Cultivating Healthful Environments Incivility is a known issue that continues to plague the workplace environment. There is no exception for the healthcare community and its surroundings. Daily healthcare workers are subject to incivility in the workplace. Incivility in nursing has the potential to cause a decrease in the quality of care, cause increased amounts of physical and emotional stress, and place patients in harmful situations (Armstrong, 2017). This paper will present the issues that incivility is placing on healthcare and how important it these issues are to nursing. Not only does there need to be communication among peers but there also needs to be communication between all hierarchical lines of the organization. Everyone’s opinions need to be valued no matter their position on the hierarchy of the organization. Incivility has been shown to have an increase in facilities where poor communication is among staff members (Abdollahzadeh, Asghari, Ebrahimi, Rahmani, & Vahidi, 2017). Building team and team spirit is a great way for staff members to work as a team and rely on one another for support. Having teams and raising employees spirts makes them feel valued within the organization. When working as a team staff feel as if they are part of a group working at reaching a common goal as their peers. Instead of individuals trying to compete against each other to finish the goal first. Both the organization and nurses must work together to help promote the healthful environment in which they all work. Abdollahzadeh, Asghari, Ebrahimi, Rahmani, & Vahidi (2017) found that to prevent incivilty in the work environment that a comprehensive and systemic attempt was needed. FNP use communication daily to speak with patients, family members, and other members of the organization they work for. To help promote and create a healthful work environment communication is a necessity in striving for this. FNP can create or help promote commincation within their organization through a staff development committee (SDC). SDC is a committee where employees come together to work on components of a healthful work environment and strive to make their organization a better place to work (Eads, 2017). A comprehensive approach can also be useful in helping an FNP to promote a healthful work environment. With FNP being in more of the middle of the hierarchy of and organization, often they can have more impact on an organization. FNP need to work together with all aspects of employees to help promote the well-being of everyone. Coming together as team and presenting their findings and ideas to the organization on how to make the health and well-being of the staff better. This is where the organization then must come in a listen and support its employees to the best of its ability. Both sides must work together to overcome the incivilty that can negatively affect an organization. Organizations and its employees play a major role in preventing workplace incivilty and FNP can help achieve this
Horizontal violence is a form of workplace relational aggression, an experience that is rampant in the nursing profession (McKenna, Smith, Poole, & Coverdale, 2003). The expression, horizontal violence, is used to describe cruel behaviors between colleagues of comparable status, such as registered nurses, in the workplace. Research has shown a variety of destructive peer-to-peer behaviors that are detrimental to the profession and healthcare systems (Farrell, 2001). Nevertheless, little research has been done on “eating their young,” horizontal violence arising between those with unequal power, such as registered nurses and students (Thomas & Burk, 2009).

Incivility is prevalent in the healthcare setting and is a broad term used to describe any negative discourteous manner all the way up to more serious bullying and violent behaviors. Nurses are caring by nature. Nurses care for family members while at home, community members who may be neighbors, church members or friends from school and sports with children in common; however, nurses are known to display uncaring attitudes towards each other. When nurses are discourteous and disrespectful towards one another this may be known as workplace incivility. Incivility is defined by Merriam-Webster as, “the quality of state of being uncivil and a rude or discourteous act” (n.d.). Alexander (2017) related incivility to the events of the 2016 United States election as “rude and impolite behaviors that may be manifested when people feel fear or mistrust” (p. 79). Healthcare is subject to the same negative influence through communication between healthcare providers, educators and patients.

Therefore, this position statement is relevant because these abuses can be seen in day-to-day healthcare environment. The effects of violence in nursing can be harmful to the proper function within a workplace. It can be damaging to the nursing profession and patient care. According to (Johnston et al., 2010, p.36), workplace violence is “spreading like a ‘superbug.’” Studies have shown, that lateral violence, nurse-on-nurse, has been one of the highest incidence of violence within the workplace. Also, statistics have shown that lateral violence has one of the most emotional impacts on an individual. This will be further discussed below. For these reasons, it is important for healthcare workers to validate the detrimental effects violence can have in the workplace, and be prepared to combat and prevent workplace violence.
In the early 1980s aggression and violence in the workplace have been a source of a lot of public discussion. (Piquero pg.383) The issues have risen again recently and have mostly been present in management and business fields. Workplace aggression often includes “behavior by an individual or individuals within or outside organizations that is intended to physically or psychologically harms a worker or workers and occurs in a work related”. Incivility can affect many different aspects of our lives. A few are causing anger problems, being in others’ way, making it difficult for somebody to concentrate, and causing workers to skip work. However, at least in the workplace, this issue can be helped (if the company is willing). Although on a small scale, incivility and rudeness may seem insignificant, they have proved to be quite problematic.
It has been estimated in a study by the American Management Association that managers spend between 20% to as much as 50% of their times dealing with conflict among their employees (American Sentinel, 2012). When workplace conflicts are left unresolved they can lead to dissatisfaction, depression and other problems such as aggression and violence (Whitworth 2008). The negative, sometimes hostile, environment created by unresolved conflict has been recognized to be a hazard not only to staff, but also to patient care (The Joint Commission 2008).
Effective communication encompasses many concepts. For a leader to be effective, one must be able to build relationships between different parties. In order to build a good rapport, one must be able to recognize the strengths and weaknesses of those involved, and within oneself. Acknowledging these differences will allow for a better collaboration between team members. A leader must also have the ability to put conflict resolution theories into practice. Allowing differing points of view may challenge the original idea and will make for a more thorough decision making process. Although this may seem counterproductive, it actually ensures the best possible outcome. One must be consistent in the message presented yet be flexible, approachable and maintain a positive attitude. Allowing for different perspectives to be heard will allow team members to feel valued. This flexibility helps to solidify the concept that everyone is in this together; thus establishing an environment where all team members can have a buy-in.
