Community Resources
In the Greensboro area, there are a lot of communities that can provide assistance for an older adult at different levels. My patient is in general an independent one in all aspects of her life and she would like to be in control of everything regarding her life. She is living in her nice apartment by herself, but in the same time, she enjoys being in the company of other people, especially people of her age or closer. Therefore, I consider that the best community for her to attend will be a day care community, where she can go almost every day and she can spend some quality time with other older adults. I chose a day care community because I know she needs a company to avoid episodes of depression that she has, occasionally,

accordingly with what she told me at one of my visits. Also, at the day care, besides all the activities that are organized there she can benefit of medical assistance in case of need, and meals will be provided for the time when she will be there. In my search for a more suitable place for my patient, first I stopped at the information that I already had available and I looked to see if that place fits the needs of my patient and also if my patient can afford that condition.
I know that Well-Spring company has under its umbrella a day care center the Pace of the Triad and I started to do some research. Looking online, I found out that the staff provides community-based services to enrolled individuals who need medical care and support to continue living at home. The Pace of the Triad is an adult day care and medical services for individuals 55 and older with chronic medical conditions. They surround people with the caring support that they need to live independently in their home and their community that they know and love. They offer medical services that help them to avoid health problems, rehab services, transportation and in-home services such as homemaker services, home-delivered meals, ramps and other home modification to accommodate your disability. easier. Their slogan is that Pace of the Triad is the place “where quality of life, person-center care and dignity really matter. We do not remember days, we remember moments”. To be able to participate in this program, an adult must be 55 years of age or older, to be a resident in Guilford or Rockingham county, to be able to live safely in the community, and to meet the State of North Carolina’s Nursing Home Level of Care criteria. Thinking of insurance, Pace of the Triad doesn’t ask for an insurance. If a person doesn’t qualify for Medicare or Medicaid, they can pay a monthly premium to cover the services. The Pace of the Triad is my choice for my older adult because during my research I found that they have provided care and improved the lives of a lot of older adults here, in Greensboro. They are ready to expand their services to me Enhancing your quality of life every dayet the growing need for health care for frail, lower income elderly and disabled adults who present the most complex and costly challenges to the Medicaid system. They reduce the need for costly, long-term nursing care. Because this program is responsible for all costs, they will do their best to keep participants as healthy as possible, to practice preventative measures, to monitor conditions, to prevent falls and to keep participants out of expensive hospitals, emergency rooms, and nursing homes. PACE of the Triad does not exclude people or treat them differently because of race, color, national origin, age, disability, or sex. What is more important for my adult is that they keep people in the community and out of nursing homes, reducing hospitalizations, and saving people money while providing quality outcomes for people. In 2010, there was a study which identify PACE as a model of effectiveness and efficiency of complex primary care, based on four processes that were described: development of a comprehensive patient assessment that includes a complete review of all medical, psychosocial, lifestyle and values issues; creation and implementation of an evidence-based plan of care that addresses all of the patient's health needs; communication and coordination with all who provide care for the patient, and promotion of the patient's (and their family caregiver's) engagement in their own health care.
