Capstone Summary Performance improvement is vital to patient safety, quality of care, and patient satisfaction. Regular and an objective view at clinical outcomes is important for the nurse practitioner to engage in to ensure that his or her practice. This continual focus on improving outcomes ensures that quality care is being provided and optimal outcomes are met. This capstone project was a critical evaluation of the performance of a large family practice clinic. The goal of this project was to quantify the performance of the providers in the practice in their treatment of diabetes.
In this quality improvement project, data was gathered and analyzed using benchmark data from the American Board of Family Medicine (ABFM) that was collected from other participants in in the quality improvement exercise. This allowed the providers in the practice to assess their outcomes as compared to their peers nationwide. Strengths as well as opportunities for improvement were identified. Interventions were planned and implemented to meet the opportunities. After a period of intervention, the assessment tool was completed again and the effectiveness of the intervention was evaluated.

Each participant agreed to participate in the survey and were asked 10 questions that were designed to assess the patient’s understanding of his or her goals of treatment. Once the patient portion was completed, a chart review was conducted to gathered objective clinical data including hemoglobin A1c, LDL, and documentation of eye exams. Once data was collected, it was entered into a database maintained by the ABFM and analyzed. The Implementation Committee must clearly identify the desired results for this project. The ultimate goal is to improve customer satisfaction as a result of the leader adoption of the rounding process and timely identification and resolution to patient issues. In order to do this, nurse leaders must adopt and be held accountable to the Orchid technology and standardized rounding script. Today health care systems are expected to meet set standards and core measures to earn everything from accreditation and recognition to payment. Reports need to filled to accomplish this, as well as what is being done to improve areas that may not be meeting standards. One way this is done is by utilizing dashboards. The purpose of this paper is to analyze the data from a dashboard and develop a nursing plan for improvement of a low scoring area.
The interpretation of quality health care varies with each person. Some place emphasis on the ability to access various treatments without interference. Others value the feature of being able to simply select one’s provider. Quality health care, according to the Institute of Medicine (2001), can be defined as care that is “safe, effective, patient-centered, timely, efficient and equitable” (p. 3).
