Effective nursing care plays a very important role in health care as it safeguards the patients and helps them recover as soon as possible. There are many processes that contribute to effective nursing care, nursing process is one of them. Nursing process consists of 5 main phases; Assessment phase, Diagnosing phase, Planning phase, Implementing phase and Evaluation phase. All the phases work hand in hand together and one links to another with every phase having its important role. This process helps to improve the effectiveness of the care as it is directed to every patient individually.
The first and one of the most important phases is the Assessment phase where the nurse has to gather as much information as possible in order to establish a database based on the patient’s lifestyle, medical history, health problems, allergies, medical results and so on. This type of database is a continuous one, always gathering new information about the patient as to be as detailed, precise and as effective as possible. This phase can also be split up into four other categories; (i) the initial assessment: where the information is gathered on admission of the patient. (ii) Problem focused assessment: This is used to monitor how the patient is doing after being diagnosed with a specific problem in an assessment carried out before. (iii) Emergency assessment: This assessment is quite crucial since it is done when patients in critical state and has to be done quickly in order to pinpoint any life threatening complications. Such assessment could monitor breathing, circulation and airway after a cardiac arrest. (iv) Time-lapsed assessment: This is done some months after the last assessment in order to determine how the patient’s health is so...

...o focuses on the social and emotional needs of the patient as most phases are tackled hand in hand with the patient making it an interpersonal and collaborative process. Another positive note regarding these phases is that it is universally applicable where there are only minor differences such as the names of the phases. Having a holistic structure like this surely provides a more effective way of nursing care.

There are events, subtle or otherwise, leading up to a critical change in health status. As nurses at the bedside, we must have strategies and protocols implemented in order to monitor changes in vital signs and trends leading towards a cardiac, respiratory, or neurologic event. In a hospital setting, patients are monitored for changes in condition, whether it be improvement or deterioration, allowing clinicians to decide the course of action to follow in their care.
This is a practice that provides a thinking framework through the provision of nursing skills to an organization. It is a useful, flexible tool in nursing practices that allows one to think and develop more creativity. Nursing process involves five phases in which when combined
There are six set standards of the nursing practice; assessment, diagnosis, outcome identification, planning, implementation, and evaluation (ANA, 2010; pp. 9-10). Throughout a typical shift on the unit I work for, I have set tasks I am expected to complete in order to progress the patient’s care, and to keep the patient safe. I begin my shift by completing my initial assessment on my patient. During this time, I am getting to know my patient and assessing if there are any new issues that need my immediate intervention. From here, I am able to discuss appropriate goals for the day with my patient. This may come in the form of increasing mobility by walking around the unit, decreasing pain, or simply taking a bath. Next, I plan when and how these tasks will be able to be done, and coordinate care with the appropriate members of the team; such as, nursing assistants and physical therapists. Evaluating the patient after any intervention assists in discovering what works and what does not for the individual. “The nursing process in practice is not linear as often conceptualized, with a feedback loop from evaluation to assessment. Rather, it relies heavily on the bi-directional feedback loop...

