leaving for the day and noticed it missing when she returned to work no July 21, 2017 at 0715 hrs. She attempted to locate the Prox card with negative results. The social worker will continue to look for her Prox card and report it missing to her supervisor. CM normally meets with the client every Wednesday for face to face meeting. On 8/4/2016, client walks in the social services office. CM completed Bi-Weekly ILP Review. In the meeting the client was emotionally teary, upset and angry. Client reported to this worker she doesn’t want to be here anymore. She continues to report this is not a Working Shelter and she have an appointment with DHS advocate today at 4pm and she will request to be transfer to a Working Shelter. This worker inquires what happened for the client to be teary and upset. Client reported she requested metro cards to attend a housing interview to fill out application. I called the phone number listed for Legend, and her mother, Kimberly Windham, answered. I asked Kimberly if she new anything about the incident, and she told she did not. Kimberly said she had received another call from a difference officer, possibly from the Port of Seattle, but she did not get a name or case number. Kimberly said she asked Legend why there was a CPS referral, and Legend told her "You don't know what you're talking about" and would not give her any information.

Once inside the business, Wilson stole a Phillips Heart Defibrillator, serial #A17I-00442, valued at $1,300.00. Wilson left the scene in an unknwon direction. On 11/08/17, Wilson was contacted for an unrelated reason and was found to be in possession of the stolen Phillips Heart Defibrillator, serial #A17I-00442. Wilson was arrested PC 496 – Receiving Stolen Property.
