Background
Nursing case management as a specialty is facing a number of workforce challenges. A gap analyses can identify differences between the current state of on the job training and education for case managers and the challenges and opportunities for improved efficiency in the role.
Objectives
Performing a gap analysis by comparing current case management standards of practice, role delineation and function studies, and best practices with on the job training and educational processes currently in place to develop new and experienced case managers while making recommendations to address any educational deficits identified in the gap analysis.

A gap analyses can identify differences between the current state of on the education for case managers and the challenges and opportunities for improved efficiency in the role. Such analyses provide the opportunities to clarify role confusion for aspiring nurse case managers.
Problem Statement To keep pace with the rapidly changing healthcare environment and the workforce challenges that nurse case managers face, nurse leaders and educators must identify a clear set of competencies, and the gaps between the current state of nurse case management education and the desired best educational practices and case management curriculum, and competency assessment, to ensure nurse case managers can perform their jobs effectively.
Purpose
The purpose of this capstone project was to determine what educational approaches and curricula are currently being offered to train and educate new and practicing nurse case managers, as well as to identify a clear set of core competencies for the nurse case manager, current gaps in education, and the knowledge, skills, and attitudes (KSAs) required of nurse case

Many health care professionals are wondering why shortage transpired when managed care cost initiatives, implemented throughout the country, are dramatically decreasing the length of patient stays (Upenieks, 2003). In fact, such a situation should be resulting in a nursing oversupply. As the nursing shortage ensues, the need for recruiting and retaining highly skilled nurses committed to the organization will become necessary to maintain high-quality patient care. The recent national nurse shortage has resulted in higher nurse workloads; fewer support resources, greater nursing dissatisfaction, and burnout, making it more difficult to provide optimal patient care (Upenieks, 2003). The primary role of nursing is to provide the best possible care to patients.
The nursing shortage is divided into four different categories. The four categories are as follows; "willing nurse" shortage, funding or perceived funding shortage, shortage of understanding that nurses are needed to deliver care, and nurse education and empowerment shortages (What is the nursing shortage and why does it exist?., 18 October, 2007). To be able to repair this major problem, all four segments of shortages need to be addressed. The first nursing shortage, A "willing nurse" shortage, is the simple fact of not enough supply to fill the demand of nursing positions. This shortage occurs either because there are simply not enough nurses to fill the open positions, or because experienced nurses are opting out of nursing and the willingness to provide care due to the current occupational environment. The National Organization of Nurse Practitioner Faculties (NONPF) describes essential guidelines required for all Advance Practice Nurses (ANPs) to enter practice, regardless of their specialty (2010). A nurse manager plays an important role on a hospital unit. Evans defines the role of a nurse manager as one who makes sure all the needs required on a daily basis are accomplished (Evans, 2011). Evans goes on to say that one primary responsibility of a nurse manager acting in the position of a leader is to “raise the level of expectation and help employees reach their highest level of potential excellence” (Evans, 2011). With this said, it is important to identify potential barriers and problems that a nurse manager would face on a given unit and create or adopt evidence-based interventions to eliminate these problems. When this is accomplished, it will help to foster a work environment that maintains safety to patients as well as staff.
Current literature continues to reiterate the indicators of a major shortage of registered nurses (RNs) in the United States. The total RN population has been increasing since 1980, which means that we have more RNs in this country than ever before (Nursing Shortage). Even though the RN population is increasing, it is growing at a much slower rate then when compared to the rate of growth of the U.S. population (Nursing Shortage). We are seeing less skilled nurses “at a time of an increasingly aging population with complex care needs and an increasingly complex technological care environment” (Mion). According to recent data from the Bureau of Labor Statistics and the Department of Health and Human Services, it is estimated that “more than a million new and replacement nurses will be needed over the next decade” (Diagnosis: Critical).
Clinical Nurse Leaders’ responsibility scope includes all nurse and patient management details except for administrative duties. They are important resources in United States medical intuitions. CNL candidates must have a master’s degree to fulfill this position. This knowledge gives them the needed skill set to manage the daily operation of the hospital’s entire nursing pool. Fordism takes its name from the mass production units of Henry Ford, and is identified by an involved technical division of labour within companies and their production units. Other characteristics of Fordism include strong hierarchical control, with workers in a production line often restricted to the one single task, usually specialised and unskilled. Scientific management, on the other hand, "originated" through Fredrick Winslow Taylor in 1911, and in very basic terms described the one best way work could be done and that the best way to improve output was to improve the techniques or methods used by the workers. There are several nurses that have established themselves successfully in the work I am pursuing. The work mainly involves care management, client teaching and patient advocacy. Care management focuses on ensuring that the care that various health care providers offer is cohesive to meet the patients’ needs. Client teaching deals with enabling patient gain knowledge about the medications and health. Patient advocacy is involved with protecting and advancing patients’ rights.
Several years ago in 2008 the RWJF and the IOM collaborated for two years to discern the future needs of the nursing profession. Most importantly, the objective was to outline the critical actions needed to ensure nursing was ready to seamlessly move towards the future. This was no easy task as nurses work in such diverse settings such as outpatient areas, acute care settings, the community, and long term settings to name a few. Couple this with the fact that nurses have a variety of educational avenues such as the associates, diploma, or bachelor’s degree open to them to achieve the status of registered nurse (Institute of Medicine, 2010). All of this considered, the committee did design four key messages regarding the future of nursing as key in the transformation of health care as evidenced in their "Future of Nursing" report.
