My Journey,Back to Clinical Area I Asia Nazir, having the 11 years of working experience as nursing instructor in PIMS college of nursing, then I took admission in MSc. Nursing program at Shifa college of nursing. Clinical practice is mandatory for completing this program. Therefore, we were assigned to preform Advance Health Assessment clinical in Surgical Intensive Care Unit. Ms. Kalsoom Ramzan our class representative on clinical and faculty Ms. Nuzhat Sultan accompanied us in clinical area, where Dr. Abid Ilyas was assigned as our clinical preceptor.We met with him and Ms. Kalsoom introduced us with all where about of Critical area and with SICU team leader. When I entered in SICU I found myself in new world which was known to me but l felt I lost it with the passage of time due to my changed interests and my job assignments. Initially, it was shocking for me and I thought how I would adjust myself in this environment. Then I encouraged myself to adopt my new role as learner and took this chance as opportunity for my educational development. Today’s clinical experience truly affected me in multiple ways. I went into this day with an open mind, and was pleased with the patients and the way I was able to conduct myself. This clinical affected me because throughout the day I felt that I experienced many emotions. A few times during my day I did have to fight back tears. I felt I had this emotion because some of the individuals expressed how they wanted to get better in order to get home to their families.
1. I think that the patient misunderstood the nurse’s intentions when he was moved from the ICU to the other places in the hospital because there was limited communication letting the know what will be happening. It seems that the patient was not familiar with the hospital, procedures and the people and because of that, the patient felt that all the interventions were to harm him. Even before the surgery, the surgeon visited the patient very quickly and told the patient this was a routine surgery. Although it may be routine, I think that it would be good to educate the patient about what will be occurring in each of the processes. In addition, when the patient was going for exams, the patient had to wonder what exam and why he was getting this procedure. Moreover, I think that he was not oriented to the time or place, and it does not seem like he was reoriented based on his responses.
Overall, I retain three goals for this clinical day: Safely and efficently administer medication, enhance my nursing/CNA skills, and determine how to implement infection control into a health care setting. This week reflects my assigned time to administer medication in a health care setting for the first time, with a resident who retains nearly twenty medications. I except this experience will be a great learning experience, but it will also subsist slightly stressful. With the assistance of my FOR, my goal is to administer all of my resident 's medications without complications. My clinical week was emotional and physically draining this week. I enjoyed being the lead on Thursday because it gave me the opportunity to stop and observe. The nurses and the CNAs were very stressed out, and I clearly saw the effect on the patients. For instance, one of the CNAs asked me to help her with an occupied bed change. I was excited. However, she kept passing a bunch of comments of how hard nursing is and how she did not want to be old. I did not acknowledge any of her comments. Perhaps she thought she could express herself (as a result of her stress) in front the patient since the patient was non verbal and could not understand. I felt very bad. When I was working at long term care, I was being a part of an incident which gave me a life time lesson. I was taking care of ninety-nine years old lady Mrs.Chang. I was assisting Mrs. Chang in transferring from bed to wheelchair. I feel Mrs. Chang physically so weak so I need someone’s sustenance. I was looking for someone and I saw a nurse in next resident’s room who was dressing a wound of resident Mr. Roger. I call the nurse to give me a hand. She rushes into Mrs.Chang room without washing her hand or using hand sanitizer to assist me in transfer. I was jolted for a moment and couldn’t say anything to the nurse. Then I felt it was too late to say anything to the nurse.
In summary using those two different settings is so good in a way that they enhance and motivate the medical students, make them ready for practice, in addition it can influence their career choice in making them aware what kind of speciality they want to be in future. In fact classroom learning alone will not be enough to achieve the competence and the self confidence, teaching clinical skills at early stages during college will have a positive feedback and has potential benefits not only on the students but for other stakeholders.
As mentioned by Hunter and Arthur (2016), one of the main reasons I could maintain and improve my practice was due to clinical placements. During clinical placements we are frequently being assessed and taught in dealing with real situations. Feelings As learning is a lifelong process, I’m sure there are many things I need to improve and learn to better myself. However, the lessons I learned during the course as a nursing student shall always remain as a bedrock for my future development.
For the outcome, Clinical Competence I have learned the importance of the nursing process in my current class, Skills and Concepts. This information is relatively new to me, so I know I have plenty of room to grow in this area. I have learned how to utilize the resources that I am provided. One resource in particular is my pocket guide. This has been a useful tool in helping learn and write a nursing diagnosis based upon a given situation. As I progress through the rest of this class; I hope by the end to be more competent in ways of providing the best possible care while utilizing the nursing process.
Clinical placements allow student nurses to put theory into practice with real patients and actual clinical situations. Every placement will challenge student, improve communication skills, build own knowledge and foster own reflective practice. It is one of the most interesting and exciting aspects of training to be a nurse. I personally believe that, students must be responsible for their own learning opportunities and understand the outcomes they need to achieve on placement, discussing any queries with their clinical supervisor. I always look for learning opportunities. I approach my clinical supervisor for her guidance to perform any new skills fall into my scope of practice.
Before my shift started, I did my research about my new client for week three clinical. I thought I was well prepared for the clinical, I knew the client’s mental and medical conditions but I was more focused on the client’s mental health issues and not the medical illnesses. When the nurse informed me that client W was experiencing shortness of breath due to his COPD, I was a bit shock because I was not expecting that to happen.
I believe placing student nurses in the clinical setting is vital in becoming competent nurses. Every experience the student experiences during their placement has an educative nature therefore, it is important for the students to take some time to reflect on these experiences. A specific situation that stood out to me from my clinical experience was that; I didn’t realize I had ignored the patient’s pain until I was later asked by the nurse if the patient was in any pain.

This reflective essay will discuss three skills that I have leant and developed during my placement. The three skills that I will be discussing in this essay are bed-bath, observing a corpse being prepared for mortuary and putting canulla and taking it out. These skills will be discussed in this essay using (Gibb’s, 1988) model. I have chosen to use Gibb’s model because I find this model easier to use and understand to guide me through my reflection process. Moreover, this model will be useful in breaking the new skills that I have developed into a way that I can understand. This model will also enable me to turn my experiences into knowledge that I can refer to in the future when facing same or similar situations. Gibbs model seems to be straightforward compared to the other model which is why I have also chosen it. To abide by the code of conduct of Nursing and Midwifery Council (NMC) names of the real patients in this essay have been changed to respect the confidentiality.
Prep for clinical practice this semester has been an excellent way to prepare me for my future semesters and career. My understanding of course material and concepts that I studied have definitely improved as a result of my participation, and has also taught me new skills and education relative to the dental hygiene profession. Observing the dental hygiene students in year 2 was also an excellent way to prepare for clinic in semester 3 and for next year. Prep for clinical has been a new way to explore skills to deal with complications that I may come across during my career.
