Implementation of Orchid Leadership Rounding Tool With the passage of the Affordable Care Act (ACA), the Centers for Medicare and Medicaid Services (CMS) has initiated reimbursement based off of patient satisfaction scores (Murphy, 2014). In fact, “CMS plans to base 30% of hospitals ' scores under the value-based purchasing initiative on patient responses to the Hospital Consumer Assessment of Healthcare Providers and Systems survey, or HCAHPS, which measures patient satisfaction” (Daly, 2011, p. 30). Consequently, a hospital’s HCAHPS score could influence 1% of a Medicare’s hospital reimbursement, which could cost between $500,000 and $850,000, depending on the organization (Murphy, 2014). Due to the increasing financial implications, patient satisfaction has become a growing priority for health care organizations, as well as transitioning the health care organization’s philosophy about the delivery of health care (Murphy, 2014). This CMS value based purchasing initiative has created a paradigm shift in health care in which leaders and clinicians must focus on patient centered care and the patient experience which ultimately will result in better outcomes. Leaders and clinicians alike must be committed to the patient satisfaction. As leaders within the organization, these groups must be role models and lead by example for front-line staff. Ultimately, if patients are satisfied, they are more likely to be compliant with their treatment plans and continue to seek follow up care with their health care provider, which will result in decreased lengths of stay, decreased readmissions, increased referrals and decreased costs (Murphy, 2014). Through the technology, data can be tracked and trended to improve processes. The Implementation Committee must clearly identify the desired results for this project. The ultimate goal is to improve customer satisfaction as a result of the leader adoption of the rounding process and timely identification and resolution to patient issues. In order to do this, nurse leaders must adopt and be held accountable to the Orchid technology and standardized rounding script. Hourly rounding is a process of interacting with patients on a planned, regular basis to anticipate their fundamental care needs. The primary purpose of this project was to assess the feasibility and effectiveness of implementing hourly rounding on a pulmonary medical-surgical unit with a goal of reducing the rate of call bells.
