Overall, there is strong evidence that fatigue associated with extended work schedules is related to adverse events and errors in patients and healthcare workers. Adverse events in this study included needlestick and sharps injuries, work-related employee injuries, patient falls with injury, and nosocomial infections. In 2011 The Joint Commission called attention to healthcare worker fatigue and the impact it was having on patient safety. They found a direct link between healthcare worker fatigue and adverse events. They recommended healthcare facilities assessed their policies to identify fatigue-related risks, such as off-shift hours and consecutive shift work, and review their staffing to address areas that may be contributing to nurse fatigue (Martin, 2015).
A considerable amount of literature has been published on the impact of working hours (8 vs. 12 hour shifts) on fatigue among the nurses. These studies revealed that twelve-hour shifts increase the risk of fatigue, reduce the level of alertness and performance, and therefore reduce the safety aspect compared to eight-hour shifts (Mitchell and Williamson, 1997; Dorrian et al., 2006; Dembe et al., 2009; Tasto et al., 1978). Mills et al. (1982) found that the risk of fatigues and performance errors are associated with the 12-hour shifts. Beside this, Jostone et al. (2002) revealed that nurses who are working for long hours are providing hasty performance with increased possibility of errors.

We have all experienced a jet-lag before, which means we feel tired or even sick because of the time zone change that we have experienced during traveling. This change of sleeping pattern also occurs in the daily lives of those who work with irregular working hours. In hospitals it is very common to change shifts from day to night time due to the fact that hospitals are needed all day around, every day of the year. The staff members mostly take turns with the night shift, which is of course not an arbitrary decision. Due to the increasing demand for flexible working hours, there is also an increase of concerns in safety and productivity regarding the employees. This analysis discusses certain mental and physical health problems that occur with shift-work in hospitals and how these effects can hinder performance. Furthermore I will compare theory with real life practice in the field of shift work.

The purpose of this paper is to discuss the National Patient Safety Goals (NPSGs) put out by The Joint Commission that went into effect January 1, 2014. The goal I chose to focus on is the first goal, improve the accuracy of patient identification. The element of performance within that goal I am going to concentrate on is to use at least two patient identifiers when administering medications (Joint Commission, 2013). The importance of this goal cannot be understated. Correctly identifying a patient prior to administering a medication is imperative to patient safety.

As it has been mentioned, this schedule results in better work-life balance and allows nurses to engage in other social and economic activities (Stimpfel, Sloane, & Aiken, 2012). However, much care should be taken while implementing any schedule. Health care administrators should be cautious and be able to identify fatigue-related risks that result due to extended shifts. Fatigue may compromise the quality of care delivered to patients. The work hours should be limited to 12 as this encourages flexibility and effectiveness in the nurses performing their duties.
