Interprofessional teams in health care are considered to be one of the best approaches to improve patient outcomes. Interprofessional teams provide the means to integrate patient care with input from many different professional disciplines (Rose, 2011). Nurses are an important part of the interprofessional team, since they are often the team member that is closest to the patient (Miers & Pollard, 2009). I recently participated in a team that developed a work flow for daily readmission rounds. The team was interprofessional, the hospitalist, who was an APRN led the team. There was the case manager and the primary nurse who were both RN’s. The team also consisted of a resident, pharmacist, nutritionist, physical therapist, and social worker.
The team researched best practice for addressing readmission, and developed a checklist that evaluated the prior discharge plan and the potential gaps in care that contributed to the readmission. The team worked with the EHR analytics team to identify patients that had been readmitted to the hospital at 30, 60, and 90 days. An automated email sent a daily list of readmitted patients. The daily rounds were organized by the hospitalist who used of a checklist for each patient and led the discussion identifying gaps in care. In the lead role, the hospitalist APRN asked the questions, specific to each discipline. During the first few readmission rounds, I was impressed at the knowledge of the team members, especially the case manager and social worker and their knowledge of community resources. The resident, pharmacist, nutritionist, and physical therapist contributions were of great value and the team was able to assess the whole patient, not just one aspect of care. The primary RN was able to give insight regarding the patient’s story, the interactions with the family members and the patient’s cognitive ability to understand the plan of care. Working to develop the team was a worthy experience. We are now collecting data to evaluate if the rounds impact readmission rates.
The interprofessional team model is a group comprised of various healthcare disciplines working together towards common goals to meet the needs of the patient population. In this event, the matter that is unusual can be the fact that I have experienced and witnessed the process for interprofessional collaboration between the community nurse and other professionals that I have never knew about before. This event made me realize that there are many aspects of community nursing that I have knew about before where in this situation it is the importance and accountability of interprofessional collaboration. From my nursing theory course I have learned that interprofessional collaboration is when the nurse forms relationships with other professionals that enable them to achieve a common goal to deliver care and strengthen the health system and clients involved in it. In the nursing leadership and management textbook it stated “interprofessional practice removes the gatekeeper and allows client access to all caregivers based on expertise needed.” (Kelly & Crawford, 2013, p.35) In this event, my preceptor and I gained knowledge about Revera and will pass on this information to patients who are interested in staying at a retirement home after they discharge from the hospital. One literature talked about how according to the Institute of Medicine, it is critical to have the capacity to work together as part of the interdisciplinary team to assist in delivering high quality, patient-centered care. In addition, effective collaboration among health care professionals results in improved patient care and outcomes. Effectively coordinated and collaborative inter-professional teams are essential to the care and treatment of patients (Rowlands & Callen, 2013; Doyle, 2008; Ruhstaller, Roe, Thürlimann & Nicoll, 2006; Simpson & Patton, 2012, p. 300). Communication is a process of conferring information between individuals through use of speech, writing or various other means, and is critical to the success of a multidisciplinary team (MDT) (Higgs, McAllister & Sefton, 2012, p. 5; Rowlands & Callen, 2013; Sargeant, Loney & Murphy, 2008). An MDT must use multiple strategies to enhance communication and ensure their success (Doyle, 2008). An effective MDT generates opportunities that benefit healthcare, which is the reason for the recent dominance of inter-professional care in health practice (Simpson & Patton, 2012, p. 300; Rowlands & Callen, 2013). Many barriers prevent effective communication within inter-professional teams. Lack of communication within MDTs presents challenges to their success, leading to numerous consequences, including the failure of the MDT (London Deanery, 2012; Sargeant et al, 2008). Communication between professionals is the key factor underpinning the potential success or failure of inter-professional teams, the outcome of the functioning of MDTs will either benefit or impair care of patients.
The Core Competencies for Interprofessional Collaborative Practice recognizes how communication places responsibility on all team members. Members must have the opportunity to speak up in a respectful way about their concerns and quality of care, and in return be treated with respect. According to the Canadian Nurses Association (CNA) position statement on inter-professional collaboration (2011), it states how the use of interprofessional collaboration is critical for improving client-centred care as well as strengthening effective collaboration with other health professionals (2011). Communication is a common theme that has repeatedly surfaced in my research.

What I find most difficult about interprofessional care is the professional hierarchy that is often in place. When physicians have a say in a patient, other members usually do not question it or try to challenge it because that physician might not agree with it or because of their level of education. When I think of team, I think of it as if everyone is equal and anyone can state what they think is right or wrong in any situation. If physicians are not approachable or open to other ideas, it may be hard to have an equal balance of power in an interprofessional team. Another difficulty or disadvantage I see to an interprofessional team is understanding everyone’s roles in the team. I am assuming that as time goes on, everyone becomes aware of what each team member does for a patient, but I could also assume that some members put other responsibilities on your plate. For example, if there is a case where a mom is suspected of child abuse and the nurse witnesses it, it would make sense for the nurse to report it because they were a witness and mandated reporter. I have talked to my supervisor at my internship and she says that nurses usually do not report to CPS because they instead let the social worker know in order for the social worker to make the report. I assume it is because nurses have a lot patients to see during their shifts and making a CPS report could take some

Effective cross-functional teaming is vital to the success of a company. Successful companies possess common traits in how its employees execute on tactics. Nurse educators are educators to not only teach the importance of nurse competencies, but also the business of nursing competencies. This was demonstrated at the University of Pennsylvania in Philadelphia with the Transitional Care Model program, nurses being involved in facilitating change is in decreasing hospital readmissions. When nurses are assigned to elderly, high risk patients that are likely to relapse, chances for readmissions occur. In this program the nurse follows the patient up to three months after discharge, attending their medical appointments, and collaborating with physicians, caregivers, and family. This program demonstrated a significant reduction in the number of hospital readmission and costs dropped by as much as $5,000

The data we have gleaned regarding implementing a nurse practitioner in triage can be used to improve patient’s LOS, LWBS, and adverse outcomes using the PDSA model. The PDSA process is necessary because it can test work process quickly and implement change effectively. Collaboration is a key aspect in team formation prior to performing the PDSA. This step is instrumental in bringing key people to the team, because the team cannot perform well without the right people and the right leader.
