Psychotherapy is the, “Treatment of emotional, behavioral, personality, and psychiatric disorders based primarily on verbal or nonverbal communication and interventions with the patient, in contrast to treatments using chemical and physical measures." (medilexicon.com) Within psychotherapy there are multiple types of therapy that are under the term psychotherapy, one of those being Behavioral Activation Therapy. Behavioral activation comes from the work of Peter Lewinsohn. When a person has preexisting vulnerability with added amounts of stress, that can lead to depression disorder. With depression there is a low mood, causing one to withdraw from the world, which leads to a low mood and further withdrawing and its a vicious cycle that is difficult to rid. In depression people exhibit ruminations and avoidance behaviors. Ruminations is when one thinks about a situation and if after two minutes you are still in the thought that everything is hopeless and sad are having rumination. In behavioral therapy people learn to stop using avoidance behaviors and rumination. Then the therapist and client work together to come up with action plans to aid with their disorder. Behavioral activation therapy helps with depression and anxiety, it can be used to help people with their disorder over come and be a fully functioning person. The therapist coaches the client, and work together to develop a plan, steps that need to be taken, and homework or activates for the client. These activities/homework are based on the core principals of Behavioral activation therapy. DBT is effective when working with clients experiencing anxiety disorder and depression. Individuals in DBT therapy are taught to notice, rather than react to thoughts and behaviors. DBT teaches clients to accept their emotional reactions and learn to tolerate distress while being mindful of their present experiences. DBT has four stages for therapy. In stage one the pre-commitment stage is where the therapist explains what types of treatment the client will receive. In this stage the client must agree to stop all self harm behavior and work toward developing other coping skills. In stage two the goal is to assist the client into controlling her emotions. Stage three and four involve assisting the client to gain the ability to develop self respect (Waltz, 2003).
A person who is depressed will see everything in a negative way, the person won’t have any pleasure in life and the person will be counterproductive having trouble completing the simplest of tasks. The depressed patients will be extremely self-critical as they seem to view themselves as a looser and that no one likes them or the feeling of being undesirable to another person and that they are unable to be loved. The depressed patients will also have Masochistic Dreams where the themes would be the same as their waking life and they were distorting situations from this belief that they had about themselves that they are a looser or inadequate. When the depressed person believes this about themselves it leads to misinterpretations, and when they change these misinterpretations, by looking for the evidence to see if the way they view themselves is logical or by looking for alternative explanations the person would normally see the flaws in their thinking and gradually get better. Dialectical Behavior Therapy (DBT) is a comprehensive cognitive-behavioral treatment developed by Marsha M. Linehan for the treatment of complex, difficult-to-treat mental disorders. Originally, DBT was developed to treat individuals diagnosed with borderline personality disorder (BPD; Carson-Wong, Rizvi, & Steffel, 2013; Scheel, 2000). However, DBT has evolved into a treatment for multi-disordered individuals with BPD. In addition, DBT has been adapted for the treatment of other behavioral disorders involving emotional dysregulation, for example, substance abuse, binge eating, and for settings, such as inpatient and partial hospitalization. Dimeff and Linehan (2001) described five functions involved in comprehensive DBT treatment. The first function DBT serves is enhancing behavioral capabilities. Secondly, it improves motivation to change by modifying inhibitions and reinforcement. Third, it assures that new capabilities can be generalize to the natural environment. Fourth, DBT structures the treatment environment in the ways essential to support client and therapist capabilities. Finally, DBT enhances therapist capabilities and motivation to treat clients effectively. This paper will provide background information about the intervention, address the target population, and describe program structure and key components. It will also provide examples of program implementation, challenges/barriers to implementing the practice, address how the practice supports recovery from a serious mental illness standpoint and provide a summary.
