The patient education brochure that I choose for this assignment connects colorectal cancer screening and scheduling colonoscopies. It was designed as an educational instrument as a health prevention tool by Indiana University Health for adult men and women. This brochure is specifically targeted for adult women and men, aged 50 and older with the special considerations of African American population beginning at age 45 and those who have had a family history of colon cancer to screen 10 years prior to when the family member had precancerous polyps, cancer or the age of 40 whichever comes first (Indiana University Health, 2012). Three samples of 100-word passages were evaluated to determine the reading level of this brochure (Doak, Doak, & Root, 1996). The first 100-word passage referring to the illuminating of what the lifetime risk includes contained 5.6 sentences and 152 syllables, which indicate an estimated 9h grade level for readability. The second 100-word passage correlating to how the exam is preformed contained 5.5 sentences and 145 syllables, which indicate an estimated 8th grade for readability. The third 100-word passage related to preparation and procedure contains 6 sentences and 171 syllables, which indicate an estimated 15th grade readability. Cancer is one of the 9 National Health Priority Areas (NHPA), areas which account for a significant portion of the burden of disease, but have sizeable potential for improvement. In Australia, CRC is the second most common cancer, after prostate (in men) and breast cancer (in women) (AIHW, Cancer incidence projections). The incidence has gradually increased (by 13% in males from 1982-2007) (AIHW, Cancer in Australia an overview). This is compounded by the ageing population and population growth, with 14,860 new cases in 2010 (http://www.cancer.org.au/about-cancer/types-of-cancer/bowel-cancer). Colorectal cancer, or CRC, affects African American men and women more than Caucasians, at a rate 20% higher. This is concerning when faced with the mortality rates among African Americans, 28% higher for women and 14% higher for men than for Whites. African Americans are also more likely to be in later stages of the disease when diagnosed. There is a need to study and evaluate why these factors exist, as proper screening and early diagnosis can severely impact survival rates for CRC. One study attempts to find the solution through testing, however, this study slightly discredits itself along the way.
Personally I understand that critical thinking is a way of using several concepts to identify solutions and determine a course of action to solve patient’s care problem. Every patient that comes to the hospital is looking up to the health care team to solve the problem with which they are presenting; therefore the nurse must be able to explore all avenues to solve their care problems.
Recognizing that I need to maintain my body weight by exercising at least 3-4 times a week for an hour would be beneficial to avoiding my risk for disease. Also, the nurse should recommend that I never start smoking and limit my alcohol consumption. An example of secondary prevention against colorectal cancer would be for my father and I to receive screenings and colonoscopies as early and as often as possible (CDC, 2016).

The assessment includes a brief manual which appears to be written for a clinician to conduct. It gives directions on how to administer and score the items. The test kit also includes answer sheets and a computer scoring package. The test is also cohesive with the other assessment tests developed by Beck and they results can be easily combined with one another.
I agree that there is a lot of challenging information in the guidelines preceding the E/M section of the CPT manual. I found the videos in the UMA Academic Resources section very helpful to me in comprehending how to interpret the codes in this section.
