The Missouri State Board of Nursing had several violation and disciplinary actions set for a hearing to review. The violation against one nurse was a second offense. A disciplinary action is for a first-time offense. Each action taken by the board is done with a group discussion and later deliberated to come to a final decision. The violators are then notified by mail. The two I chose are a violation and then a disciplinary action.
The violation was due to the failure to abide by the probation set forth during her first disciplinary action. The nurse was put on five years’ probation in June of 2015. She explained how she relapsed after being 3 months clean. Since the relapse in 2015 to 2016 she became clean again in August of 2016 with the help of her fiancé and her family. Since she took responsibility for her actions and explained that she does have a problem. I believe that is the first step to recovery. She was very adamant about how she does do wrong and then she had a plan to do what was right in her life for herself and not for anyone else. It may seem selfish, but that is the only way to control an addiction within one’s self, in my opinion. Accepting what is wrong and willing to deal with it is a start and having a great support system will help tremendously. The State Board of Nursing should allow her a second chance to prove that she can overcome and control her addiction while preforming the career she worked hard to achieve. The board should place a strict probation, and tasks that she can perform should be evaluated periodically. Drug diversion is a type of drug dealing, nurses have access to many drugs and it is a part of the nurses’ responsibility to ensure those drugs go where they should, precisely document and closely supervised. Criminal conduct can happen in the work place or on personal time. If a nurse is convicted of a crime, such as Driving While Intoxicated, it could affect their ability to practice nursing. It is out of a RNs legal scope of practice to medically diagnose any patient, order a medical treatment or conduct a medical treatment that has not been ordered by a physician. It is the nurse’s duty to their patient’s to exhibit sound clinical judgment, with in their scope of practice to ensure patient safety.
Overcoming an addiction to alcohol can be a long and bumpy road. Many people feel that it is impossible to overcome an alcohol addiction. Many people feel that is it easier to be an addict than to be a recovering addict. However, recovering from alcoholism is possible if one is ready to seek the help and support they need on their road to recovery. Recovery is taking the time to regain one’s normal mind, health and strength. Recovery is process. It takes time to stop the alcohol cravings and pressure to drink. For most, rehab and professional help is needed, while others can stop drinking on their own. Recovery never ends. After rehab, professional help or quitting on your own, many people still need help staying sober. A lot of time, recovering
“We can’t turn away from a patient’s pain just because it’s difficult” (chapman, 2015, p. 88). I know the path of least resistance is taking a path of ignorance. Easy, is to ignore or neglect the true pain patients experience in times of crisis. As caregivers I believe we all want to heal others or we wouldn’t be in the field of nursing, but there are only handfuls willing to be present during the healing process because sharing one’s pain hurts. As a surgical nurse, I find being genuinely present takes hard work on my behalf, not only mentally but emotionally. On a unit where patient’s needs and conditions are changing at astonishing rates, being present requires mental strength in order slow down enough to recognize the value presence
This experience helped me to recognize the internal struggle that a substance abuser faces on a continuous basis. In addition, I know that an individual can have a difficult time changing their behavior even when they have a strong desire to change; the smallest thing can cause a person to relapse.
Nursing is a profession that requires a unique skill set. A few of the traits include compassion, understanding and empathy. Clayton State University has a goal to produce competent, compassionate, professional nurses with communication and technical skills. Over the years, substance abuse in the United States has become a persistent issue affecting many individuals. In 2008, it was estimated that 17.8 million Americans over the age of 18 where substance dependent (Epstein, Burns, & Conlon, 2010). Many of these individuals being affected are nurses. Ponech (2000) stated that "approximately 10% of the nursing population has alcohol or drug abuse problems, and 6% has problems serious enough to interfere with their ability to practice" (as cited in Talbert, 2009, p.17). Studies show that nurses have a 50% higher rate of substance abuse compared to the rest of the public (Epstein et al., 2010). Among the many factors that contribute to the nurse’s issue of substance abuse, accessibility to drugs in the work environment has played a significant role. Substance abuse among nurses is an arising issue in need of attention, it is alarming to know that patient safety and care is in danger when a chemically impaired nurse is in the workplace.
Substance abuse is a mental disorder that has numerous negative effects to everyone involved. When a person has an addiction they turn into someone different, their brain chemistry is changed and brain cells are lost. From a nursing standpoint there are many things we must consider when caring for a person who has an addiction.
I had a patient that was a threat to others, was not redirectable, and was refusing to come out of the bathroom. I informed the doctor that this patient was threatening staff, the doctor told me what to give; nurses usually collaborate to get security, additional staff, draw IM medications, etc. As another nurse and I were drawing up Haldol, Benadryl, and Ativan, the doctor peeked in and told us not to bother, that the patient was now calm. This patient was refusing all PO medication and was grossly psychotic. It did not matter if in that instance she was calm, she was a threat to staff because she had threatened them, and my note reflected this. When I gave report to the next shift, I warned them. The next day in morning report the evening/night staff reported that security was called and that this patient had to be medicated. This doctor does not understand, that if this patient had hurt staff, the doctor would be liable because my note stated that she was a threat and that the physician refused to medicate. This doctor was angry at me because I documented the facts, so

Local government agencies, national, and global evolvement needs to be present to overcome substance abuse. It is important to try and overcome substance abuse due to the negative impact that it is having on society, financially and