PACE is effective and efficient in treating individuals with multiple and complex health care needs. To apply for this program, there is no need for a doctor’s recommendation and an important thing that I found out is that when somebody enrolls in the Pace program, he/she is assigned a PACE primary care physician who is a part of an interdisciplinary team skilled in managing care for aging adults. In the case of emergency, PACE participants use the after-hours phone number to speak directly to a PACE nurse who is familiar with his/her care needs. If it is an emergency, an ambulance will be dispatched to the participant’s home. If not, the participant will be transported to the PACE Center the following day for
treatment. I believe that Pace is the ideal place for my adult and I think it fits the needs she has. As I noticed in the interview that I’ve done with my adult, she has an increased risk of falls and therefore she would benefit from physical therapy and practice in using various devices to help her walk safely. Sometimes she needs assistance with She needs help in maintenance of the apartment in good condition, as she wants, or to procure their monthly medicines. She needs people around her to communicate and interact with them by participating in various activities organized by Peace. In conclusion, I would say that Pace of the Triad is my choice because it addresses the most complex and costly health needs of a rapidly expanding elderly population, prevents costly long-term care institutionalization. Participants have better health outcomes, participants and their caregivers have a better quality of life, and another important thing is that Pace saves money. References NC PACE Association. My first observation was at Head Start. I talked to Tracey Claflin about some of the principles they use in their program and how they use them. I wasn’t able to observe the children directly because my TB skin test results came up positive. Tracey explained to me that because my results were positive I couldn’t meet children or be in the same room as the children. Due to this, I had to make some arrangements to instead speak with Tracey about some principles she has seen and that they follow. I met with her for an hour discussing the principles and how they were used in the Head Start program. One of the principles she said was highly important for Head Start was principle number 2. She explained that when there is enough time and opportunities to spend time with a child individually the caregiver puts full attention on one child. She explained that they like to focus attention on a child throughout the day so that the child doesn’t feel left out. She says that alone time for a child with a caregiver is usually tummy time. They allow the infants and toddlers to roll around and look at the carpet designs. Tracey explained to me how they asses each child through this quality time that the caregiver and the child have alone. This quality time the caregiver has with the child is important,
There is a problem in Texas impacting children’s futures; many childcare centers and homes are not providing children with quality care. In an article about cost and quality in Texas childcare, child development experts Susan Eitel and Joyce Nuner quote a study stating “that [nationally] only 10% of infants and toddlers are in high quality [childcare] programs” (34). The term ratio describes the number of children one caregiver is watching. This number is one of the major factors in the quality of a childcare program. Organizations such as the National Association for the Education of Young Children know as NAEYC release recommendations regarding ratios and accredit programs based on their compliance. Childcare centers and homes must comply with certain minimum standards enforced by Texas regarding ratios; however, these standards often set the bar lower than recommendations made by these early childhood experts. The current standards in Texas do not ensure that all young children receive quality care; for this reason, the Texas Department of Family Protective Services should strengthen the minimum standards regarding ratios to better regulate the quality of care children receive.
Over time, the debate of childcare issue within the Canadian public policy context has been raging. Rise in the media attention of the social issue of childcare policy in Canada concentrates on the relationship and coordination among the federal, provincial, and territorial governments in addressing the social issue (Finkel, 20013). According to Lewis Steven of The Star Canada News Agency, childcare is one of the priorities the Canadian Federal Government need to consider when designing its social policies. According to Lewis (2013), the Canadian Federal Government needs to consider the safety net income as an important factor contributing to affordable childcare services in the country. In Canada, parents or members of extended family have the responsibility of providing childcare services without the consideration of family net income. However, outside the family, there are many childcare providers operating as private individuals or agencies. Such childcare providers operate as private businesses.
A substantial amount of research considers how children develop and form attachments from a few months old, as this period can have huge implications later in their life. Children’s experience of attachment will provide them with a schema about how attachments form so it’s critical they have a positive childhood and form close bonds, especially with a primary caregiver. This essay will attempt to assess the impact early daycare has on children’s ability to form attachments and to what extent it affects a child’s development.
