Define a critical thinking task that your staff does frequently (Examples: treat high blood sugar, address low blood pressure, pain management, treating fever etc.). Create a concept map or flow chart of the critical thinking process nurses should take to determining the correct intervention. A critical thinking task that staff on our unit are frequently challenged with is treating low blood glucose levels, which are considered to be a blood glucose level less than 80 mg/dl. The flow chart that follows is what our diabetic educators would like to see being done by nurses to address low blood glucose levels.
Although the flow chart is straight forward and easy to follow, nurses often use nursing wisdom, which is based on their knowledge and experience (McGonigle & Mastrian, 2012). For example, rather than giving orange juice or one tube of glucose gel and rechecking a patient’s blood glucose in 15 minutes, nurses may wait for a breakfast tray to arrive and recheck a patient’s blood glucose after breakfast. Possible risks include a patient not eating his or her breakfast and their blood sugar continuing to drop. Possible benefits are a patient eats his or her breakfast and his or her blood sugar returns to normal. However, there are applications in the computer where a diabetic educator is able to view when a patient had a low blood sugar and it was not rechecked in a timely manner to ensure that the blood glucose returned to normal. Managers are often being talked to by diabetic educators, because nursing staff at times decide to use their own judgment when treating low blood sugars rather than following the hospital protocol and then nurses are forced to explain their decisions. Thus, the importance of documenting and ensuring a patient’s blood glucose has returned to normal, even though the flow chart was not implemented is often emphasized.

The method of ethical decision making which was developed by Dr. Cathryn A. Baird presented two components contained in all ethical decisions which are; The Four ethical Lenses and the 4+1 Decision process. The Four Ethical Lenses issue claims that different ethical theories and the means in which we tend to approach the situations which form part of our ethical traditions are looked at in four different perspectives. From each perspective there are different values on which to decide whether the action taken is either ethical or not and each lens also lays emphasis on determining whether the decision made is of ethical requirement. In the 4+1 Decision Process, people who are responsible for making final decisions in an organization do it using four specific decision making steps and eventually will end up with one extra decision which gives a chance to reflect. The 4+1 decision process allows the decision makers to give solutions when faced with complicated ethical issues (John Muir Institute for Environmental Studies, 2000).
The nurse must make sure the patient understands what the signs and symptoms are for hypoglycemia and hyperglycemia. They must be advised when to seek medical help or to call the doctor if they blood sugar levels either get too high or too low. They must also know to avoid any sugary drinks, and substitute sugar or sweetener, while also at the same time monitoring their salt intake.

Through centuries nurses were given the title “Caregivers”. Unlike some doctors, nurses actually care for their patients, not necessarily saying doctors do not; they both just have a different way of caring. Yes, doctors cure illnesses, but nurses are just as important because they help with the healing process. Most nurses can have the same exact education or knowledge as a medical physician but the only thing individuals see is a name tag with either the acronym CNA, LPN, R.N. and PH.D. Of course PH.D will get all the credit, seeing as how nurses do not exactly diagnose patients. A nurse could just become a doctor but there are different aspects of each title. Nurses take instructions from a higher administrator, which is sometimes a doctor. What needs to be known is doctors are not the only ones that stress and have rules to abide by. Nurses have ethical codes, daily ethical dilemmas, morals, and ridiculous distress, but some of these examples differ with country, state, and hospital. If nurses are capable, then they should be given the opportunity to make medical decisions or diagnosis in critical situations.
Since ethical dilemmas are not always easily answered through the use of The Code of Ethics, ethical decision-making models are effective tools that can assist nurses in dealing with ethical issues. Ethical decision-making models provide a framework for working through difficult choices. They seek to define the limits of what is morally acceptable and help clarify the guidelines for making those difficult decisions. Ethical decision making models assist nurses in analyzing situations by focusing on understanding the patient needs, need for responsiveness to circumstantial considerations, and recognizing the uniqueness of each situation.
Reflection and analysis of critical incidents is widely regarded as a valuable learning tool for nurses. Practice requires us to explore our actions and feelings and examine evidence-based literature, thus bridging the gap between theory and practice (Bailey 1995). It also affords us the opportunity to change our way of thinking or practicing, for when we reflect on an incident we can learn valuable lessons from what did and did not work. Deontology is an ethical theory concerned with duties and rights. The founder of deontological ethics was a German philosopher named Immanuel Kant. Kant’s deontological perspective implies people are sensitive to moral duties that require or prohibit certain behaviors, irrespective of the consequences (Tanner, Medin, & Iliev, 2008). The main focus of deontology is duty: deontology is derived from the Greek word deon, meaning duty.
