A: MHS assisted the member with applying for SNAP benefits at the SNAP office. MHS and member will visit the local food bank office.
R: The member started the application with her name and date of birth. The member did the same thing with her Medicaid application. She fills out her name and date of birth and expects the MHS ro complete the rest of the application. MHS informed the member that she would assist with the application, but MHS will not do it for help. The member aptitude changes, the member is now upset the MHS will not complete the application. The member went to the restroom and got lost. The member said she forgot which room I was in. The member said “You could have been done by now.” The MHS
Mrs. Hylton is a 45 year old female who presented to the ED via LEO under IVC by her therapist, Melanie, from ADS. Per documentation Mrs. Hylton denies suicidal ideation and homicidal ideation to nursing staff and MCM before the evaluation. She also contracted for safety with MCM. Dr. Horton requested a mental health assessment on Mrs. Hylton. Before the assessment Ms. Melanie and her supervisor Melissa were contacted. Ms. Melissa reports Mrs. Hylton verbally contracted for safety, however left before ADS could type up terms of verbal agreement. Melissa reports afterwards she was not aware of Mrs. Hylton symptoms of psychosis when speaking with her until being informed by Melanie of findings after conversation with Mrs. Hylton. Melanie upon
The Board received a complaint on 04/02/2014 regarding patient Gloria Kinder from Dena Andrews who has a POA for health care matters on the patient. The complaint was regarding Dr. Negron taking over care of the patient after her primary care doctor retired. The complainant states that the doctor would not refill her potassium, did not do follow up labs, and would not care for the patient.
CCIB received SOC 341 from reporting party (RP) stating that resident Sam Bordeaux (DOB: 02/25/40) was sent to the hospital by staff who stated that they could not handle the resident's level 4 dementia and aggression. When the resident was medically stable for discharged from ED, staff at the facility refused to accept the resident back into the facility. RP stated that the facility is not taking the proper channels for eviction and they are not assisting with arranging for higher level of care for the resident.
Samantha agreed to consider this. Once discussing the cost, this counselor then briefly went over all the services that would be in her plan, her rights/responsibilities, and the 7 employment factors to working (VNA). This information is all outlined in her plan. Samantha stated she didn't have any questions or concerns regarding her plan. Counselor then informed her that she would finish drafting her plan and send it to her through email to review one more time, sign, and send back. Samantha stated she would send it back as soon as she can. Once it is received this counselor will enter it into the system, send her a finalized copy in the mail, and complete an authorization for her to received her hearing aids. The reporting party (RP) stated on 4/22/16 she arrived at the facility to visit her mother, resident Maria Reeves DOB: 3/17/25. The RP observed the resident in bed and when the RP approached the bed to sit, the resident winced in pain. The RP disclosed when she sat on the resident's bed she yelled out in pain. The RP revealed the resident was unable to sit up in bed due to pain. The RP stated when the first medication technician entered the resident's room she was informed the resident was experiencing pain to her left hip. The RP stated the medication technician (Resha) made no effort to exam the resident for possible signs of injury. Mother is concerned because she can no longer afford to utilize the General Academic Pediatric Clinic (GAP) services because currently the children does not have health insurance coverage. Mother reported that Veteran Affairs does not provide health insurance coverage for her children because of an eligibly level ranking system. She reported, previously, she paid for services through making a payment arrangement in which was out-of-pocket. However, today she was informed of accountancy services that going forward all clinic visits must be paid in full as a result of a previous 3,000 balance. The Supplemental Nutrition Assistance Program (S.N.A.P.) is a federal program founded in 1964, with the purpose of reducing hunger and malnutrition in the United States. Low-income families and individuals that qualify for this federal assistance can buy food every month in the form of an electronic benefit transfer (EBT). These EBT cards are similar to debit card; cardholder’s recipient can use their cash benefits or buy eligible food at most stores and retailers. Over the years S.N.A.P expenses have dramatically increase. S.N.A.P needs reform due to the never-ending of the amount of waste, fraud, and abuse.
I placed a call to Thomas, who was frantic, but extremely grateful that his wife was safe. After a period of conversation, Thomas related that his suffers from a psychosis metal condition. Thomas related that he checked himself out of the hospital and was unaware Margaret had taken her day’s medication. Thomas requested that I take Margaret to the local hospital for medical evaluation due to her detreating mental status. I then transported Margaret to Jefferson Hospital Crisis Center where I remained until evaluated by medical staff. Thomas was then notified of the hospital and his Margaret’s status. Thomas was very grateful and related that he was responding immediately this morning. No further police action
Thomas reported that she is not working and was approved for social security disability benefits. This worker asked about her own drinking and stated she does not drink. She denied child hood abuse and neglect. Ms. Thomas was about discipline techniques in the home and she state that she takes away toys and TV. She denied the use of physical discipline.
Ms. Lowe stated that she has been working to obtain benefits and Medicaid for the children through social security. She stated that she is in need of medicinal vital information to obtain benefits. This interviewer suggested that she will speak to the caseworker, in attempt to help assist her to achieve these benefits.

The students that I observed in the classroom were of middle to high school. I went to see 8th, freshman, 10th , and seniors classes, they seemed excited and very curious to why I was there. The middle school was more alive and rambunctious while I observed them. The High school kids were more relaxed, more comical. Some were paying attention while others seemed tuned out to the lecture or involved in socialization with friends within the class. By the end of the class Mr. Hasgil had restored the attention of everyone by using tactics such as history jeopardy with candy as the prize with the high school kids. In both he middle school and high school the kids were mostly Caucasian with a mixture of black, Asian , and Hispanic in the classes.
