The need for health promotion
Recently at Shadow Health General Hospital, this nurse came across a patient, we will call TJ, who had an infected, slow healing wound on her foot. TJ is 28 year old African American woman, 170 cm tall, 90 kg and body mass index (BMI) of 31, who was diagnosed with type 2 diabetes four years ago. It was evident from the onset of meeting TJ that she wasn’t a very compliant patient, in terms of self-assessing her blood sugar or taking her medication. Due to her lack of compliancy, she displayed very poor glycemic control, there was plenty of both subjective and objective data supporting this.
During her health history upon admission TJ provided ample amounts of subjective evidence that presented her failure to properly control her blood sugar levels. First and foremost, TJ admitted that she not only rarely checked her blood sugar but when she did the “numbers were always all over the place” and “she wasn’t really sure what they meant”. Secondly, she stated some symptoms that are common when blood sugar levels are out of control. TJ stated that of late she has been “really thirsty” and urinating more than normal. Polydipsia

Diabetes and uncontrolled sugar levels is now the seventh leading cause of death in the United States, and the number one cause of adult onset blindness, lower limb amputations and kidney failure (healthy people citation). Additionally, those diagnosed diabetes have an amplified risk of having a heart attack and an overall increased mortality rate, both of which are proportional to 1.8 times greater than those not afflicted with diabetes (Healthy people). Glucose is a sugar that plays a big part in a human’s health and well-being. This sugar is a major source of energy for the body’s brain and cells. The Cells that receive energy from glucose help in the building of the body’s muscle and tissue. Although glucose may be important to the body too much of this sugar can cause a chronic condition called Diabetes. Diabetes, also known as Diabetes mellitus, is a chronic condition that is caused by too much sugar in the blood. This condition can affect all age groups. In fact, in 2010 a survey was taken by the National Diabetes Information Clearinghouse, on the number of newly diagnosed diabetes. Out of 1,907,000 people: 24.38% were ages 20-44, 55.17% were ages 45-64, and 20.45% were ages 65 and greater. Diabetes is a very serious condition, and it can be deadly if left untreated. This paper will help better educate the reader on the signs and symptoms, the testing process, and the management of diabetes.

Possible benefits are if a patient eats his or her breakfast and his or her blood sugar returns to normal. However, there are applications in the computer where a diabetic educator is able to view when a patient had low blood sugar and it was not rechecked in a timely manner to ensure that the blood glucose returned to normal. Managers are often being talked to by diabetic educators, because nursing staff at times decide to use their own judgment when treating low blood sugars rather than following the hospital protocol, and then nurses are forced to explain their decisions. Thus, the importance of documenting and ensuring a patient’s blood glucose has returned to normal, even though the flow chart was not implemented, is often emphasized. D.P. receives an abundance of support in her life. She finds most of her motivation from her family because family means the world to her. The members in her family will encourage her to do her best along with attending diabetic information groups. She finds the support and motivation from her family to be a great way to help deal with her diabetes. Other family members who have diabetes or know someone else with diabetes helps her realize that this is not necessarily an ending of freedom or her life. The health care professionals also have an impact on D.P.’s motivational level as they give her options and techniques to improve her ways of living in order to cope with diabetes. The purpose of this study is to research the disease Type 2 Diabetes and to discover whether it is being effectively treated and prevented in Merced County, California. According to the National Institutes of Health, Diabetes Mellitus is “a lifelong (chronic) disease in which there are high levels of sugar (glucose) in the blood. It is also the most common form of Diabetes”.1 Furthermore, there are many risk and lifestyle factors associated with this disease, but the most prevalent are; obesity (#1 risk factor), sedentary lifestyle, unhealthy eating habits, family history and genetics, increased age, high blood pressure and high cholesterol, and a history of gestational diabetes.2 The long term complications of having type 2 diabetes can include but are not limited to; eye problems (cataracts and glaucoma), foot problems (neuropathy/ nerve damage), skin problems (infections), high blood pressure (which raises your risk for heart attack, stroke, eye problems and kidney disease), hearing loss, oral health, mental health and early death.3
The figure also shows that one in every eleven adults suffers from diabetes under the Sun. The facts and figures researched in this report clearly shows that diabetes has become one of the most common diseases, and we need more doctors and scholars who can make continuous and serious efforts to come up with a better medicine or treatment for diabetes.
Mrs. Jones was admitted to the hospital for evaluation due to hyperglycemia related to diabetes. Her blood sugar was 350 and her physical exam revealed dry skin and mucous membranes.
During one of my rotations, I was assigned a young adult patient who had run out of insulin and had been admitted to the hospital following a Diabetes Ketoacidosis (DKA) episode. I realized that my patient was probably torn between buying insulin and buying healthy food because her chart showed several admissions in the past following the same problems. This particular patient was in her room, isolated in a corner, and she was irritable. As her student nurse, I was actively involved in her care; I was her advocate for the day. The patient lived with her single mother and worked at a fast food restaurant. Since this was my first time dealing with a patient with DKA, it became a definite challenge for me.

