Necrotizing fasciitis, known commonly known as "flesh eating bacteria [infection]", occurs in a wide range of people 1. It occurs in the elderly, middle aged and younger patients. It occurs in athletes and debilitated individuals. It can occur in drug abusers with self inflicted wounds and healthy individuals with incidental injuries. It may occur in those with metabolic disorders such as diabetes and those with suppressed immune systems. It may occur in those with no underlying disorders and no known particular injury. It occurs under ordinary circumstances such as a seemingly harmless cut or scrape and in wounds resulting from major trauma such as an auto accident.
There is no single way of contracting necrotizing fasciitis. One patient reported that he caught it while he was at the beach and may have scraped his skin. There are some cases that have been reported after bug or dog bites. We have also seen confirmed drug users who got the infection but declared they didn’t know how the bacteria could have entered their body. There have been numerous cases where injection of illicit drugs can produce infections that present with signs of a simple cutaneous abscess and yet unpredictably evolve into an extensive necrotizing soft tissue infection 1.

Necrotizing fasciitis is a deep infection of the subcutaneous tissues that results in progressive destruction of fat and fascia. The progression is usually rapid and, as an example, this author has seen it spread throughout one leg in a little over one hour. Of course, this has an instant effect on the blood supply to these areas. Alternative names for necrotizing fasciitis are fasciitis-necrotizing; infection-necrotizing subcutaneous; subcutaneous infection-bacterial5. Untreated and unconstrained, it can destroy massive amounts of tissue and result in amputation and even death in a short period of time.

Necrotizing fasciitis is caused most commonly by Streptococcus pyogenes, group A streptococcus, which is the same bacteria that causes common strep throat 8.
Plantar fasciitis is caused by muscles and ligaments that alter the calcaneous (the big bone on the foot) (Daniels and Morrell 2012). Alteration of these muscles and ligaments will inflict pain and discomfort on the patient, and if not treated will cause failure of ligaments, bones, and muscles. Brucellosis remains the most common and serious problem in some parts of the world.1 Many of Brucella species could infect animals through direct contact. If left untreated, streptococcus pyogenes can also cause post-streptococcal glumerulonephritis, or a disease that causes inflammation of the kidney to occur following a strep throat infection. Although this is rare, standard precautions must still be taken to prevent these repercussions. Staphylococcus aureus and streptococcus pyogenes are the main causes of impetigo, but they are not the only.

This parasite is spread through the bite of sandflies. There are three different types of infections and they each show varying degrees of severity. The cutaneous form produces mild skin ulcers, mucocutaneous produces ulcers in the mouth and nose, and the visceral form of the disease starts with skin ulcers and then fever, low red blood cell count, and an enlarged spleen and liver. The parasite is detected by a microscope and visceral can also be found by doing blood tests. 12 million people are in infected in 98 different countries and 2 million new cases are found every year. The disease also kills around 20 to 50 thousand people a year.
Bacillus anthracis or anthrax is bacteria that infect thousands of people a year worldwide. Mostly found in the hinds and hair of grazing animals like cattle and sheep, the bacteria can stay dormant for years until contacted. Contact can occur through broken skin, eating infected meat, or inhaling infected dust from hides through the mouth or nostrils. Normal contact through the skin begins forming malignant skin ulcers that are dark black in appearance. If left untreated, the blisters lead to blood poisoning and eventually death. Eating contaminated meat not fully cooked can cause intestinal infection also leading to death. Surprisingly, many people survive contact of the skin and very few cases of intestinal infection exist. However, inhalation of infected dust or spores are much more deadly because the symptoms resemble a common cold...
In this paper I will describe gangrene and the different types, explain the risk factors and causes, discuss the complications, as well as treatment options and outcomes. I will also give details on limb amputation and how it affects the psychological well-being of the patient. By the end of this report I hope to have portrayed a different side of this tragic condition; the patient’s side.

The normal wound healing process mainly consists of four main stages being haemostasis, inflammation, proliferation or new tissue formation, and tissue remodeling or resolution. For a wound to heal well the above mentioned stages should occur in a sequential and orderly manner. Disturbances, abnormalities and delays in any of the above stages may lead to impaired healing or even chronic wounds. In adults, this process of normal healing takes place in the following steps (1)rapid haemostasis (2)appropriate inflammation (3)mesenchymal cell differentiation, proliferation, and migration to the wound site (4)suitable angiogenesis (5)prompt re-epithelialization and (6) proper synthesis, cross-linking, and alignment of collagen to provide strength to the healing tissue.
Necrotizing enterocolitis (NEC) is a life-threatening condition that manifests as inflammation of the intestines. This devastating disease affects mostly premature babies, who account for up to 8 out of every 10 cases. In NEC, bacteria invade the intestinal wall and the local infection and inflammation lead to destruction of the intestinal lining and/ or its entire thickness. A breach in the integrity of the bowel wall enables bacteria to escape into the abdominal cavity. By this means, the bacteria cause a massive infection, which has the ability to progress quickly and may lead to death if not treated immediately. Hence, NEC is a medical emergency.
When a wound is determined as non-healable, as described by Sibbald et al (1), it should not be treated with a moist treatment and should be kept dry in order to reduce the risk of infection that would compromise the limb. It is also important to consider the patient 's preferences and try to control his pain, his discomfort in activities of daily living and the odour that their wound may produce. In this case, special attention must be given to infection prevention and control. Some charcoal dressing would be interesting in the care of our non-healable wounds at St. Mary 's Hospital.
