• Describe how the things that are most important to you in your RN role have prepared you for the APRN role. During my career as a registered nurse I have had the privilege of caring for my patients at the bedside and meeting their needs holistically. Additionally, the safety of my patients is one of the most important aspects of my current role. During one of my undergraduate courses, “Community and Population Health”, I completed a paper on my community and access to healthcare. During the research and community outreach performed to meet the goals of the paper, my eyes were opened to the plight of small communities in regards to access to quality healthcare. This plight has become my passion, and has formed the basis for my vision of the Family Nurse Practitioner role. My vision for the Family Nurse Practitioner in a primary healthcare setting is to bring health care to small communities, comprised of predominately poor working class people, who are classified as underprivileged. Performing exams on the sick, providing immunizations to the young, educating all, and assisting the elderly with chronic illness and disease management, while providing preventative care and education on wellness. As a medical / surgical RN, I provided care for the elderly, the infirm, the mentally challenged, the young, and the psychologically disturbed. The wide variety of patients exposed me to the effects of life style choices, health care choices, and the resulting impacts to the patient as well as to the family of the patient. This experience has fully matured my view of the awesome responsibility that we, as health care professionals, have been charged with, and it has furthered my desire to obtain the skills necessary to provide more advanced care for my patients. In addition to exposure, maturity and experience, my career as a medical / surgical RN has also sharpened my critical thinking abilities and provided insight on observing signs and symptoms that a patient may be unware of. The role of Advanced Practice Registered Nurses (APRN) in health care has been identified as a critical role in the goal to provide high-quality health care. APRNs have a bigger role to play in the health care system and it is important that students are provided with relevant and valuable knowledge as well as experience that improve their abilities. Every person’s needs must be recognized, respected, and filled if he or she must attain wholeness. The environment must attuned to that wholeness for healing to occur. Healing must be total or holistic if health must be restored or maintained. And a nurse-patient relationship is the very foundation of nursing (Conway et al 2011; Johnson, 2011). The Theory recognizes a person’s needs above all. It sets up the conducive environment to healing. It addresses and works on the restoration and maintenance of total health rather than only specific parts or aspect of the patient’s body or personality. And these are possible only through a positive healing relationship between the patient and the nurse (Conway et al, Johnson).
Patient-Centered Care is a vital step for the underserved populations. In the clinical practice attention, and focus should be priority for these disadvantage patients. As a nurse educator, I should take a proactive role to to improve the health of America's underserved populations. participating in support group will enhance the care to serve the underserved population. In United states there is a disparity in the health of racial , ethnic groups, minorities, and African Americans. Most of the underserved population lives in underserved rural areas. These population lack access for health care, because their poverty, low income, and less access to transportations. Serving in shortage area is sacred mission for health care provider. Nursing role in the underserved areas has a dominant factor to reduce children mortality rate, and reduce elderly diseases. It is an opportunity to address all the barriers that prevent this special group for health care access. It is the real challenging for nurses to use the utmost knowledge, and explore new ideas that need further investigations.
Over the last few years, I shadowed a Primary Care Adult Nurse Practitioner whose office is located in the underserved urban area of Irvington, NJ. She also takes care of patients from the surrounding areas of Irvington, Newark, and East Orange, all of which have very large underserved populations including African Americans, Latinos, and patients from the Middle East. During my clinical shadowing, I gained a appreciation for the complexity of treating long term chronic conditions such as asthma, diabetes, HIV, and hypertension. In many cases these conditions were exacerbated due to poor nutrition, non-compliance, and lack of education about healthy lifestyle choices. I gained a keen understanding of the importance of patient education and the ability to connect patients with community services to help them with their economic and social challenges.
As a future nurse practitioner, I have given immense thought to the selection of a clinical practice based on the primary care setting that utilizes the collaboration model. I have selected this type of clinical practice because it best suits my professional and personal goals. I value autonomy while having the ability to work within a healthcare team and enjoy teaching my patients. Nurse practitioners (NPs) are a valuable part of the healthcare team. In the 1960s, from a vision to improve primary healthcare to under-serviced communities, the development of the first NP program developed out of need as a public service and focused on the care of pediatrics and since then the care of other specific populations such as families, adults, geriatrics, and women health has emerged (Anderson & O’Grady, 2009, p. 380).
It is no secret that the current healthcare reformation is a contentious matter that promises to transform the way Americans view an already complex healthcare system. The newly insured population is expected to increase by an estimated 32 million while facing an expected shortage of up to 44,000 primary care physicians within the next 12 years (Doherty, 2010). Amidst these already overwhelming challenges, healthcare systems are becoming increasingly scrutinized to identify a way to improve cost containment and patient access (Curits & Netten, 2007). “Growing awareness of the importance of health promotion and disease prevention, the increased complexity of community-based care, and the need to use scarce human healthcare resources, especially family physicians, far more efficiently and effectively, have resulted in increased emphasis on primary healthcare renewal…” (Bailey, Jones & Way, 2006, p. 381). The key to a successful healthcare reformation is interdisciplinary collaboration between Family Nurse Practitioners (FNPs) and physicians. The purpose of this paper is to review the established role of the FNP, appreciate the anticipated paradigm shift in healthcare between FNPs and primary care physicians, and recognize the potential associated benefits and complications that may ensue.
The purpose of this paper is to describe how I visualize my future role as a mastered prepared nurse professional. The Association of American Medical Colleges estimates that by 2015 the nation will face a shortage of 62,100 physicians, 33,100 primary care practitioners and 29,000 other specialist. The nursing shortage evaporated during the recession, because many nurses returned to the workforce, but nurse practitioners remain a scarce resource in many areas (Expanding the Role of ANP Risk and reward). American Association of Retired Persons (AARP), a supporter of transforming health care to meet the challenges of an aging population and a shortage of primary care providers, agrees that advance practice registered nurses (APRNs), could provide seamless, accessible, affordable, and equitable quality care if they are utilized to the full extent of their education and training (AARP). In 2010, the Institute of Medicine published The Future of Nursing: Leading Change, Advancing Health, a report which offered an intensive examination of what the nursing profession is now and should become. It states that nursing is at the heart of patient care and is therefore crucial to changing the way health care is delivered so that patients receive care at a cost they can afford. Nurses are a linchpin for health reform and will be vital to implementing systemic changes in the delivery of care (AARP).

A community can be defined as a group of people, who live, learn, work and play in an environment at a given time. (Yiu, 2012, p.213) There are many factors that may influence the community’s development and health status. These can include resources available, accessibility, transportation, safety, community needs etc… These influences may combine together to form community strengths and as well as community challenges or weaknesses. As a community health care nurse, it is significant for us to assess and identify these strengths and challenges within the community in order for us to intervene and provide the appropriate needed health care services for the community members. From the literature I learned about the importance of driving the nursing profession towards the community health-care system. There is currently a need for revitalization of the primary health care system as stated in this document. There is a need to change some of the focus of education and training in new nurses in an effort to respond to the health care demands of today and the future, since many students are not going into this field. In addition, nurses already working in community health should begin focusing on current prevention and promotion aspects of health. I plan to apply these ideas in my own nursing practice to this rotation.
