The following are commonly used forms and the reasons you might use them.

This form is used to register for your courses for the academic year. If you have not yet completed this form for your program, please do so as soon as possible.
Incomplete Request
This form is used to request an incomplete. Please see Incomplete Grade Policy for more information.
Independent Study Request
An independent study can be requested when a desired course is either not offered in the academic year you need to take it, or when a course is not offered that would be beneficial to your career or degree (i.e. original content). All Independent Studies are at the

By filing a leave of absence, you will not have to reapply to the program when you are ready to return and will not collect Continuous Enrollment fees. All access to College resources will be turned off during the length of the leave.
Application For Degree
This form should be completed during your second year in the program. It will be used to determine your intended graduation date, as well as the preferred name for your diploma.
Continuous Enrollment (CE) Policy
In order to remain a full•time student, any session that you are not taking a credited course, you will need to enroll in Continuous Enrollment (5000). There is a fee of $100 for this placeholder course. This fee is waived in the event that there are no required courses available in a session. Students are able to take up to 4 consecutive continuous enrollment sessions • if you do not register for a course after that time, you will be administratively withdrawn from the program.
To be enrolled in continuous enrollment you must complete the request form. Once approved you be automatically be enrolled in CE for the session specified. Continuous Enrollment Form
Incomplete Grade

Any appropriate documentation must be supplied within five business days of the request.
This form will be sent to the instructor who will review the request. If approved, the instructor will complete the Graduate Programs Incomplete Approval form and outline any stipulations. All incompletes must be completed within 3 weeks of the last day of the course. An incomplete not cleared by the deadline will result in an administrative grade of “F” assigned to the student for that course.
If a faculty member becomes ill, dies, or leaves the institution, then any final decision about grades will be determined by the program director.
The Academic Standards Committee may consider a student’s appeal and recommend steps for resolution when a dispute arises in the process of granting an incomplete under the categories listed above.
When incompletes are completed, the action appropriate for the student’s standing becomes effective as soon as possible: Academic Honors, Warning, Probation, etc.

Students understand the consequences of low grade averages, so they constantly evaluate grades by keeping up with their averages. Low grades cause academic probation. During the probationary period students are encouraged to focus on applying themselves to bring grade averages up to standards. While focusing on averages, students are able to change their study habits or learning styles for instance; some students learn by hands-on-training and others by reading comprehension. The grade scale can determine if a student needs further instructions or tutoring. One the other hand, students study habits play an important role in their grades. If a student makes the wrong decisions about studying and preparing for class their grades will suffer drastically. Study habits need to be evaluated and processed for tips to succeed.
The Federal Government has set guidelines concerning students who receive Title IV Aid and completely withdraw from school or stop attending courses. Any student making a complete withdrawal or that stops attending their classes who received a Federal Pell Grant, SEOG, Subsidized Stafford Loan, Unsubsidized Stafford Loan or a Parent (PLUS) Loan, will be required to pay back money to the school immediately if they drop out or stop attending courses before 60% of the semester has been finished. After the calculations are figured, the Financial Aid Office will notify any student that might owe money back to the Federal Government. Your Financial Aid Satisfactory Academic Progress status will also be evaluated at the end of the semester and you will be notified accordingly
At this time I am attempting to complete another sap appeal for the fall term, I do realize that this is my second appeal. Past appeals were approved due to my medical issues and caring for a spouse who had surgery in June, I did have an agreement to pass math with a grade of B, however, I did not receive an B, my grade in the class was a C. Although I did not meet the grade requirement, I did the best that I could on my own being as though my GPA was extremely low, my grade improvement did not impact my GPA they way it needed to.
