Nursing Care for the Patient with Disturbed Sleep Pattern Sleep is important and necessary for quality mental and physical health as well as quality of life and safety (NHLBI, 2012). People with sleep deficiency are at an increased risk of motor vehicle accidents, are prone to reduced work productivity, and have decreased levels of concentration (Hedges, 2012). Continued sleep deficiency can raise one’s risk for chronic health problems (NHLBI, 2012). Therefore, a consistent sleep pattern is necessary to achieve and maintain quality of life. During this step, the nurse specifies the patient outcomes to reduce, prevent, or resolve the problems identified in the nursing diagnosis (Taylor, et al., 2011). The planned outcome for this patient would be as follows: the patient verbalizes a willingness to implement four interventions for sleep promotion by the end of the clinic visit (Gulanick and Meyers, 2014). The forth step in the nursing process is implementation in which the plan of care is carried out (Taylor, 2011). In this case, the nurse will instruct the patient on four actions that the patient can take to help promote consistent and adequate sleep (Gulanick and Meyers, 2014). Those actions are addressed in the paragraphs that follow. The nurse instructs the patient to follow a consistent schedule for waking and going to bed. Consistency in sleep schedule helps to promote regular circadian rhythm, thus reducing the energy required for the body to adapt to changes (Gulanick and Meyers, …show more content…
Sleep is important for health and overall well-being. Through the implementation of the nursing process the nurse and patient together can work to reduce the patient’s sleep disturbance problems and restore consistent and adequate sleep.

In the recent past, nursing has come to the forefront as a popular career amongst students across the globe. The demand for nurses has kept increasing gradually over the years. In fact, the number of registered nurses does not meet the demand of the private and public health sector. This phenomenon has resulted in a situation where the available registered nurses have to work extra hours in order to meet the patients’ needs. With this in mind, the issue of nurse fatigue has come up as a common problem in nursing. According to the Canadian Nurses Association (CNA), nurse fatigue is “a feeling of tiredness” that penetrates a persons physical, mental and emotional realms limiting their ability to function normally. Fatigue does not just involve sleepiness as has been assumed before. It involves utter exhaustion that is not easily mitigated through rest. When nurses ignore the signs of fatigue, they risk the development of chronic fatigue and other health problems that may not be easily treated. Additionally, fatigue may cause nurses to lose more time at work as they may have to be away from work for several days to treat it. The issue of nurse fatigue has permeated the nursing profession to the extent of causing errors in the work performed by nurses. Fatigue causes a decrease in a nurse’s ability to make accurate decisions for themselves and their patients. It is therefore important to find ways to curb nurse fatigue such that it is no longer a problem. Nurse fatigue is a danger to the patients, organizations and to the nurses themselves and must be mitigated adequately.

The definition of sleep health is more extensive than just sleeping for 8 hours and not feeling tired throughout the day. According to a research article, “definition of sleep health should focus on those measurable characteristics of sleep that are most clearly associated with physical, mental, and neurobehavioral well-being,” there has been an acronym created to define excellent sleep health SATED (Buysse, 2014). SATED stands for the adequate sleep health every individual should be receiving in order to live a prolonged healthy lifestyle without disorientation in physical, mental, or behavioral actions. Results showed that for people to have sufficient sleep they must analyze sleep in these five terms, “these dimensions are Satisfaction with sleep; Alertness during waking hours; Timing of sleep; Sleep Efficiency; and Sleep Duration,” there is more to sleeping than simple laying in bed and resting for a few hours (Buysse, 2014). Nursing students are in desperate need of sleep awareness; many students are sacrificing rest in order to accomplish responsibilities. The nursing process is based upon five steps. The first step is the assessment phase; this can range from body system specific to head-to-toe assessment. These assessments are both subjective and objective and must be properly documented, organized and validated (Taylor et al, 2011). The second phase of the nursing process is formulating a diagnosis. I will also discuss the plan I created in order to successfully meet my learning needs prior to becoming an RPN, and how I will evaluate whether or not I improved in these two areas. Learning Needs / Goals The two areas I feel I need further development in are understanding the scope of practice for an RPN, and Intravenous (IV) therapy. The night shift nurse informed the oncoming nurse if the patient was scheduled for any tests or procedures. Also, if a patient was planned to be discharged. Based on the information from the report, the nurse prioritized her patients. She would see the patients that didn’t do well during the night. For instance, the nurse went to see the patient who had fever all night. During her nursing assessment, with this particular patient, she check to see if the patient was still running a fever as well as if the patient’s dressing had any sign of infection. During the nursing assessment, the nurse would introduce herself once again and would address the patient by their last name. Based on the information she received, she would assess the patient for any changes. Then, the nurse would give the patient’s medications. According to the American Nurses Association, the Nursing process is the essential core of practice for the registered nurse to deliver holistic, patient-focused care (ANA, 2018). It is a systematic method of providing nursing care. It embraces all steps taken by the nurse in caring for patients including; Assessment, Nursing diagnoses, Planning, Implementation of the plan and Evaluation. The process requires a systematic approach to the person's situation, beginning with assessment and including evaluation and reconciliation of the perceptions by the person, the person's family, and the nurse. A plan for the nursing actions to be taken may then be made, and, with the participation of the person and the person's family, the plan may be set. The plan developed with the person and the person's family is then implemented. The outcome is evaluated by the person and the person's family.
