Integrated Care Management
Care Management at UAMS is not a unified, integrated model. Care managers at UAMS are found throughout the Organization and provide not only telephonic client support but are located in clinics and inpatient settings. Education, experience, expectations and roles vary from location to location. Supervision is done at the local level primarily by leaders without care management experience or an in depth understanding of the standards of practice.
Regardless of the setting, care managers are health care professionals, social work or registered nurses, who incorporate the theory and practice applications of care management into clinical practice. It is a care delivery model focused on coordinating and managing the

Some are imbedded in the service line, others provide expertise for all. Education is as varied as the location. Care managers must be educated and be able to apply the theory and concepts of Care management regardless of the setting of the practice or the job description defined by an employer. Education programs are designed to be comprehensive for this purpose.
Recognition of the value of care management programs within UAMS continues to grow. Redesigning the organizational structure to promote greater integration across various care settings and expand roles to strengthen transitions of care, partner with community resources and organizations to optimize resources to achieve population management.

Due to WellStar being a multi facility health system, its organizational design is constantly being reviewed for simpler and more efficient processes. WellStar’s two smallest hospitals, WellStar Paulding and WellStar Douglas, previously under went reconstruction with regards to their hierarchical structure in Patient Access Services (PAS). WellStar Paulding, the smallest facility of the five hospitals, renovated their managerial chain of command in PAS. Health care systems deal with many different cultures, values, and interests making it increasingly more difficult for management to provide their employees with a clear vision of the future (Lega, Longo, & Rotolo, 2013). Begun, Hamilton, and Kaissi (2005) explain health care centers utilize strategic planning to better understand their environments and ensure the organization’s structure, culture, and important decision-making are compatible within their current surroundings. By making changes to the scheduling department and incorporating inter-departmental meetings, schedulers will have a greater respect for technologist’s workload. Customer service at the outpatient clinic will no longer suffer due to communication gaps. By implementing inter/intra-departmental meetings staff will be able to focus more on the patient and provide them with an overall better experience. What does ‘care’ mean? Care is the provision of what is necessary for your health, welfare and protection of someone or something. However when you talk about ‘care’ in a care practice the term changes and becomes more about enabling people to meet all their needs which would refer to their social, physical, emotional, cognitive and cultural needs. The individual is central to the meaning of care in this context.

A nurse manager plays an important role on a hospital unit. Evans defines the role of a nurse manager as one who makes sure all the needs required on a daily basis are accomplished (Evans, 2011). Evans goes on to say that one primary responsibility of a nurse manager acting in the position of a leader is to “raise the level of expectation and help employees reach their highest level of potential excellence” (Evans, 2011). With this said, it is important to identify potential barriers and problems that a nurse manager would face on a given unit and create or adopt evidence-based interventions to eliminate these problems. When this is accomplished, it will help to foster a work environment that maintains safety to patients as well as staff.
Changes in the current health care system can help prevent unsuccessful transitions of care. In order to move away from the “silos” of care, many institutions are starting to trend towards primary patient centered and interdisciplinary care. Having a team in charge of the care for a patient will allow more effective treatments and more communication between the different providers. While this is only within an inpatient setting and not necessarily transitions of care, the variety of clinicians involved in the care of a patient allows more information to be transmitted across different setting. This type of environment is accomplished by nurses working together towards a strategic plan outlined with facility policies and knowledge-based nursing and skills to achieve desired outcomes and accomplish organizational goals. I believe my facility does a great job with structural empowerment. Our nurse manager makes sure we know what our goals are as a unit. We review hospital satisfaction scores and infection rates to determine what practices need to be addressed for achieving organization and desired outcomes. We work to provide cost-effective care that exceeds national standards for excellence while working to strengthen bonds between staff members and making sure that at our facility “caring comes first”

Support of a decentralized organizational philosophy can transform organizations, staff, and patients because it affects the culture, improves staff outlook, promotes personal involvement and encourages staff to reach higher levels of quality care.
