Over the last few years, I shadowed a Primary Care Adult Nurse Practitioner whose office is located in the underserved urban area of Irvington, NJ. She also takes care of patients from the surrounding areas of Irvington, Newark, and East Orange, all of which have very large underserved populations including African Americans, Latinos, and patients from the Middle East. During my clinical shadowing, I gained a appreciation for the complexity of treating long term chronic conditions such as asthma, diabetes, HIV, and hypertension. In many cases these conditions were exacerbated due to poor nutrition, non-compliance, and lack of education about healthy lifestyle choices. I gained a keen understanding of the importance of patient education and the ability to connect patients with community services to help them with their economic and social challenges.
I believe that every citizen deserves good healthcare services regardless of his or her geographical area, income, or race. An underserved community is a community in which people are unable to obtain health care or have limited access to the health care system for different of reasons. It is no secret that the current healthcare reform is a contentious matter that promises to transform the way Americans view an already complex healthcare system. The newly insured population is expected to increase by an estimated 32 million while facing an expected shortage of up to 44,000 primary care physicians within the next 12 years (Doherty, 2010). Nursing should focus on patient and family centered care, with nurses being the patient advocate for the care the patient receives. Patient and family centered care implies family participation. This type of care involves patients and their families in their health care treatments and decisions. I believe that it is important to incorporate this kind of care at Orange Regional Medical Center (ORMC) because it can ensure that we are meeting the patient’s physical, emotional, and spiritual needs through their hospitalization.
In consequence, this will limit poor adults finding the proper treatment since many doctors do not accept Medicaid patients. High rates of uninsured populations were associated with lower primary care capacity (Ku et al., 2011). Thus, expanding insurance coverage can support more primary care practices in rural areas and can help equal the gap in primary care positions. Since the 1990’s, the interest in nursing and the profession as a whole has decreased dramatically and is still expected to do so over the next 10-15 years according to some researchers. With this nursing shortage, many factors are affected. Organizations have to face challenges of low staffing, higher costs for resources, recruiting and reserving of registered nurses, among liability issues as well. Some of the main issues arising from this nurse shortage are the impact of quality and continuity of care, organizational costs, the effect it has on nursing staff, and etc. However, this not only affects an organization and community, but affects the nurses the same. Nurses are becoming overwhelmed and are questioning the quality of care that each patient deserves. This shortage is not an issue that is to be taken lightly. The repercussions that are faced by both nurses and the organization are critical. Therefore, state funding should be implemented to private hospitals in order to resolve the shortage of nurses. State funds will therefore, relieve the overwhelming burdens on the staff, provide a safe and stress free environment for the patient, and allow appropriate funds needed to keep the facility and organization operational.
The health care system of today will be invaluable without the help of well-trained registered professional nurses. According to Wilkinson, Treas, Barnett, Smith (2016) registered nurses in today’s health care have the training required to develop a care plan with other health care workers that is specific for each patient in their care, instead of the same general approach for every patient. This is known as patient centered care because each patient is different from the other and the registered nurse role includes developing a good plan for every patient they care for. In performing such duties, registered nurses tend to create a strong bond with their patients by caring for their patients by all means possible. By caring for their patients, they tend to gain their patients and their families trust. According to Wilkinson et al. (2016) There are many support systems for patients in our communities that have good intentions of assisting sick people and those in need. Most of them are not known by the patient or their families. The registered nurse is the one who is in the position to inform the patient or family about such resources that may be very helpful for them if they are interested. By doing so, the patient knows that the nurse really cares about him/her and that feels very comforting and may help the patient get well
My vision for the Family Nurse Practitioner in a primary healthcare setting is to bring health care to small communities, comprised of predominately poor working class people, who are classified as underprivileged. Performing exams on the sick, providing immunizations to the young, educating all, and assisting the elderly with chronic illness and disease management, while providing preventative care and education on wellness. The Family Nurse Practitioner sees patients from young to old in need of a wide variety of care. As a Family Nurse Practitioner I will provide education and holistically meet the health care and wellness needs of my community, and others like it, by providing critical access to health care for the most vulnerable
