There are various definitions of “models of care”. However, the most important feature remains how health care services are delivered. Model of care was defined more precisely as a conceptual tool which is a standard that associates concept, belief and intent linked in several ways (1). A model of care depends on a descriptive image of practice which characterizes the real objective of care. Therefore, it can be different according to the target group or health system in use. For better understanding, examples of some of the health care models (strength and weaknesses) studied during the course are listed below;
1. Guided care model; one of the most efficient model for chronic patients, it affords high-quality chronic care for these patients, as more health care providers (interdisciplinary) are available for them, ensuring patient satisfaction(2).
2. Chronic care model; takes a wide approach to ensure improvement of patient care and improved health outcomes, as it is patient centered and involves community(3).
3. Preventive care model; prevention is one of the most efficient way of reducing health cost, though if used unnecessarily, it could drive costs up. However, this is a unique model that focuses on patient-physician relations and aims at averting illnesses/mortality by screening and regular hospital visits(4).
4. Patient-centered care; this depends a lot on the relationship between the doctor and patient. It revolves around the patients’ need, what matters to the patients and can be very effective in reducing both costs and symptoms if communication is effective(5).

Mr. J.
Mr. J. is a 73 year old man, retired and lives in Leiden. He has three children and five grandchildren. He suffered a heart attack some years back and continued rehabilitation program which he still attends presently to control his condition, hence he sees a physiotherapist weekly, his cardiologist monthly and calls his General practitioner when he needs him. Presently, he feels discomfort in his knee but was told by his orthopedic doctor that nothing can be done to improve this; hence he takes some pain killers when necessary.
