Health changes overtime on every individual as they age. Since birth until older adulthood, health continues to alter in various forms depending on culture, lifestyle habits, and family genetics. One of the leadership skills of the nurse is to promote health from client to client. In order for the nurse to implement health strategies, the nurse must be aware of different cultures that affect the health due to diversified practices. From the nursing processes, the LPN must identify specific areas, formulate strategies, and bring attention not only to the client, but with the family, culture, and spiritual needs to meet the desired patient outcome: the achievement of possible high quality of health.

In accordance to the goals of health promotion by the World Health Organization, they perceive health promotion is “To improve equity in health, reduce health risks, promote healthy lifestyles and settings, and respond to the underlying determinants of health” (Organization, 2018). The general aspects that need improvement for this client will be (1) physical, (2) emotional, (3) mental, and lastly (4) spiritual. The health promotion for physical aspect will focus on Ms. Russo’s diagnoses: hypertension, mild heart attack, leg ulcers, and recovery from the last surgery. The physical self will require proper nutrition, appropriate weight, beneficial exercise and adequate rest (Garcia, 2018). The nurse should implement strategies to help the client on how to manage the blood pressure and blood glucose to at least stabilize the fluctuation despite the client’s age. To enforce the plan, the nurse should conduct a holistic physical assessment; especially with blood pressure and blood glucose level and analyze it from the baseline. Furthermore, the nurse can consult to the dietician, concerning about the appropriate diet and texture to regain the appetite and to avoid malnutrition. The value of appetite can seriously affect the food and fluid intake of the patient. The nurse can also collaborate with the client and the family about food choices based on their culture and other preferences. Furthermore, if the client recovered successfully, the nurse can start with other concerns by collaborating with the special wound nurse and physiotherapist for the leg ulcers and appropriate exercise the client can attend to improve blood circulation and cardiac output. Other suggested interventions about the physical aspect include proper wound and pain management for it can affect physical mobility and the sense of well being.
She really enjoys having someone correct her mistakes or strengthen her performance. If a dietician does not think she has a good meal plan, D.P. wants the meal plan to be adjusted properly. When she is out buying sweets, she wants her husband to say no. If her glucose levels are not where they should be she wants the nurse to tell her. But with all of the challenges that occur with D.P.’s diabetes, she appreciates the recommendations the most. Recently at Shadow Health General Hospital, this nurse came across a patient, we will call TJ, who had an infected, slow healing wound on her foot. TJ is 28 year old African American woman, 170 cm tall, 90 kg and body mass index (BMI) of 31, who was diagnosed with type 2 diabetes four years ago. It was evident from the onset of meeting TJ that she wasn’t a very compliant patient, in terms of self-assessing her blood sugar or taking her medication. Due to her lack of compliancy, she displayed very poor glycemic control, there was plenty of both subjective and objective data supporting this.
