I have been a Nurse Practitioner (NP) for ten years. My co-worker asked me if I am interested in pursuing DNP and I have been holding the thoughts of going back to school for five years. In my early career as a novice nurse, I was involved on direct patient interaction and care. A nurse is capable of helping patients understand their medical conditions, educate them and their families on diseases, and provide input on healthy behaviors. I am delighted in my career as a professional nurse and seeking the Doctor of Nursing Practice (DNP) gives me the opportunity to expand my knowledge base in order to practice at a higher level, and obtain the terminal degree in my profession. My goal is to become an adult nurse practitioner, with primary focus to improve the practice of nursing. The Doctor of Nursing Practice will equip me with knowledge and skills needed to understand and appreciate research and facilitate the process of putting evidence into practice, with the overall goal of achieving improve patient safety, satisfaction and outcomes. The Doctor of Nursing Practice will enable me to assume more leadership role, in practice and in research. I should be able to participate in research and formulate nursing models guided by evidence based practice models of care. Having practiced as a professional nurse for five years, obtaining the Doctor of Nursing Practice gives me the opportunity to foster my professional growth and development.
Nursing, as a crucial part of the health care system, keeps evolving while dealing with an increasingly complex clinical situation that involves quality of patient care. Therefore, to successfully handle these scenarios, nursing needs to become more advanced, clinically and academically. A DNP degree is needed in this situation because according to Dunbar- Jacob, Navito & Khalil (2013), a DNP degree is considered to have more impact on nursing than a Master’s or Bachelor’s degree due to the advanced clinical education a DNP provides (p. 425). Hence, the DNP degree has an enormous impact in nursing practice especially in terms of improving clinical care, promoting leadership roles, and improving educational status.
Racial disparities in The United States health care system are widespread and well documented. Social and economic inequalities between racial minorities and their white counter parts have lead to lower life expectancy rates, higher infant mortality rates, and overall poorer health for people of color. As the nation’s population continues to become increasingly diverse, these disparities are likely to grow if left unaddressed. The Affordable Care Act includes various provisions that specifically aim to reduce inequalities for racially and ethnically marginalized groups. These include provisions in the Senate bill and House bill that aim to expand coverage, boost outreach and education programs, establish standards for culturally and linguistically appropriate practices, and diversify the health care workforce. The ACA, while not a perfect solution for eliminating health disparities, serves as an important first step and an unprecedented opportunity to improve health equity in the United States.
Licensed practical nurses (LPN 's) fill an important role in modern health care practices. Nursing is a knowledge-based profession within the health care sector that focuses on the overall care of individuals. According to The American College of Nurse Practitioners (ACNP), “defines nurse practitioners as registered nursed who have received graduate-leveling nursing education and clinical training, which enables them to provide a wide range of preventative and acute health care services to individuals of all ages. They deliver high-quality, cost effective care, often performing physical examinations, ordering tests, making diagnoses, and prescribing and managing medication and therapies”. Nurse Practitioners are able to specialize in a particular area, such as family and adult practice, pediatrics, and women’s health; and refer patients to other specialist when necessary. Some Nurse practitioners work under the supervision of a physician; while others run their own practices.
The ACA expanded Medicare/ Medicaid, strengthened employer based care, and included an individual mandate. Before the ACA there were 32 million people uninsured and “approximately half, or 16 million, will gain coverage through an expansion of Medicaid” (Barr, 2011, p. 292). To improve the cost of care the ACA required employers with more the 50 employees to offer plans and individuals would have to purchase plans from the government. “ACA does not address directly the issue of disparities in access of care based on a patient’s race or ethnicity, it does impose on providers the responsibility for collecting data on the race or ethnicity, primary language, disability status, and similar demographic characteristics of patients cared for” (Barr, 2011, p. 293). The ACA strives to give health coverage to all but the power still lies in the private sector.
One of the most prevalent and pervasive social issues in the United States today is the provision of equal access to health care for the impoverished. Far too many people live in conditions of poverty and struggle to find the means by which to meet their basic needs. For those without insurance, access to medical care is often preempted by other necessities. An unexpected medical expense can push this group further into poverty. Those who do have insurance may find themselves underinsured in the event of an emergency and unable to make the necessary co-payments. Alternatively, the insured’s provider may refuse to cover certain conditions. Besides the cost of adequate insurance and the booming cost of medical care, there are other factors that affect equal access to medical care for the impoverished. Among these are race, age, and geographic location. Poverty and the resulting inadequate medical care is a ubiquitous social problem that merits further discussion of the issue’s causes and implications.
Healthcare is a continuous emerging industry across the world. With our ever changing life styles and the increased levels of pollution across the world more and more people are suffering from various health issues. Once upon a time, my best friend, Bryan Martinez, often heard his mother’s medical conversations with friends. One day at school, our teacher confronted Mrs. Martinez and told her that she was able tell that Bryan was a son from a nurse. Apparently there was an incident at school where a little boy was acting out and Bryan told our teacher that the little boy was agitated, and to give him some medication to calm him down. Implementing care plans within legal, ethical, and regulatory parameters is a competency that all registered nurses but abide by. As stated before as a baccalaureate nurse you must include not only patients and their families but also the community and population (The Texas Board of Nursing, 2011). Following the nursing process the next competency include evaluate the results of the implementations that have occurred. Once again the biggest difference between the two degree plans is baccalaureate nurses will also include the community and population as well as the patient and their family (The Texas Board of Nursing, 2011). Education is a vital piece of nursing and must be completed at every possible opportunity. Educated patients and their families on promoting health and marinating health is a very important concept. Expanding this education to the community and population is a vital step in helping reduce risk for our patients (The Texas Board of Nursing, 2011). The last competency is the nurse’s role in coordinating human information and material management resources for patients and their families as well as the expansion to include communities and populations as ones transition to a baccalaureate nurse (The Texas Board of Nursing, 2011).
