The design collects data through observation, personal interviews, photographs, the study of records and other evidence-based methods. The tradition of qualitative research design involves the same methods that this specific study used. The use of raw and archival data is characteristic of the design chosen by the authors. The hospital ethnographer observed the actions for a total of 60 hours over a 3 months’ period. The interviews provided a clear understanding about the issue of the call bell from both the patients perspective and that of the nurses. The ethnographer observed the various areas of the medical surgical unit, such as the hallways, nurses’ station, patient rooms and lounge area. As mentioned earlier, photographs were taken early in the morning to maintain patient privacy. The ethnographer used the pictures to help document the status of the call bell console, the number of lights that were on during the different shifts, who was standing near the call bell console and if they answered the lights. The outlook on the patients and appointments all depends on the area of practice and the health professional themselves. Working in the emergency department, the nurses and doctors there typically do not see the same patient more than once and if they do the chance of them remembering them is slim to none just for the simple fact of the pace of the department. Therein a gap of what the patient’s perception of quality care and how the healthcare providers perceive quality of care is created. Patient satisfaction surveys have been in debate being that the concept of ‘patient satisfaction’ is not clear and because this term means something different to everyone (Junewicz & Youngner, 2015). In her paper emerging model of quality, June Larrabee discusses quality as a construct that includes beneficence, value, prudence and justice (Larrabee, 1996). She speaks of quality and value as integral issues that are intertwined with mutually beneficial outcomes. Her model investigates how the well-being of individuals are affected by perceptions of how services are delivered, along with the distribution of resources based on the decisions that are made (Larrabee, 1996). She speaks of the industrial model of quality and how the cornerstone ideas of that model (that the customer always knows what is best for themselves) does not fit the healthcare model (Larrabee, 1996). Larrabee introduces the concept that the patient va provider goal incongruence affects the provide (in this case the nurse) from being able to positively affect healthcare outcomes (Larrabee, 1996). The recent introduction of healthcare measures such as HCAHPS: Patients' Perspectives of Care Survey has encouraged the healthcare community to firmly espouse an industrial model of quality.
