Reporting party (RP) is a medical assistant for Dr. Seema Sangwan's office who is the PCP for resident Steven Staub (age 62, DOB: 09/06/54). Resident is a paraplegic and dysphagia. Resident arrived with unknown caregiver to the doctors appointment. Dr. Sangwan spoke with the resident privately and resident disclosed that the caregiver was in love with him and wanted him to move in with her. Caregiver wants resident to discontinue home help services and she would provide the services at a lower cost and pay her out of pocket. When resident declined, the caregiver. As the EAI team was discussing Molly’s case, one of the ED Residents made a few telephone calls. Molly’s PCP reported that during her last visit about 2 weeks ago, Mollie was alert and able to respond to questions appropriately. He confirmed that Mollie’s daughter and son in law have experienced psychiatric problems, adding that the son in law has expressed anger regarding Mollie’s living arrangements. The home health care agency was contacted. The RN and aide both report they have never met the son in law and have had very limited contact with Mollie’s daughter. When contacted by telephone, the daughter provided no explanation for Mollie’s extensive bruises noted on admission to the hospital. I cared for a 76-year-old end-staged chronic obstructive pulmonary disorder patient who was admitted for respiratory distress. The doctor requested that my nurse and I get the family together for a family meeting. During the meeting, the doctor communicated to the patient and his family members that the patient will be palliative and no longer be in the ICU. The family members were concerned about the transfer of care to the medicine unit, what to expect from palliative care and other options for care. This scenario did not go well because the patient and family would have benefited from a palliative nurse with expertise, respiratory therapist to discuss other options, pharmacist about medication change if needed, social worker to help guide the family through end of life care for their father. In addition, there was no collaboration with interprofessionals prior to the family
The reporting party (RP) stated her son Michael Girard DOB: 2/13/68 is a resident living in the facility. The RP disclosed the facility does not provide meals that meet the recommended dietary allowance. An example of breakfast would be two donuts and a glass of milk; one hard-boiled egg. The residents were served hot dogs and can soup. The residents are not provided fresh fruits or vegetables. The residents are sent to their rooms at 6PM and receive cookies and soda as a snack. According to the RP the residents don't have access to the food. All the food is locked up and a resident named Dale has the only key. The residents do not have access to the microwave oven to warm their food. Additionally the residents have no access to the television and are forced to watch CNN or programs preferred by the caregivers.

It was a personal conflict between two mutually coexist friends that live together with in the same house. MHP met with the member today. The member report that he walk from Oasis to TWC and was tried and wanted some money to buy him something to eat and drink. MHP told the member she did not have any money to give him. The member states that he needed a referral for clothing. MHP wrote a referral for clothing for the member. The member states that he need a ride back to Oasis shelter but did not want to arrive to the shelter until 5:30 or 6:00 pm. He want to hang out with his friend. MHP told the member that he need to see the doctor to get his injection. Patients seek medical attention from the nursing homes. There nursing homes get a large amount of financial aid on behalf of the government. The financial assistance is given in order to ensure that all the necessary health care facilities are available at the nursing homes. There are few fraud cases that have seemed to occur in the nursing homes. One of the fraud cases that is becoming very common in nursing homes is that the patients are charged wrong amounts for the services that they acquire from the nursing home. The patient generally comes with some disease to seek medical attention. The nursing home raises fraud cases by advising unnecessary tests and procedures to be done on the patients. These tests or procedures may not be required for the patient. As the patient is limited in knowledge, the tests and procedures are done on the patient while charging the patient with a heavy amount of bill. There is wealthy families who are caregivers for a parent, and refuse to rotate the patient because they don’t want to disturb the parent during their sleep. However this is completely against the nurse’s advice, because the patient is covered in bed sores. No one on the patients care team wants to call EPS. Even though the social workers believe they should. Then there is another patient, whose caregiver is taking care of them, at minimal level, but the patient is taken care of. This patients SES is much lower than the previous patient mentioned. There was instantly and EPS call on this patient. The social workers do not believe the call should have been made. Just because one caregiver is wealthy there is no EPS call, but the other patient who is poor had an EPS call made. If Lia’s parents would have been American parents or just different parents, would Neil have made the call? Or did Neil make the call just because of who her parents were? His co-workers believed he made the wrong call because she was in a nurturing family and well taken care of otherwise. Natty said that visits with RM are going well. RM comes frequently. That RM is the most supportive of Natty through this process and the most accepting. Natty still wants to live with RM. Natty does not like living at his paternal grandmother place but better than foster care. Natty has his own room. Natty leaves early and returns late in order to avoid everyone in the home. Natty’s grandmother says offensive comments to Natty and Natty responds by locking himself in his room. In the grandmother’s home Natty’s aunt, uncle, and cousin reside there as well. Natty said that they aren’t supportive of him at this time so he just does his best to avoid all of them. Natty said RF rarely visits. RF comes infrequently. Natty gave us permission to speak with RM around scheduling of appointments.
Counselor met with Pt. to conduct his monthly individual session. Counselor started the session greeting Pt. inquiring how things have been going since the last session and whether the patient has used any drugs or alcohol. Pt. disclosed to counselor that he is doing his best to maintain financially, as well as, to stay clean. Pt. reported that he still using illicit drugs, however, he stopped heroin use. Pt. extensively shared with counselor about how he is trying to stay strong due to his son not living with him and he is under the care of grandparent. Pt. further disclosed that his father-in-law has been refusing to give him his son and he called the cops on him.
