Skin Grafting
Skin grafting, or, the transplanting of skin and other underlying tissues types to another location has been used for almost three thousand years. It originally began with the Hindu Tilemaker Caste System where skin grafting was used to reconstruct noses that had been amputated as punishment. Over time, it evolved, and in this contemporary age we possess two major means by which to transplant skin: split-thickness skin grafting and full-thickness skin grafting.
In split-thickness skin grafts, the skin graft includes only the epidermis and a portion of the dermis. The thickness of the slice can be broken down further into a thin layer (.008-.012 mm), a medium layer (.012-.018 mm), and a thick layer (.018-.030 mm). This type of skin grafting is used primarily when cosmesis, or, the concern for the appearance of the patient, is not a key concern, or when the defect is too large for the use of a full-thickness skin graft. Split-thickness skin grafts are employed in various situations including: chronic non healing cutaneous ulcers, temporary coverage to allow for the observation of a possible tumor reoccurrence, surgical correction of depigmenting disorders, and coverage of burn areas to accelerate wound healing and reduce the loss of fluids. The procedure for harvesting and transplanting split-thickness grafts begins with the measurement of the skin defect. A purse-string placed around the defect reduces its size and thus also the size of the graft that must be harvested from the donor. Effective donor sites include thighs, buttocks, and upper arms. Then the donor site is lubricated with sodium chloride and a sterile tongue depressor is placed across the donor site to create a flat surface. The surgeon then applies the dermatome to the donor site and slices off the necessary layer of skin. The newly harvested skin is placed in a sodium chloride solution and is prepared to be meshed. Meshing is advantageous when one wants to increase the ratio of the skin graft. By placing it in the mesher, the graft becomes flatter and obtains a diamond plate appearance. Keep in mind that while it is acceptable to trim the graft to fit the recipient site, it is better to have some overlapping between the donor tissue and the recipient bed. The slight-thickness skin graft is secured by staples and a bolster may be applied to encourage graft apposition to the recipient bed, to decrease shearing forces, and to maintain a moist environment for the graft.
When microdermabrasion removes the outer layer of skin the body then goes into a healing mode that promotes the making of new healthier cells. Microdermabrasion is like when one sustains an injury to his skin and the body sends treatment molecules to heal the damage. Mariane Fernandes conducted an experiment with the aim to evaluate the effects of microdermabrasion on skin rejuvenation. Fernandes stated in her article “Effects of microdermabrasion on skin rejuvenation” in the Journal of Cosmetic and Laser Therapy that “Microdermabrasion treatment promoted skin rejuvenation through an increase in skin thickness due to an increase in epidermal thickness and collagen organization” (Fernandes 26). Acne weakens and thins the skin epidermal layers, causing acne scars. Microdermabrasion heals the weak, thin, and fragile skin, which makes the skin smooth and more resistant to other damaging conditions. Doctor Bruce M. Freedman an aesthetic plastic surgeon and assistant professor of plastic surgery at Georgetown University also analyzed the dermatological changes associated with microdermabrasion in “Clinical and histologic changes determine optimal treatment regimens for microdermabrasion” in the Journal of Dermatological Treatment. If there is not enough tissue for the reconstruction, the surgeon will insert a tissue expander under the skin and gradually fill it with saline solution, causing the skin to stretch and grow. Eventually, there will be enough room for the implant. Then reconstruction surgery can proceed. The surgeon will make a small incision and insert the implant.

Bone is one of those tissues that receives extensive use so it is necessary that it is strong in order to carry out its functions; however, it will occasionally face injury. I viewed a “TED Talk” lecture, which discussed a new way of regenerating bone with the help of our own bodies. Molly Stevens, the head of a biomaterials lab, presented “A New Way to Grow Bone” where she discussed a new technique called “in vivo bioreactor”. She also answered why this new procedure is beneficial. Researchers like Stevens are constantly trying to find innovative new techniques and they do this by asking questions. The question that Stevens presented in the video was an intriguing one: “Can we recreate the regeneration of bone on demand and transplant it?”.

After the investigation of what Ultrasonic Peeling is and how it works, I will also state my opinion on how well I think the treatment benefits its users, give my evaluation if I would consider using it, what I think the positives and negatives are and in scenario, assess whether it is a treatment that more salons should consider offering.

Liposuction is also known as lipoplasty and liposculpture and is the most popular form of cosmetic surgery performed in the United States. Liposuction has been a means of contouring the body in one or more areas for the past twenty years. This surgery is mostly performed on women, but among men and older people, the surgery has become more popular. Also, this surgery has been classified as the rich person’s surgery (Pavlovich-Danis, 2001, p. 1). Liposuction begins by the surgeon making tiny incisions throughout the areas where the liposuction is going to be performed. Then the surgeon takes the cannula, narrow tube, and vacuums out the fat layer deep beneath the skin. The cannula then breaks up the fat cells by being pulled continuously back and forth throughout the skin. The broken up pieces of fat are then suctioned up by the cannula. The fat that is taken out is replaced by fluid, so that the patient does not go into shock ("New Image," 2001, p.2). Even after this surgery, the results are not guaranteed.
Skin is particularly rich in stem cells because so much skin is lost through normal wear and tear; you shed thousands, sometimes even millions, of dead skin cells every day. In mild cuts and burns, stem cells work to repair the damaged tissue. But in severe burns the stem cells in the burn area are destroyed, so doctors have to take skin from an undamaged area. The biggest obstacle in performing a skin graft is that, currently, only the burn patient’s own skin works reliably. If doctors try to use skin from another person, the patient’s immune system will eventually reject the graft. However, scientists are working on ways to grow skin that’s genetically compatible with the patient so that, even if the patient doesn’t have enough undamaged skin to use, the burns can still be treated

Cosmetic Surgery is a great discovery that has been around for many centuries. “The history of plastic surgery origins in the early 800 B.C. when surgeons in India restored noses to persons who had them taken off as a form of punishment.” (History of Cosmetic Surgery, 2008) Many people have used over the years cosmetic and reconstructive surgery to enhance their physical appearance weather it be for health problems associated with birth defect or problems that they have developed over the years or to even alter physical appearances for instance nose jobs and tummy tucks. There are numerous types of cosmetic surgery that can be done today in the United States, over the year’s surgeons techniques have improved dramatically. There are a number of pro’s and con’s that go along with cosmetic surgery, such as the amount of recovery time and the initial cost of surgery. Statistics also show that cosmetic surgery has grown so rapidly over the years it is phenomenal people who receive cosmetic surgery goes up by the millions every year. While there can be many related health risks due to cosmetic sugary, many times cosmetic surgery is necessary and its is made more affordable than thought to be.
