The practice of medicine has been characterized as of late by a departure from the professionalism and humanism that once acted as the basis for all medical care. The current medical model of education and training, as well as an increase in technological reliance and the overburdening of healthcare workers has generated a shift in how practitioners behave in the medical setting. As a student of the PA profession, much of my success as a healthcare provider will be determined by the success with which I perform my technical responsibilities and generate meaningful interactions with my patients. The virtues of professionalism and humanism should be exemplified in all medical professionals’ approaches to patient care, yet often, as was depicted in the film “The Doctor,” are often under-exemplified in the medical community. Professionalism can be aptly described as the altruistic dedication of a provider towards promoting the health and overall well-being of their patients. Humanism transpires when a provider is able to empathize with their patients, perceptive and appreciative of their unique concerns during their state time of health or ill-health. In “The Doctor,” Jack began his journey as a callous physician; arrogant and disengaged from his patients, concentrating his attention on the disease processes of his patients rather than their psychosocial needs. While he was technically successful in his professional as a surgeon, he did not exemplify humanism or professionalism. In the earlier stages of the film, Jack can be heard saying, "When you've got 30 seconds before some guy bleeds out, I'd rather cut more and care less." This attitude reflects not only his lack of humanism but also that of many other physicians and h... ... middle of paper ... ...ated a generation of providers who place all of their trust and reliance in the technologies and procedures that they utilize. They have dissociated themselves from the provider-patient bond, so much so, that they are unable to be receptive to their patients. Tenets such as humanism and professionalism, however, are vital to the effective practice of medicine and can counteract the disengaged culture of care. This film was an important reminder that providers are entrusted with the duty to observe, listen, and use their knowledge in a meaningful way. By listening to the patient and keeping their needs at the forefront of everything that we will do as providers, we will ensure that we are always practicing to the full extent of our capabilities. Patients will not struggle to receive effectual care, as they will be reinstated as the core of the healthcare experience.
In the healthcare system many times patients are just patients and appointments are just appointments. The outlook on the patients and appointments all depends on the area of practice and the health professional themselves. Working in the emergency department, the nurses and doctors there typically do not see the same patient more than once and if they do the chance of them remembering them is slim to none just for the simple fact of the pace of the department. When it comes down to Physicians in the hospital setting, the care is not just quick and done. Great patient to healthcare professional relationships are formed and for some it may feel as if they are taking a “journey”(209) with their patients as they receive their medical care. This essay will be based off the book Medicine in Translation: Journeys with My Patients by Danielle Ofri, in which Ofri herself gives us the stories of the journeys she went on with several of her patients. Patients are more than just an appointment to some people, and when it comes to Ofri she tends to treat her patients as if they are her own family.
When we see patients, we must remember that we are not simply treating a disease. We are caring for people with lives, hobbies, jobs, families, and friends, who are likely in a very vulnerable position. We must ensure that we use the status of physicians to benefit patients first and foremost, and do what we promised to when we entered the profession: provide care and improve quality of life, and hopefully leave the world a little better than it was
Diligence is a virtue. This is a theme Atul Gawande presents to the reader throughout Better: A Surgeon’s Notes on Performance. In each story, Gawande provides insight on medical studies he has previously embarked upon. For example, in “The Mop-up” the author tells us about a time when he went to India to observe the efforts to eradicate polio. Gawande explains how he followed a supervisor around and how vaccinations were performed. Additionally, in another chapter he debates on whether physicians should take part in death sentences. Throughout his adventures Gawande provides numerous enriching personal accounts of controversial events and what it is like to be a doctor; each with diligence playing a key part.
This internal conflict is a result of the mistakes a physician makes, and the ability to move on from it is regarded as almost unreachable. For example, in the essay, “When Doctors Make Mistakes”, Gawande is standing over his patient Louise Williams, viewing her “lips blue, her throat swollen, bloody, and suddenly closed passage” (73). The imagery of the patient’s lifeless body gives a larger meaning to the doctor’s daily preoccupations. Gawande’s use of morbid language helps the reader identify that death is, unfortunately, a facet of a physician’s career. However, Gawande does not leave the reader to ponder of what emotions went through him after witnessing the loss of his patient. He writes, “Perhaps a backup suction device should always be at hand, and better light more easily available. Perhaps the institutions could have trained me better for such crises” (“When Doctors Make Mistakes” 73). The repetition of “perhaps” only epitomizes the inability to move on from making a mistake. However, this repetitive language also demonstrates the ends a doctor will meet to save a patient’s life (73). Therefore, it is not the doctor, but medicine itself that can be seen as the gateway from life to death or vice versa. Although the limitations of medicine can allow for the death of a patient to occur, a doctor will still experience emotional turmoil after losing someone he was trying to
MaKee is presented in the film as a successful and well-respected doctor, quite adept at his surgical skills who, along with his colleagues, performs procedures on and glibly interacts with patients, referring to them as diagnoses rather than people. Jack’s relationships with his wife and son appear to have suffered due to Jack’s work schedule and time away from home. Jack’s wife seemingly has created a personal and professional existence devoid of Jack’s presence, while Jack’s son interacts only with his father via phone conversations. Though Jack lives in a beautiful home, providing amply for the basic needs of his family and himself, Jack’s life consists only of long and frequent workdays, with a history of family neglect on a personal level, complete with the repeated disregard of important events for both his wife and son.