...r investigation and then devise a plan for best possible action recognizing the rights of the patient and its benefits followed by the application of the chosen intervention with positive outcome in mind (Wells, 2007). Delivery of excellent and quality of care at constant level (NMC, 2008) must be marked in any responsibilities and duties of the care provider to promote exceptional nursing practice
The six standards of practice are very important. Under the first standard, assessment, the nurse evaluates health information related to the patient. This information could be a health issue such as asthma, or a psychological issue such as anxiety that is necessary knowledge needed before treatment can begin. Once this is accomplished the second standard, diagnosis, begins. Under this standard the nurse takes the information gained from the assessment and utilizes it to derive a diagnosis of the individual. The third standard, outcomes identification, has
One of the pivotal roles of a nurse is the ability to recognise patient deterioration. The skill of identifying crucial elements of deterioration and acting appropriately is fundamental for positive patient outcome. A vital skill performed primarily by nurses is the act of respiratory rate measurement. This skill is performed in addition to five other physiological parameters, which form a basis for a scoring system. The scoring systems commonly used are known as NEWS (National Early Warning Score) and EWS (Early Warning Score). As many adverse events are preceded by a period of time where by the patient exhibits physiological dysfunction, there is often time to correct abnormalities. This has significance for nurses, as they are responsible
Additionally, the clinical staff has shown very low level of confidence in the RR documentation on observation chart. Lack of time, laziness, lack of training and knowledge and unawareness of the importance of the respiratory assessment are main reasons to neglect this important aspect of nursing as stated in this study (Philip, Richardson, & Cohen,
ANA describes “The Scope of Nursing Practice (as) the “who,” “what,” “where,” “when,” “why,” and “how’ (8).’ In other words, it is the responsibility of the nurse to know who their patient is, what the patient’s diagnosis and treatment are, where it is they will be delivering treatment, the rationale behind their actions, and how they will deliver the care. By following the scope of practice, nurses reduce avoidable errors and are aware of the liability their actions entail. The ANA also puts forth a nursing process to guide nurses in treatment. The constantly evolving process is currently assessment, diagnosis, identification of outcomes, planning, implementation, and evaluation (ANA 9). Though this method has dramatically improved nursing care, it may be necessary to repeat steps to adapt to a patient’s changing needs and pathologies. By following guidelines set by the ANA, nurses are able to better connect with their patients and instill the image of professionalism to the public while also optimizing safety
According to the American Nurses Association, nursing is defined as “the protection, promotion, and optimization of health and abilities, prevention of illness and injury, facilitation of healing, alleviation of suffering through the diagnosis and treatment of human response, and advocacy in the care of individuals, families, groups, communities, and populations” (American Nurses Association, 2016). Nurses have many jobs and responsibilities and wear many different hats. Nurses can perform at many different levels depending on their scope of practice which is defined by the board of nursing in one’s state of residence. It is important as nurses to understand and follow

Next, there is person that is to be cared for. A person is someone who needs nursing care. A person is made up of many parts including their mind, spirituality, and their body. The nurse is responsible for caring for all of these parts as considering the whole person is essential to facilitate healing. The nurse may also need to care for the patient’s family and the community. The family being in a healthy emotional state may be what is necessary for the patient’s improvement. Caring for the community can help prevent disease and screen for diseases. Preventing and early detection allows for quicker healing
The nursing process is based upon five steps. The first step is the assessment phase; this can range from body system specific to head-to-toe assessment. These assessments are both subjective and objective and must be properly documented, organized and validated (Taylor et al, 2011). The second phase of the nursing process is formulating a diagnosis. The nurse identifies the patient’s needs and strengths from reviewing the previous assessments and determines what the nursing diagnosis should be. Then comes the planning phase where the nurse organizes the interventions by priority based upon the assessments and creates a plan for the patient to work on ...
Profession of Nursing is always regards as a complex, dynamic and noble. The nursing profession requires many things such as hard work, a vast knowledge, excellent communication skills, and a passion for the noble profession. All these qualities are needed to be an effective nurse. Today, nurses are living in a world of ever changing field of medicine .The present patient statistics poses, nurses are expected to take responsibility of accomplishing the requirements. The requirement for excellent nursing skills is added as well as well-formed skills in different aspect. Because of the high demand in excellent nursing care, the IOM have declared the recommendations for the future of nursing.
In the healthcare setting, a systematic process to ensure maximum care and maximum recovery in patients is needed, which is called the nursing process. This process consists of four steps: assessment, diagnosis, planning, implementation, and evaluation (Walton, 2016). The nursing process is important to ensure quality care and to get the preferred outcome. In the nursing process, critical thinking is used to recognize the issue and come up with a logical solution to solving it. One important aspect of the nursing process is that the plan is not set in stone; it is meant to be manipulated in order to better suit the patient. Nurses must be able to think critically in order to recognize the issue, develop a way to correct it, and be able to communicate the issue to others. Throughout the nursing process, critical thinking is used to determine the best plan of care for a patient based on their diagnosis.
