The purpose of this goal is to provide better patient centered care, while applying a greater importance of each individual. “Nurses have the opportunity to play a central role in transforming the health care system to create a more accessible, high-quality, and value-driven environment for patients” (IOM 2010, p. 85). Advanced education and fewer limitations on the nursing workforce will lead to more primary care for each and every patient, thus leading to greatest possible patient outcome. To practice significantly in a primary care setting I must implement and acquire new changes in nursing practice, patient care performances, and improved my participation in the innovative and enriched health care organization. The IOM report states that that nurses should be able to practice their profession at full potential based on training and education while it effectively collaborates with the other health care professionals. This leadership process has been related with improved patient outcomes, reduced medical errors, and a lower turnover rates. Nurses must serve as strong patient advocates and contribute in decision making about how to progress the provision of care. “Everyone from the bedside to the boardroom must engage colleagues, subordinates, and executives so that together they can identify and achieve common goals” (IOM, 2010 p. 223). To contribute patient safety and excellent care, nurses in all levels must have strong leadership skills. Nurses should have a voice in health policy decision making and should be an active part of advisory committee, commissions and board of nursing where policy decisions are made to improve patient care (IOM, 2010). The IOM report had four key messages needed for advancing the future of nursing. “Nurses should practice to the full extent of their education and training; achieve higher levels of education and training through an improved education system that promotes seamless academic progression; be full partners, with physicians and other health care professionals, …and; effective workforce planning and policy making require better data collection and an improved information infrastructure” (Institute of Medicine, 2011). The report also included eight recommendations needed to facilitate the necessary changes to in the nursing profession so meet to demands of the healthcare reform.

Through reflecting on my previous placement, I have identified that my lack of knowledge and organisation skills have impacted on my confidence and ability to hand over to qualified nurses. In healthcare it is very important to have strong leaders, especially in the nursing profession. A nurse leader typically uses several styles of leadership depending on the situation presented; this is known as situational leadership. It is important that the professional nurse choose the right style of leadership for any given situation to ensure their employees are performing at their highest potential. Depending on which leadership style a nurse leader uses, it can affect staff retention and the morale of the employees as well as nurse job satisfaction (Azaare & Gross, 2011.) “Nursing leaders have the responsibility to create and maintain a work environment which not only promotes positive patient outcomes but also positively influences teams and individual nurses” (Malloy & Penprase, 2010.) Health care reform has been a major issue over the past decade. The Nursing industry has in particular experienced a period of unpredictable change. On Dec 24, 2009, a landmark measure was passed in the senate by a vote of 60 to 39. This decision to pass the health care reform will change America forever. Nurses will constitute the largest single group of health care professionals. They will have a huge impact on quality and effectiveness in health care. The nursing industry will help hold this new program together by acting as the glue (The nursing industry will be the glue holding the new health care in tact.) It is estimated that by 2015 the number of nurses will need to increase to over 4 million. Nurses are the backbone of the health care industry thus creating better polices for this profession will help ease the workload and high demand. A nurse’s main concern is always to insure quality care and the safety of their patients. Under the new health care reform several new measures have been set into place to ease the transition and improve the quality of care for all patients. One program is designed to fund scholarships and loan programs to offset the high costs of education. Nursing shortages and the high turnover has become a serious epidemic. In today’s society, leadership is a common yet useful trait used in every aspect of life and how we use this trait depends on our role. What defines leadership is when someone has the capability to lead an organization or a group of people. There are many examples that display a great sense of leadership such being an educator in health, a parent to their child, or even a nurse. In the medical field, leadership is highly used among nurses, doctors, nurse managers, director of nursing, and even the vice president of patient care services. Among the many positions in the nursing field, one who is a nurse manager shows great leadership. The reason why nurse manager plays an important role in patient care is because it is known to be the most difficult position. As a nurse manager, one must deal with many patient care issues, relationships with medical staff, staff concerns, supplies, as well as maintaining work-life balance. Also, a nurse manager represents leadership by being accountable for the many responsibilities he or she holds. Furthermore, this position is a collaborative yet vital role because they provide the connection between nursing staff and higher level superiors, as well as giving direction and organization to accomplish tasks and goals. In addition, nurse managers provide nurse-patient ratios and the amount of workload nursing staff has. It is their responsibility to make sure that nursing staff is productive and well balanced between their work and personal lives.
Nurses comprise of three million of the United States healthcare professionals. Several years ago in 2008 the RWJF and the IOM collaborated for two years to discern the future needs of the nursing profession. Most importantly, the objective was to outline the critical actions needed to ensure nursing was ready to seamlessly move towards the future. This was no easy task as nurses work in such diverse settings such as outpatient areas, acute care settings, the community, and long term settings to name a few. Couple this with the fact that nurses have a variety of educational avenues such as the associates, diploma, or bachelor’s degree open to them to achieve the status of registered nurse (Institute of Medicine, 2010). All of this considered, the committee did design four key messages regarding the future of nursing as key in the transformation of health care as evidenced in their "Future of Nursing" report.
From the literature I learned about the importance of driving the nursing profession towards the community health-care system. There is currently a need for revitalization of the primary health care system as stated in this document. There is a need to change some of the focus of education and training in new nurses in an effort to respond to the health care demands of today and the future, since many students are not going into this field. In addition, nurses already working in community health should begin focusing on current prevention and promotion aspects of health. I plan to apply these ideas in my own nursing practice to this rotation.