Although some individuals don’t see the importance of communication and emotional connection with patients in the medical field, doctors who have problems properly interacting with their patients will have a lower chance of success in healing them. Doctors receive so much education but are never taught proper bedside manners, which is the way that physicians interact with patients. In order to ensure a patient’s comfort, psychological well-being, and physical health, a physician must truly understand their patient.
Traditionally, the physician was expected to use all of their talents and training in an effort to save the life of their patient, no matter the odds. More recently, the physician’s role has been redefined to preserve the autonomy of the patient. Now physicians must give life saving care only in so far and to the degree desirous of the competent patient.
The demand of a constantly developing health service has required each professional to become highly specialised within their own field. Despite the focus for all professionals being on the delivery high quality care (Darzi, 2008); no one profession is able to deliver a complete, tailored package. This illustrates the importance of using inter-professional collaboration in delivering health care. Patient centric care is further highlighted in policies, emphasising the concept that treating the illness alone whilst ignoring sociological and psychological requirements on an individual is no longer acceptable. Kenny (2002) states that at the core of healthcare is an agreement amongst all the health professionals enabling them to evolve as the patient health requirements become more challenging but there are hurdles for these coalitions to be effective: for example the variation in culture of health divisions and hierarchy of roles. Here Hall (2005) illustrates this point by stating that physicians ignore the mundane problems of patients, and if they feel undervalued they do not fully participate with a multidisciplinary team.
It is quite obvious that morals, ethics and common courtesy are not enough to encourage the respect of patients in the educational atmosphere, as is seen in the story. I believe it is the responsibility of the medical school to encourage their teachers to demonstrate ways to connect with patients rather than just teaching the anatomy of health care. Teachers are supposed to be role models for students and if they are not taught to treat patients with respect, the only way they can learn that kind of skill is the hard way; through the loss of patients because of their feelings of irrelevancy at the doctor’s office, or through the complaints of people who are unsatisfied with their quality of health care.
Effective interactions between radiologic personnel and patients can make the patient’s visit pleasant and meaningful (Adler & Carlton, 2012). To interact effectively with patients, understanding the patients’ needs and concerns are important. Fields et al. (2011) illustrate that empathy is an important component of professionalism, and is mutually beneficial between medical staff members and patients across all health professions. Sim and Radloff (2009) assert that altruism is an important attribution of professionalism, and that it is the responsibility of the healthcare professional to place patient interest and well-being over monetary gain. Therefore, radiologic professionals acting altruistic and emphatic with patients leads to better patient compliance and fulfillment. As with most medical professionals, radiologic professionals have a great responsibility when interacting with patients. Patients tend to view medical staff as powerful and omniscient (Adler & Carlton, 2012). According to Yielder and Davis (2009), this dominance derives from the extraordinary knowledge, techniques, and skills of the medical staff. As a result, patients rely on professional’s competence to provide high quality healthcare. Professionalism is defined as the accrued knowledge and behavior possessed by individuals in order to obtain successful goals and attributes for their organization, facility or work environment (McDonagh, 2008). Being professional while at work let others know that you are reliable, respectful, and competent at what you do. Professionalism in a medical office is very important, it means putting patient needs as top priority. Professionalism in a medical office is taking responsibility of your patients, committing to patient confidentiality, and having patient-centered care for the patients.
Professionalism is an adherence to a set of values comprising both a formally agreed-upon code of conduct and the informal expectations of colleagues, clients and society. The key values include acting in a patient's interest, responsiveness to the health needs of society, maintaining the highest standards of excellence in the practice of medicine and in the generation and dissemination of knowledge. In addition to medical knowledge and skills, medical professionals should present psychosocial and humanistic qualities such as caring, empathy, humility and compassion, as well as social responsibility and sensitivity to people's culture and beliefs. Today, many Americans face the struggle of the daily hustle and bustle, and at times can experience this pressure to rush even in their medical appointments. Conversely, the introduction of “patient-centered care” has been pushed immensely, to ensure that patients and families feel they get the medical attention they are seeking and paying for. Unlike years past, patient centered care places the focus on the patient, as opposed to the physician.1 The Institute of Medicine (IOM) separates patient centered care into eight dimensions, including respect, emotional support, coordination of care, involvement of the family, physical comfort, continuity and transition and access to care.2
The movie “The Doctor” is a good example of how communications in the health field work to benefit not only the patient, but the doctor too. In this movie, the main character, also known as Jack McKee, is a heart surgeon. The movie begins by showing how McKee’s attitude towards his patients tends to be inappropriate. Jack jokes about his patients and laughs at their concerns. His home life is also a struggle; his relationships with his wife and son are falling apart. The movie takes a turn when Jack becomes suddenly ill. He begins coughing up blood. He meets with a specialist by the name of Lesley. Tests reveal that Jack has a serious tumor on his vocal cords. He has now become the patient. He begins treatment but the results are not what they expect. Along the way, he befriends a brain tumor patient by the name of June. She will teach Jack how to empathize. He will learn how to feel and communicate not only with his wife and son, but for his patients as well.
Deprofessionalization of medicine affects the way those of the medical profession interact with patients greatly. Healthcare providers are forced by law to violate the values that make them who they are because of the request of customers demanding goods and services in the free market. This signals the end of medicine as a professional practice. It makes medicine just another exchange of goods and services as well as putting patients in the role of customers, ordering whatever they want from physicians.
