Interprofessional collaboration is an important aspect associated within the medical field. This is a time when different kinds of professionals collaborate with one another about a patient’s health status and condition. “Specifically, interprofessionality is a ‘process by which professionals reflect on and develop ways of practicing that provides an integrated and cohesive answer to the needs of the client/family/population…(involving) continuous interaction and knowledge sharing between professionals” (Black, 2014). The physician was responsible for listing and writing down everything about the patient’s current status. The collaboration was needed so that they can come to an agreement on weather or not to discharge the patient. Verbal communication was used. The communication was effective because both the nurse and the resident were able to confirm how the patient was capable of doing his finger stick, and counting his carbs on his own. I was the student nurse for this patient, and my responsibilities consist of taking the patient’s vital signs. I was not involved in the collaboration, but my thoughts during the situation was that it would be a really good idea to talk about the patient’s situation in order to verify that everyone is on the same page. If I were to place myself in the patient’s shoes during this situation, the feeling I would endure is safety and security. The reason why I would feel this way is because of the fact that so many health care professionals are collaborating with one another in order to ensure adequate teaching and information. An ethical principle to include related to this experience would be beneficence. In order to lead to a beneficial outcome for this patient and to prevent any harm from occurring, the health care providers collaborated with one another for the patient’s benefit and safety. The patient outcome is related to interprofessional communication because multiple professionals make the patient’s goals. This will give the patient the best possible outcomes upon discharge. Many years from now, I’ll take this experience with me to better myself as a nurse. The nurse of my patient collaborated with the resident to make sure the patient counted the carbs of his food correctly. The nurse wanted to make sure that the right insulin dose would be administered to the patient. My patient’s nurse was being an advocate for this patient, so he would not be sent home without knowing to correctly count his carbs. I will take this experience with me as I approach my future-nursing career, because I see how important it is to achieve positive patient outcomes. Without any interprofessional communication, many patient errors can occur. The reason many errors can occur is because the patients situation wont be viewed my multiple individuals, which can cause a wrong decision to be made. Interprofessional teams in health care are considered to be one of the best approaches to improve patient outcomes. Interprofessional teams provide the means to integrate patient care with input from many different professional disciplines (Rose, 2011). Nurses are an important part of the interprofessional team, since they are often the team member that is closest to the patient (Miers & Pollard, 2009). I recently participated in a team that developed a work flow for daily readmission rounds. The team was interprofessional, the hospitalist, who was an APRN led the team. There was the case manager and the primary nurse who were both RN’s. The team also consisted of a resident, pharmacist, nutritionist, physical therapist, and social worker.
Nurses should foster collaborative planning to provide safe, high-quality, patient-centered health care. As nurses, we probably know the patients the best since we spend much more time with them than any other members of the care team. We are the liaison between the patient and the rest of the care team. We need to identify a potential problem or issue, and bring it to the attention of the whole care team. We should facilitate mutual trust, respect, shared decision-making, and open communication among all relevant persons in the care of the patients. With the collaborative support of the whole care team, the patients are in a much better position and are more likely to make an informed and deliberate decision. It is obvious that a great deal of interprofessional research has been aimed to educate practitioners and nurses over the past decade for interprofessional practice (Orchard, King, Khalil & Beezina, 2012). The Institute of Medicine (IOM) “The Future of Nursing Leading Change, Advancing Health” (2010) recommend that private and public organizations, nursing programs and associations increase opportunities for nurses to lead and manage collaborative teams. Health care reform has created a shift in the healthcare delivery to place more emphasis on interprofessional health care teams (Sinfield, Donoghue, Horobi & Anderson, 2012). New implications are directed towards continuing education for health care workers to understand the meaning of interprofessional collaboration to support the changes in collaborative practice to improve patient outcomes (Orchard et.al, 2012). Encouraging health care professional to collaborate as a team more effectively may seem as the answer to improve the quality of care, but ineffective communication from team members to collaborate on the care needs often attributed to patient safety issues. Consequently, even when professional collaborative teams work together, there is no means to validate and measure the impact on continuing education for nurses about interprofessional collaborative practice (Sinfield, et al., 2012).

Many barriers prevent effective communication within inter-professional teams. Lack of communication within MDTs presents challenges to their success, leading to numerous consequences, including the failure of the MDT (London Deanery, 2012; Sargeant et al, 2008). Communication between professionals is the key factor underpinning the potential success or failure of inter-professional teams, the outcome of the functioning of MDTs will either benefit or impair care of patients.

I have come to fully understand that in order to treat my patients in a way that is person centred, I have to treat each person as an individual and realise that every individual has different needs and different rights and preferences to me which may go against my morals and beliefs but I always have to maintain my professional boundaries and treat each individual with respect and dignity. If I was a nurse who witnessed a similar situation to Kat’s, where another healthcare professional was disregarding my patient or any patients views or requests I would go into the patient’s room and find out what the problem was. Then I would politely ask the healthcare professional to step outside of the room and I would gently remind them of the code of ethics ((Kozier, Erb's & Berman, 2010, p.97) and the Registered Nurses standards of practise (2016), and how every individual has the right to make their own independent decisions about their healthcare needs/goals based on their own values, morals and beliefs. I would further explain that the patients are our main priority and it is our responsibility as nurses’ to ensure that the patients are safe and are receiving the proper care. I would then explain to the patient what was happening and apologise to them about the situation, and I would rearrange and try to negotiate with the patient when the procedure could be performed. Effective communication between patient and clinician is an important aspect to patient care. Proper communication has a direct positive impact on patient care and adversely poor communication has a direct negative impact on patient care. I will define the seven principles of patient-clinician communication and how I apply these communications with my patients. I will also describe the three methods currently being used to improve interdisciplinary communication and the one method that my area of practice currently uses. Then, I will explain the ethical principles that can be applied to issues in patient-clinician communication. And Lastly, the importance of ethics in communication and how patient safety is influenced by good or bad team communication.
