The necessary resources included technology and space for the Nursing Student to collect data and the expertise of the various stakeholders. The space for the student was readily available, and communication between the student and clinic staff was excellent throughout the duration of the project. The Pharmacists were able to identify the reasoning for faxed requests, which included controlled substance prescriptions, 2nd or greater requests, and a mismatch between the name associated with the prescription and the name of the provider the request was being sent to. Another barrier was with a specific pharmacy and single provider, which included a mismatch between the name in the pharmacy’s system and the clinic’s system. Another example of successful collaboration was between the Medical Assistants and the Nursing Student, and the Assistant Director of the clinic. The Assistant Director helped educate the Medical Assistants about the survey project, its importance, and their role. The Nursing Student was then able to distribute the surveys to the Medical Assistants, explain the process, and answer any questions. Although there were some barriers to distributing to the surveys to patients, there was minimal resistance from the Medical Assistants.

The key stakeholders for this system change, and to help implement the strategy on providing new patient navigators would be the financial director, chief nursing officer, floor nurses, the hospitalists, and a group of patients and their family. Identifying the key stakeholders is important because with providing new services to a health care facility this group of people will be responsible for accepting the strategy to put in place which includes adding a new job title, approving the salary and the number of people to be hired, on down to how each navigator will be trained and oriented. Although the patients and their may not have much choice in the beginnings of the process of the system change, they can have a say and impact on helping in figuring out the role, and where there are gaps in the care during stays at the hospital, as well as helping in the interview process.
According to ASHA Evidence Based Practice is the combination of clinical expertise opinion, data, and patient’s perspectives, with the goal of providing high-quality services (2013). The process of evidence-based practice consists of formulating a research question, collecting evidence, including views, and then evaluating the entire process. This project introduces the research topic I’ve chosen, which identifies a researchable problem, and formulates an answerable question that is relevant to nursing and evidence-based practice.
