Create a Survey for Nursing Staff Regarding their Perceived Needs for Discharge Education by Week 2. I have discussed this survey in a couple other sections of this paper. The information I got from this survey was very interesting. Not all of it was pertinent to my final project, but it helped me realize what our nursing staff feels is important regarding discharge education. We have decided as a team to leave the folder as-is for now and not change anything else. There are a lot of changes happening with the discharge education, so they did not want to do too much at one time. We also discussed providing hard copies of the eBooks that are available on our website. These should be available to patients who are not comfortable using the internet or who don’t have access. We are working on getting hard copies of all three eBooks, so we can be prepared to give them to our patients as needed. The learning module explains how to create printed education and medication leaflets as needed for patients. We can add suggested information based on patient diagnosis or problem list. This helps us provide education that is patient specific and also assists the hospital with meeting meaningful use requirements. If there is education that is not generated by the medical record, we can add any other topic that we find necessary. We can also edit the education leaflets as needed. All of this is covered in the education module. All these leaflets can be printed either during the hospital stay, or upon discharge. Although it was completed early, I continued editing it as parts of my project changed. When I first did the teaching plan, I did not have a purpose, or a goal written. While reviewing my peers’ plans, I noticed that they had those pieces. I went back to the notes I took form the lecture and found the information that explained the purpose and goal. I added them to my teaching plan at that time. I have to say I was disappointed that none of my peers noticed that I was missing them. I spent a lot of time assisting others on their projects, helping them get it right. So, when I saw that no one tried to help me out, I was a little upset. I am glad I was able to find the problem and fix it myself. I am very satisfied with the final teaching plan that I formulated. I feel it is important to have multiple ways to assess someone’s understanding. That is why I included a written test for every section, as well as either a chart audit and/or observation tool. There are many players involved including health care insurance, business, government officials that play an important role in the transformation of our broken health system.
The IOM report had four key messages needed for advancing the future of nursing. “Nurses should practice to the full extent of their education and training; achieve higher levels of education and training through an improved education system that promotes seamless academic progression; be full partners, with physicians and other health care professionals, …and; effective workforce planning and policy making require better data collection and an improved information infrastructure” (Institute of Medicine, 2011). The report also included eight recommendations needed to facilitate the necessary changes to in the nursing profession so meet to demands of the healthcare reform.
The College of Nurses of Ontario (CNO) is the governing body of all registered nurses in Ontario and is regulated. The CNO provides expectations and guidelines to follow, which need to be met by each Registered Practical Nurse (RPN) individually. As a nursing student, I am taught about the CNO and the importance of referring back to the guidelines while caring for patients. While gaining experiencing in the nursing field through my clinical settings, I have realized as a nursing student there are areas I need further development in. However, upon securing a job, they find that things on the ground are not as they had expected them to be and this results in some of them deciding to leave the profession early. Research shows that turnovers within the nursing fraternity target person below the age of 30 (Erickson & Grove, 2011). The high turnover within the nursing fraternity results in a massive nurse shortage. This means that the nurses who decide to stay have to work for many hours resulting in exhaustion. A significant percent of nurses quitting their job sites exhaustion and discouragement as the reason that contributed to their decision. In one of the studies conducted on the issue of nurse turnover, 50% of the nurses leaving the profession argued that they felt saddened and discouraged by what they were unable to do for their patients (Erickson & Grove, 2011). When a nurse witness his/her patients suffering but cannot do anything because of the prevailing conditions he/she feels as if he/she is not realizing the reason that prompted him/her to join the nursing profession. In closing, as the healthcare climate consistently changes across the United States, it is imperative that registered nurses understand the demand for superior knowledge and lifelong commitment to nursing. As nursing theory and practice expand, our role as leaders is to place a high priority on our commitment to educate and guide nurses, thus improving the lives of their patients through the support of transformational leadership. The Institute of Medicine’s recent report, The Future of Nursing – Leading Change, Advancing Health, specifically calls for nurses to advance and embrace leadership roles on all levels and fronts – from collaborative improvement to positive identification of areas facing challenges within the healthcare setting.

Teaching is part of being a nurse (Angelo, 2015). Being a nurse does not only end at providing full care to the patients but also includes proper teaching about the patients’ disease and medications. Patient’s education should happen when opportunities come, especially before discharge. By teaching them prior to discharge and providing them hand-outs, pamphlets or websites to find important information about their disease and medication, they will be ready to administer their own medications and be informed about their disease. Good personality is also vital in nursing practice (Angelo, 2015). It is not only what a nurse has learned in school or the skills and knowledge that he or she acquired from clinical instructors that are important, but also the whole make-up of a nurse’s own self. For example, a knowledgeable nurse is fully equipped with the skills in dressing a wound, but will never be an effective nurse if he or she does not know how to behave while performing the skill. The facial expressions and body language showing that a nurse is uncomfortable seeing the wound makes him or her unsuitable for the job. It is important that a nurse must not only limit his or her learning inside the skills lab or hospital (Angelo, 2015). The QSEN has initiated a taskforce to focus on nursing research to improve the quality of nursing education. Since Nursing Excellence is to promote holistic patient centered care, quality and innovation, education, evidence-based practice, and nursing research, QSEN plays a pivotal role in teaching new nurses these competencies. It provides educational institutes and educators with resources and training needed for producing well-trained nurses for future (QSEN Institute, 2014). The QSEN consultants are available throughout the country for nursing schools to guide them in improving quality and safety education. I am a clinical nurse educator and am very passionate about teaching. I would support QSEN as a nurse and as an educator. In future, I would seek out volunteer opportunities and attend conferences offered by QSEN to improve quality and safety education for current and future nurses.
In conclusion, teaching and learning is a complex and essential component of the nursing process. As seen in this case study, different teaching domains and strategies need to be utilized in order to help clients recall and apply important aspects vital to their health and ultimate quality of life. By providing J.L. with a diverse teaching tool that encompasses different types of learning domains, J.L. has all the information at hand to help him implement his diabetic foot care regimen.
The campaign envisioned accessibility to high-quality, patient and family tailored care in a health care system for all Americans.(IOM,2010) The campaign provides a platform for all nurses can lead system change to improve health. Further, the initiative was designed to help initiate progress by supporting Action Coalitions in implementing the IOM report recommendations. Comprising of diverse teams of stakeholders that are able to effect sustainable change both the state and regional level, Action Coalitions, work to set well-defined goals that align with the IOM report recommendations to advance educational transformation, removal of barriers to practice and care as well as nursing
From the literature I learned about the importance of driving the nursing profession towards the community health-care system. There is currently a need for revitalization of the primary health care system as stated in this document. There is a need to change some of the focus of education and training in new nurses in an effort to respond to the health care demands of today and the future, since many students are not going into this field. In addition, nurses already working in community health should begin focusing on current prevention and promotion aspects of health. I plan to apply these ideas in my own nursing practice to this rotation.
