Over the last few years, I shadowed a Primary Care Adult Nurse Practitioner whose office is located in the underserved urban area of Irvington, NJ. She also takes care of patients from the surrounding areas of Irvington, Newark, and East Orange, all of which have very large underserved populations including African Americans, Latinos, and patients from the Middle East. During my clinical shadowing, I gained a appreciation for the complexity of treating long term chronic conditions such as asthma, diabetes, HIV, and hypertension. In many cases these conditions were exacerbated due to poor nutrition, non-compliance, and lack of education about healthy lifestyle choices. In addition to my clinical shadowing, I have been involved in community service work during my college days in the Big Brother Program in Providence, Rhode Island.
In this role I served as a mentor and engaged in reading activities in urban school systems with minority students. This experience taught me the importance of role models in these communities in order to motivate students to achieve higher goals than they might have thought
possible. As an athlete I not only developed a strong sense of teamwork, but a strong sense of focus on achieving my goals. Athletically speaking, I frequently had to accept defeat and motivate myself to try harder at the next opportunity. I fully expect that health care work with an underserved population with limited resources will often be frustrating but I will use my long term orientation to keep my focus on the goal of providing exceptional primary health care services. Over the past two years I have been developing my clinical skills as a certified nursing assistant at both a rehabilitation and hospital setting. In this capacity I have seen the difficulties that patients face due to the lack of insurance and poor quality of healthcare which often results in the worsening of chronic conditions. Finally, I volunteered at two soup kitchens that provided services to homeless and economically disadvantaged residents of the New Brunswick area. As a result of this experience I gained a greater awareness of the social medical and economically challenges faced by the underserved populations of an urban area.
I believe that people everywhere should always have access to adequate medical care. Where you live should not determine whether you live, and the PA profession was created to improve the availability of healthcare in rural and other underserved areas. As a PA, I would be eager to help people have not had access to the care they needed. I want to serve those that need medical attention but don’t have the means to obtain it—whether in rural Michigan, the inner-city of Atlanta, or the backwoods of Arkansas. My vision for the Family Nurse Practitioner in a primary healthcare setting is to bring health care to small communities, comprised of predominately poor working class people, who are classified as underprivileged. The purpose of this paper is to illuminate and discuss healthcare vulnerabilities of the elderly rural population in Baker County, Florida and describe how the nursing profession can address these problems. Rural health has been a complex and multifaceted challenge for government and healthcare practitioners. The elderly who live alone in the county suffer from low socioeconomic status, low health literacy rates, declining cognitive and physical health and lack of healthcare facilities. At this time, I became a group leader in an after-school program for a 3rd-grade class. This was my first time working with children and as imagined, it was a tough transition. The students were Latino and African American, living in poverty stricken neighborhoods with a dysfunctional home life. The program focused on students who were at risk of retention. A large number of students were below average and had been recommended to receive extra academic support. As a result of my time spent working and learning with the students, I choose an educational path.
African American senior citizens face a health care crisis too. They have worked all of their lives to secure retirement, but their retirement has been threatened because of the rising cost of long-term medical care. Insurance companies have failed to provide affordable long-term care, protection that most senior citizens need. This lack of long term care and affordability has been a serious problem for the health care system. In some cities, the shortage of hospital beds is so serious that it is common for patients to stay in emergency rooms before they can be admitted to an inpatient room (Drake 109). More than one thousand hospital beds are occupied by people who could be better care for in nursing homes or through home health care (Drake 110). Of the disabled elderly 1.3 million reside in nursing homes (Drake 10). These patients are unable to perform two or more of the basic activities of daily living without assistance.
The educator role of the APHN and NP includes health education within a nursing framework and professional nurse educator roles. The APHN analyzes groups at risk within a community and implements health education interventions. The APHN and NP boost wellness and contribute to conserving and advocating health by teaching the importance of a healthy lifestyle, stress management, physical exercise, and good nutrition. My background in healthcare came as an environmental pass down with a father as a Registered Nurse and a mother in healthcare management. It was inevitable that the journey of healthcare started being instilled without my knowledge of it, as a young child. A constant learning in the health sciences and management directed my way. By the start of college, the intrigue lead to compassion, lighting a fire for the administration of healthcare. This calling spurred by a great woman, my mother, who is a national redesign award winner by the Bureau of Primary Healthcare in Health Disparities. Improving population health necessitates a variety of the contributions from health entities. These health entities can be state, local governments, hospitals, health centers, and community organizations. Unfortunately, these contributions and services are not equally distributed throughout the population. Lack of a supermarket in a neighborhood limits residents’ access to healthy food and other resources. Furthermore, ethnic minority and/or low-income communities are burdened with several health disparities such as greater risk for diseases, or limited access to healthcare services (Jackson, 2014). Shadowing in autopsies satisfied my early high school curiosity in that I was able to understand the anatomy of the human body. Knowing the appearance of a normal organ helped me to recognize when an organ was abnormal whether that was through weight or appearance. I have learned that the human body encapsulates teachable knowledge and evidence that leads to the diagnosing of disease and corresponding treatment. My clinical rotation in the acute care hospital has definitely been a great learning experience, it was a big transition for me having been working in a long-term care facilities for six years. There were three positive aspects about my performance that I will take away from this learning experience.