The goal of having an inclusive child care setting is that it will succeed, helping each child to grow in every way possible. General Child care settings and inclusive ones are not too different. In order for a child care program to succeed, it must have certain components in it. I consider the staff to be the most important element they are the once that make the big differences. It is their demeanor that set the stage for inclusion. If the staff consist of people that are passionate about making inclusion work they will be positive to the settings that they are in and look at everyone with a smile, they will be willing to fit the needs of each individual child. It is not a one size fit all when working with children with disabilities we as staff must be creative in finding things that work with each child. In this setting being a team
A normal day for a daycare facility is to have many classrooms full of students like a typical public school; however, they are younger children and are being supervised by an adult. That adult is responsible for, on average, four to ten students at any given time, ages ranging from zero to six. During that time, a lot can happen. These busy workers have many responsibilities throughout the day. The most important one is safety. Because these children are so young, they are also adventurous therefore need a lot of attention. Daycare workers have to spend much time supervising these children while performing other tasks throughout the day. Not only do children need to be supervised to be safe, but they also need to have a safe, clean and presentable environment that the daycare workers of the facility are to maintain. One way to keep things clean is to sanitize, in which is a big task of these workers. Children are always putting things in their mouth and then passing it off to other kids; therefore, sanitation is a big deal in these types of facilities. Another responsibility that daycare providers have are to enforce rules. When rules are broken, these providers are to then enforce discipline policies. These can sometimes become an issue seeing that these providers are not the parents of the children thus they have to consult with the parents on certain discipline techniques to use on specific children. On the other hand, these providers are similar to parents in the sense that they wash children's clothing and clean up after every child. In addition to all the other responsibilities, daycare providers also have to maintain current, accurate and confidential client files per child. These files are to consist of observations and r...

These clinical staff will make house calls to United Healthcare clients secured by Medicare who face perpetual, and conceivably costly, conditions, for example, diabetes or congestive heart failure (Triad Business Journal, 2013). It 's a speculation United Healthcare trusts enhances tolerant health through more financially savvy, higher-quality care, and thus, helps the organization 's main concern (Triad Business Journal, 2013). It is expected that the House Calls system will be looking after 75,000 Medicare patients before the current years over (Triad Business Journal, 2013). United Healthcare right now covers around 250,000 seniors in the state with its Medicare Advantage items. Moreover with the strategic plan to hire more nurses and healthcare providers, such as in home visits offer the opportunity to assess the patient’s medication regimens, offer routine physical evaluations and react to any healthcare demands (Triad Business Journal, 2013). United Healthcare plans and suppliers envision such normal, preventive care will take off emergency room office visits or healthcare facility stays for more genuine, and costly, problems in the future (Triad Business Journal,
Each and every daycare has their own menu for the children’s breakfast and lunch. Through my observations, I have noticed that the “Don’t Worry Childcare” has a variety of food selections throughout the month. For example, the children could either choose from fruit loops cereal, or frosted flakes for breakfast. The children’s snacks eaten varies every day as well. For instance, the children ate milk and cookies one day, and cheese crackers another day. An example of lunch would be, milk, chicken fingers, French fries, green beans, and mixed fruit one day, and spaghetti, corn, peaches, and bread another day. I observed the children eat ham, mashed potatoes, green beans, and peaches one day for lunch. All those food options seemed very delicious.
In today's busy society where most homes are dual income families, finding time to take on a full time caregiver position is difficult. Not only will doctor's appointments, grocery shopping, medical bills and finances take up several hours a week, but also the one...
When it comes to individuals that are in need of home care, there’s just no place like home and if given the choice, I’m quite sure we would all rather stay in our own home and receive the care that is needed. However, sometimes people need help to remain at home and at times this care cannot be provided by family and friends. Therefore, a loved one may want to find a facility with a homelike setting in order to make their loved one feel as if they were at home. These facilities that provide homelike settings for their residents at an affordable cost are limited in half the globe. At this point, the main goal for the patient and the caregiver is to slow down the decline of the failing health of the patient.
The group would has chosen to assist the older adults in their community by finding ways to help them improve their health and or remain healthy. The key to the plan is information, and assistance in finding this information. The hope is to implement this health plan by educating the older adult in what is available to them. The hope is have more health care personnel come on board to assist with this plan as it grow and to share the results with others through correspondence with other health care personnel from other communities. The health care plan would first be promoted in a community building where healthcare classes and physical activities programs can be offered in the same community building. Future plan may involve simply in getting the information related to other programs via flyers and or email correspondence. Surveys will be taken at the beginning of the health care promotion and at least every year to gather information in order to decipher if individuals find the information interesting and useful. Upon completion of the surveys it is the hope to have the nurses who have been involved in assisting with this plan to reconvene and plan together what activities need to be changed and or added to best assist in the quality improvement of this health care