Diabetes is a metabolic disease defined by high blood glucose concentration, also known as hyperglycemia (Mertig, 2012). Hyperglycemia is the result of having a problem with insulin release and/or a problem with insulin action. In other words, a person living with diabetes produces little to no insulin (type 1 diabetes) or does not have the ability to utilize efficiently the insulin produced (type 2 diabetes) (Mertig, 2012). Diabetes is a growing epidemic in the United States. In an effort to better manage and reduce the incidence of diabetes, researchers dedicate an enormous amount of time each year trying to gain a stronger understanding of the disease (Philis-Tsimikas and Decker, 2011). After all, the long term complications of uncontrolled diabetes (i.e. blindness, renal failure, heart disease, amputations, etc) can be devastating and needs to be prevented and/or controlled (Mertig, 2012). Individuals living with diabetes need to incorporate nutritional management, physical activity, compliance with medications, proper monitoring of blood sugars, self education and most importantly actively participate in their own diabetes care. According to Inzucchi et al, a patient centered approach is best and means, “Providing care that is respectful of and responsive to individual patient preferences, needs, and values and ensuring that patient values guide all clinical decisions.” An approach proven to be effective in managing diabetes is the diabetes self-management education (DSME) approach. DSME is a “patient centered” approach that actively involves the client in all aspects of their diabetes care and provides the necessary tools to encourage self-directed growth. In 2004, at the age of 49, my uncle was diagnosed with Type-2 diabetes, or late onset diabetes. He had been an unhealthy eater much of his life. At the time of his diagnosis, he weighed two-hundred forty-seven pounds. That may not sound like a lot, but at five-foot two-inches tall, he was ninety pounds heavier than average male of his height should be. Although, possible in the future, he was spared from daily injections of insulin. However, his battle with a new enemy was only beginning. In lecturing him on better eating habits, his doctor indirectly blamed sugary foods such as soda for his weight and overall health. Not knowing any better, my uncle took what he was told as grave warning. For the first time, he was forced to change what he eats. His life-long relationship with fast food and casino buffets were over, and in their place came a new and healthier diet. He is not alone, according to the Center for Disease Control and Prevention estimates that there are over twenty-six million people living with diabetes, seven million of that the CDC believes are undiagnosed cases. For my uncle’s age group, 40-49, the percentage of people with diabetes increased by fifty-five percent between 1990 and 1999. Furthermore, there were over one-million new cases of diagnosed cases for his age group, in 2010. (National Database 1-12). Health promotion is the process of improving health status of a person and prevention of disease by enabling the person to take control of their health. It is not just the absence of disease (Maben, & Clark, 1995). Health promotion is commonly used term in health care world, and in current society the promotion of health has greater significance, especially with the rise in consumerism. Health promotion is a vital concept for nursing, symbolizing notions that nursing is related today. Nurses are being urged to take a health promotion role, and are deemed by others as an ideal role for them. It is therefore crucial that nurses cognize the meaning of health promotion and also what is expected from them by undertaking this

In conclusion, diabetes is a serious disease and can be life threatening. With the right research and implementation of new studies diabetes can be reduced among the populations. We as a people need to realize this epidemic and all get together and beat it. Restaurants need to stop putting chemicals and unnecessary fats in their food. Not only diabetics but everyone should educate themselves on what is going in your body when you eat. Also, exercising and getting off the couch, in children, needs to be addressed and that alone would help reduce diabetes in the younger population. I can’t stress enough on education. If you know what you are eating or how you are exercising you protect yourself from poor health and live a great life. Type 2 diabetes can be dwindled down with a conscious individual and group effort.
Mrs. Anna suffered from a two medical conditions: hypertension (HTN) since10 years which were diagnosed in her postpartum period and recent high blood glucose level which suggest a type-2 diabetes mellitus (T2DM).
